[Characteristics and prognosis of retinal detachment following uncomplicated extracapsular extraction of the crystalline lens].

In a retrospective study dealing with 52 eyes, we compare the features and prognosis of retinal detachment following intracapsular cataract extraction, non complicated extracapsular cataract extraction, extracapsular cataract extraction followed by YAG laser posterior capsulotomy, extracapsular cataract extraction complicated by peroperative rupture of the posterior capsule with phakic retinal detachment. Our results show several common features between retinal detachment after non complicated cataract extraction and phakic retinal detachment. Cataract extraction, no matter the technique, is not per se an anatomic or functional prognosis factor for a subsequent retinal detachment.

[Simplified treatment of retinal detachment: pneumatic retinopexy].

The pneumatic retinopexy is a simplified technique for the rhegmatogenous retinal detachment surgical repair. We describe the procedure, indications, contra-indications, results and complications. We conclude that this method is safe and effective provided the indications are well selected.

[Spontaneous evolution of macular holes].

We followed up 13 patients with a macular hole. The hole is unilateral in every case and idiopathic in 10 cases. After a mean follow-up of 27 months, the visual acuity is unchanged. On account of our observations and those of several authors, we conclude that there is no justification for treating the idiopathic macular hole nowadays. We recommend frequent examinations of the fellow eye to look for an impending macular hole, which could be benefit from a surgical treatment.

Excimer laser trephination in penetrating keratoplasty. Morphologic features and wound healing.

The imprecision of trephination of donor and recipient corneas is a major factor in post-keratoplasty astigmatism. In order to improve the quality of trephination, the authors developed a rotating slit delivery system for noncontact penetrating keratoplasty trephination using the excimer laser at 193 nm. Scanning electron microscopy (SEM), transmission electron microscopy (TEM), and light microscopy (LM) demonstrated the superior quality of excimer-cut buttons and recipient beds as compared with those obtained by free hand and suction trephines in human cadaver and rabbit eyes. The laser trephined more regularly and precisely without distortion of corneal topography and with less damage to adjacent corneal tissue. The authors morphologically examined wound healing at 6 hours, 12 hours, 3 days, 5 days, 2 weeks, 2 months, and 3 months after penetrating keratoplasty with laser and mechanical trephination in an animal autograft model. The laser did not cause any adverse alteration of wound healing processes including cellular migration, proliferation, and production of new tissue.

[Familial adenomatous polyposis: what is new for the clinician?].

Familial adenomatous polyposis (FAP) is a rare autosomal-dominant disease characterized by the development of more than 100 colorectal adenomatous polyps in young adults. In the absence of surgical intervention, colorectal cancer ineluctably develops in all affected patients. Recent progress in the isolation of the gene responsible for the disease allows to detect gene carriers before they present with symptoms attributable to polyps. Moreover, the presence of four or more lesions of congenital hypertrophy of the retinal pigment epithelium is an extracolonic manifestation of FAP allowing presymptomatic screening of this disease. An effective screening programme combined with the elaboration of a registry for FAP and prophylactic colectomy should reduce mortality related to colorectal cancer. Two other extracolonic manifestations of FAP remain major causes of death: abdominal desmoid tumors and duodenal adenocarcinoma. At this time, no effective medical or surgical therapy has been found to cure these lesions. Restorative proctocolectomy with ileal reservoir is another major advance. This procedure is now regarded as the treatment of choice for patients with FAP because radical removal of all premalignant colorectal mucosa eliminates the risk of subsequent development of a colorectal adenocarcinoma.

[Trephination using an Excimer laser].

We compared histologically and ultrastructurally donor buttons and recipient beds trephined mechanically with those trephined by the Excimer laser at 193 nm. The laser allowed for superior cutting precision and perfect centering. This non-contact method obviated the need for any pressure or suction during trephination, thereby eliminating problems associated with distortion of corneal topography. We studied wound healing after penetrating keratoplasty with mechanical and laser trephination in an animal model. The laser did not adversely affect wound healing processes.
