[Kirschner wire transfixation of syndesmosis rupture--an alternative treatment of type B and C malleolar fractures].

After completing ORIF of the lateral malleolus, the standard technique for fixation of the syndesmosis involves placement of a 3.5 mm locking screw across the fibula to the tibia. Alternative there is a possibility to make the transfixation with two 1.6 mm Kirschner wires introduced obliquely across the distal tibiofibular syndesmosis. No early removing of the implant is necessary. This retrospective study was conducted on a total of 50 cases of Weber type B or C malleolar fractures with syndesmotic rupture between 1988 and 1996. In 45 patients (90%) there is no complication seen for the transfixation, but in five patients a Kirschner wire dislocation was observed. We were able to review 36 of these patients after a median follow-up of 8.3 years (range 5-12 years). The results were evaluated using objective, subjective and roentgenographic criteria. Subjective rating had 29 patients (81%) with very good or good results. Good radiological results were found in 29 patients (81%). Concluding of this results the Kirschner wires transfixation is a technical simple method with good or very good results.

[Prevention of thromboembolism in conservative ambulatory fracture treatment. Verification of an out-of-court FMH (Federation Mediation Helvetica) expert assessment].

In an extra judicial assessment for the Medical Assessment Centre of the FMH (Federation Mediation Helvetica) a clinical case of a female patient who had suffered from deep vein thrombosis was reviewed. The patient had been treated conservatively for a malleolar fracture and had not received any pharmaceutical thrombosis prophylaxis. The question of false treatment had to be discussed. The final conclusion of the judicial assessment is that the medical treatment with a physical thrombosis prophylaxis of the patient was correct. Due to the various controversial judgements found in the literature concerning the thrombosis prophylaxis, a certain insecurity remains for the treating physician. Taking into account the different risk factors, the indication for a prophylaxis must therefore still be decided on an individual basis.

[Long-term results following fibulo-tarsal ligament reconstruction. Comparison of the Watson-Jones and the (modified) Chrisman-Snook techniques].

The chronic lateral instability of the ankle joint presents a major problem after an acute tear of the lateral ankle ligaments. In this study we controlled 26 patients clinically and radiologically, who had undergone ankle ligamentoplasty (Watson-Jones and Chrisman-Snook (modified) techniques) between 1987 and 1991 because of chronic lateral ankle instability. The aim of this study was to evaluate if the more difficult Chrisman-Snook technique had better results. The patients had been suffering from a chronic lateral instability on the average of 6-7 years before surgery. The operations were performed by experienced surgeons. The follow-up-period was 5 years. The reduced postoperative ankle mobility revealed no clinically relevant difference between the two groups. Also, the complication rate were the same. The functional assessment showed good or excellent results in over 80% for both groups. Long-term clinical results of the two operations are similar and gave good or excellent results in more than 80% of the cases and are similar to other reconstructive techniques.

[Fatigue fracture: a frequently overlooked injury?].

From 1987 to 1989, 7272 Swiss infantry recruits were analysed for stress fractures in a retrospective study. In 33 patients 46 fractures were found. The average time of onset of the stress fractures was 4 weeks. Diagnosis at first medical consultation was correct only in 20%. Localization was calcaneus (23x), metatarsals (10x), tibia (6x), femoral neck (6x) and pelvis (1x). Immobilization was sufficient for therapy in all cases. None of them had operative procedures. Compared to 100 recruits without stress fractures, the patients had significantly more often foot pathologies, higher body length and were smokers. Physical fitness had no influence on the incidence of stress fractures.

How safe are ceramic heads as hip endoprostheses? A series of three head fractures within 3 months.

A report is given of a series of three ceramic head fractures involving articulation with polyethylene cups. No sure cause for the marked increase in occurrence could be found, as the heads were not from the same manufacturing lot, nor was there evidence of a material defect or an error in the implantation technique. Head fractures in the combination ceramic-polyethylene are reported very rarely because of the very low statistical probability. For this reason the present authors are continuing to use ceramic heads, but wish, in the interests of "informed consent," to draw attention to the risk of head fractures.

[Keller/Brandes surgical technique in the treatment of hallux valgus--indications, surgical method and results].

The Keller/Brandes operation for the correction of hallux valgus consists of a resection of the base of the first phalanx and narrowing of the forefoot by resection of the exostosis of the first metatarsal head and reduction of the luxated sesamoid complex, thus reducing the metatarsus primus varus. The indication to this procedure is given in the elder patient with arthritic changes in the first metatarsophalangeal joint. Though this operation is "mutilating", we have found favourable results in the recent literature as in 50 of our patients being very good and good in 82%, moderate in 8% and poor in 10% of the cases. In order to obtain a good result we emphasize the importance of a correct indication, adequate preoperative measures, a perfect operative technique and a well guided postoperative management.

[Legal guidelines for the prevention and control of animal salmonellosis].

Rules of legislation are an important instrument of the state leading to prevent and control the Salmonella-infections of animals. The legislation is not limited on Salmonella-infections of several animal species. It encloses also the veterinary inspection of animals ready for slaughter and their meat after slaughter, the hygiene of food and animal feed stuff, the recycling and/or disposal of waste and carcasses. An account is given in this paper about the several fields of legislation to prevent and control the Salmonella-infections.

[Girdlestone's hip resection. Clinical and electromyocinesigraphic study].

Forty-five patients with unilateral excision of the femoral head and neck and 2 patients with bilateral excision were studied with a follow-up of 6 months to 6 years. In 36 cases the hip excision was performed after a total hip prosthesis. We have personally monitored 22 patients. The others either died or suffered from other general diseases not related to the operation, making it impossible to check their locomotor apparatus. Two criteria were studied in particular: walking and pain. Fifty-five percent of the patients can walk well with one cane, 31% with two canes, and 14% require assistance to walk. Thirty-one can stand and walk without pain, 55% have some discomfort when fatigued, and 14% have constant pain. Electromyocinesigraphic examination was performed in 6 patients in order to study the automatic function of the muscles when the patient was standing and walking. We found the following: (1) no innervation of the hip abducters with high activity of the rectus anterior during the standing phase of the leg operated upon; (2) abnormal symmetrical and permanent activity of the erectores trunci during the usual standing posture and during walking; (3) increased activity of the hip abductors of the nonoperated leg when standing and walking.

Femoral neck fatigue fracture.

A report is presented on 25 fatigue fractures of the femoral neck in 20-year-old military recruits with an otherwise healthy skeleton. The problems of diagnosis are described. Principles for the treatment of these stress fractures are elaborated on the basis of pathophysiological considerations and of the clinical experience. A fracture in the radiological stage I (Endosteal callus formation) should be treated conservatively with immobilisation. In stage II (fissure at the calcar), and indication for surgery is relative, whereas in stage III (complete fracture of the femoral neck), a screw osteosynthesis should be carried out. Whereas the fatigue fractures in stages I and II usually heal without postoperative effects, complications are common in stage III, as in all femoral neck fractures.

[Statics and dynamics after total hip arthroplasty. An electromyographic study (author's transl)].

The weightbearing of the hip of 147 patients after total prosthesis operation was controlled by the two wages method. In most of them the weightbearing becomes normal in a few months after the operation. In 39 patients with 48 total prostheses an electromyographic examination of the erectores trunci and the glutaei as well as the quadriceps of both legs in standing and walking was performed. In standing the patients demonstrate an hyperactivity of the paravertebral muscles and glutaei before as well as after the operation. They cannot find the normal standing posture with its typical lack of activity of the back, pelvis or leg muscles. In standing on one leg as well as in walking the innervation of the glutaei is diminished. In many cases it becomes normal after the total hip operation. Only in 1/3 of the cases we can observe a fully normal electrical activity of the glutaei with the classical "fan sign" which corresponds to the normal rotation of the pelvis during the stand phase. The erectores trunci demonstrate before and after the operation the same anomalies of innervation.
