Discovering the value of nursing in high-technology environments: outcomes revisited.

The article argues that the current approach to specifying outcomes of nursing care in high-technology environments obscures rather than illuminates nursing's contributions to health care. Two nursing situations are used to illustrate a reframing of the idea of outcomes from the perspective and language of nursing as caring. Outcomes of nursing care are reconceptualized as value experienced within the nursing situation.

Reframing outcomes: enhancing personhood.

The concept of "enhancing personhood" is a key aspect of the theory of nursing as caring. It is tenuously, though importantly, related to the currently popular notion of outcomes of care. While the standard connotation of outcomes as discrete economic results or end products of nursing care is incongruent with nursing as caring, the ethical professional practice of nursing requires attention to value. This article presents a theoretically congruent reconceptualization of valuing of advanced practice nursing. It offers a model for reframing the concept of outcomes within the theoretical context of enhancing personhood.

The dark side of caring: challenges of caregiving.

1. Because of the subtleness of the progressin of Alzheimer's disease, caregivers found themselves gradually allocating increased amounts of time and energy to the care of their loved ones. 2. All of the relatives mentioned the need to share the experience either formally with support groups for caregivers, or informally with relatives and friends. 3. Nurses' support of caregivers may prevent them from viewing their loved one as an object and prevent some of the prevailing guilt expressed by participants in this study.

Aesthetic knowing grounded in an explicit conception of nursing.

This article presents an expanded perspective of aesthetic knowing in nursing grounded in the theory of nursing as caring. The authors highlight Carper's contributions to nursing, applauding the value of her work. However, a major limitation of Carper's work on aesthetic knowing is the failure to provide an explicit conception of nursing to guide the search for patterns and structure of nursing knowledge, thus the limited development of the aesthetic pattern of knowing in nursing. The authors propose that aesthetic knowing in nursing is the creating experience in the nursing situation, expression of the experience, and appreciation of it through encounter.

Story as link between nursing practice, ontology, epistemology.

This paper proposes the use of story as a method of organizing and communicating nursing knowledge which assures groundedness in the ontology of nursing. The nursing situation is viewed as the unit of nursing knowledge, and the nursing story is the re-creation or representation of that situation.

Caring in nursing: analysis of extant theory.

Caring in nursing as a substantive area of nursing science has been the focus of considerable scholarly effort. Based on the assumption that caring is the central concept in nursing and is uniquely known and expressed in nursing, this paper focuses on an analysis of major theoretical works related to the concept. Five categories of questions provide a framework for the analysis: ontological, anthropological, ontical, epistemological, and pedagogical.

Creating a caring environment: moral obligations in the role of dean.

Mayeroff (1971) states "from a loose stringing together of ideas a tight fabric emerges; ideas intertwine and tend to reinforce each other, making for a mutual deepening of meaning and gain in precision". The fabric throughout this discussion is nursing. Commitment, compassion, confidence, competence, and conscience represent the ideas that, when uniquely and beautifully woven together, create the aesthetic experience of caring in nursing. The dean of a caring-based nursing program is morally obligated to foster an environment that reflects a commitment to caring.

A method to label cigarette smoke with 99mTc.

A method is described to label with 99mTc the particulate matter in the smoke from commercially obtained cigarettes. A solution of technetium heptasulfide is applied to the paper wrapper of the tobacco rod and then dried. Smoke from a cigarette treated in this way contains measurable amounts of 99mTc in the particulate phase. In contrast, no measurable radioactivity is found in the non-particulate (gaseous) phase of smoke. This method yields a gamma-emitting cigarette smoke which will permit future observation of particulate intake, deposition and clearance in experimental animals and man.
