Omeprazole vs. ranitidine in short-term treatment of Helicobacter pylori positive duodenal ulcer patients.

Forty-three patients with active duodenal ulcer and Helicobacter pylori positivity in gastric antrum were randomly assigned to either omeprazole treatment (20 mg once a day) or ranitidine treatment (300 mg once a day) for 28 days. Re-evaluation of the patients (clinical and endoscopic examination and assessments for H pylori detection) was repeated after 2 weeks and at the end of the treatment. Healing rates in the omeprazole group were 40% after 2 weeks and 90% after 4 weeks, in the ranitidine group ulcer healing was recorded in 20% of patients after 2 weeks and in 80% after 4 weeks. Differences between treatments at 2 and 4 weeks were not statistically significant. Clinical response (disappearance of ulcer-related symptoms) was better in the omeprazole group at 2 weeks (p less than 0.05) but not at 4 weeks. At the end of the trial H pylori positivity in gastric antrum disappeared in 95% of the patients treated with omeprazole and in 5% of the patients who received ranitidine (p less than 0.001). The results confirm the effectiveness of omeprazole in short-term treatment of duodenal ulcer and re-emphasize the powerful activity of the drug on H pylori infection.

Ultrasonographic study of the Wirsung duct caliber after meal.

The pancreatic duct or at least parts of this structure can be demonstrated today by sonography in 50-82% according to various authors. We have measured the caliber of the sonographically visualized pancreatic duct in 20 normal subjects after physiological stimulation with meal. The mean caliber of the duct markedly increased after meal. During dilatation a longer segment of duct is more clearly visualized. It is possible to document an increase of caliber of Wirsung duct "in vivo" as an evident sign of pancreatic secretion.

Cisapride and gastric emptying of a solid meal in dyspeptic diabetics without autonomic neuropathy and in healthy volunteers.

Gastric emptying was studied in 10 insulin-treated, long-standing, diabetic out-patients with upper gastrointestinal, dyspeptic symptoms. Autonomic neuropathy, mucosal lesions and chloropeptic hyposecretion were excluded. Gastric emptying of a labelled solid meal (99mTc-sulphur colloid-infiltrated chicken liver) was clearly delayed by comparison with normal subjects: the mean gastric emptying half-time was almost 5-times longer (245.6 vs 52.5 min), and the gastric emptying rate at 120 min was 75% slower. Cisapride 10 mg i.v. significantly accelerated both parameters, and placebo had no effect upon them. In conclusion, gastroparesis may be present in diabetics without autonomic neuropathy, and cisapride may improve gastric emptying in such patients.

[Gastric emptying of solids in patients with nonorganic dyspepsia].

The purpose of this work was to evaluate gastric emptying of a solid meal in 20 patients with functional dyspepsia and in 20 healthy controls. Gastric emptying time (T 1/2 = 188.45 +/- 93.79; p less than 0.001 - E.I. = 0.43 +/- 0.16; p less than 0.001) was significantly delayed in patients with functional dyspepsia compared with controls. The conclusion is drawn that either antral motor activity or antral-pyloric-duodenal motor activity is impaired in such patients.

Copper (II) complexes of aminoacids: gastric acid antisecretory activity in rats.

The effect of simple and mixed copper(II) complexes of L-tryptophan (trp) and L-phenylalanine (phe) on the gastric acid secretion was investigated in Shay rats. Graded doses (from 15 to 75 mg/kg) of [Cu(trp)2(H2O)], [Cu(phe)2(H2O)] and [Cu(trp)(phe)(H2O)], administered i.g., caused a progressive inhibition of gastric acid secretion. The antisecretory activity of the mixed complex, [Cu(trp)(phe)(H2O)], was significantly greater (ID50 = 33 +/- 3 mg/kg) than that of the simple complexes [Cu(trp)2(H2O)] (ID50 = 48 +/- 3 mg/kg) and [Cu(phe)2(H2O)] (ID50 = 43 +/- 3 mg/kg). At the experimental conditions of intragastric pH, the different biological activities were directly related to the percentage of neutral species present in solution as determined by means of potentiometric measurements (at 37 degrees, I = 0.15 mol dm-3 in NaClO4). A possible mechanism of action of the investigated compounds is discussed.

[Effects of graded doses of copper complexes with L-aminoacids on gastric acid secretion in the rat].

Gastric acid responses to intragastric administration of graded doses (14-30-45 mg/Kg) of copper (II) complexes with aminoacids (two simple complexes, Cu-Trp2 and Cu-Phe2, and on mixed complex, Cu-Trp-Phe) were investigated in Shay rats. The results (% inhibition of H+ mEq/4h) shown that graded doses of copper complexes caused a progressive inhibition on gastric acid secretion, and that percentage inhibition values were, for all complexes, in linear correlation with employed doses. A possible mechanism by which copper complexes affect gastric acid secretion is discussed.

Common bile duct caliber following cholecystectomy: a two-year sonographic survey.

The aim of this study was to evaluate the size of the common bile duct (CBD) in patients with gallstones, before and after cholecystectomy. Ten female patients (22-61 years) were examined in whom gallstones were shown by ultrasound. The caliber of CBD was measured before cholecystectomy and then 6, 12, 18, and 24 months after cholecystectomy. No significant change of CBD caliber in each interval prior to or following cholecystectomy was observed.

Sonographic study of postprandial gallbladder emptying and common bile duct changes in patients with diabetes or cholelithiasis.

The aim of our study was to evaluate postprandial emptying of the gallbladder and common bile duct (CBD) in 20 patients with gallstones and in 20 diabetic patients with or without autonomic neuropathy. Gallbladder fasting volume was significantly increased in patients with multiple stones. Diabetics with autonomic neuropathy showed a decreased gallbladder emptying rate. CBD size did not show any significant change in all groups examined.

Comparison between gastric emptying of digestible and indigestible solids in diabetic patients.

We measured simultaneously gastric emptying of digestible and indigestible solids in 10 normal subjects and in 14 insulin-requiring diabetic patients. Our results demonstrate that in both diabetics and in controls the gastric emptying of digestible and indigestible solids occurs during the same phase of gastric motor activity, i.e. during the post-prandial period. However, gastric emptying of both digestible and indigestible solids is delayed in diabetic patients compared to controls.
