Fetal malnutrition--the price of upright posture?

The pattern of preterm fetal growth faltering, normally seen in man, differs from that observed in animals. This type of fetal growth cannot be considered as an adaptation to facilitate birth but is more likely to be due to rapid evolution and imperfect adaptation to the upright posture. The pattern of posture and physical activity during pregnancy may therefore be an important determinant of fetal growth. Differences in intrauterine nutrition existing between social groups, usually ascribed to variations of maternal diet and nutrition, may well result from different patterns of maternal activity in the weeks preceding birth.

Is acute watery diarrhoea an important cause of morbidity and mortality among rural Bangladeshi children?

To assess the relative importance of acute watery diarrhoea (AWD) and other types of acute diarrhoea as causes of morbidity and mortality among infants and 1-4 years old children, we examined 3 different data sources from the Matlab field project of the International Centre for Diarrhoeal Disease Research, Bangladesh. In infants, prevalence rates for AWD and non-watery diarrhoeas were similar. In children, prevalence of AWD was 1.8 times lower than prevalence of other acute diarrhoeas. In infants, admission rate to a diarrhoea hospital was 4.1 times higher for AWD than for other acute diarrhoeas (P less than 0.001). In children, admission rate was only 1.7 times higher for AWD than for acute diarrhoeas (P less than 0.001). Infant mortality was 1.7 times higher for AWD than for other acute diarrhoeas, but child mortality was 3 times lower for AWD. These data suggest that, while diarrhoeal disease control programmes should give more importance to oral rehydration therapy in infants, field management of the other types of acute diarrhoea should receive more emphasis in children.

[Infantile diarrhea in Senegal: epidemiologic survey in a suburb of Dakar].

Before the launching of a national program against diarrhoea in Senegal it was interesting to survey the exact impact of this disease on the under 5 children and the way the mothers and the community are dealing with it. We investigated a very crowded suburb of Dakar where we found that diarrhoea was the first cause of death among under five children. This investigation showed us that Oral Rehydration Therapy (ORT) is widely known by the approximately 1000 mothers interviewed, thanks to the gambian and senegalese broadcasts . However, only 1% of the mothers consider it as a useful and efficient treatment of diarrhoea. Expansive and inefficient medical treatments as well as traditional therapy are systematically preferred to ORT. We analyzed the reasons of that failure and we proposed a new strategy to generalize the use of ORT.
