The wrist is frequently involved in the course of inflammatory rheumatism. The clinical and radiological features of the arthritis may guide the diagnosis when wrist involvement is isolated. The rheumatoid wrist may associate articular and tendon sheath synovitis, nerve compressions, muscle atrophy and deformities. X-rays reveal increased volume of the soft tissues, followed by cartilaginous destruction. Magnetic resonance imaging may detect the lesions early in their course. RS3PE, rheumatoid arthritis of the elderly, never induces destructive lesions. Still's disease is distinguished from rheumatoid arthritis by the predominant involvement of the radiocarpal and intercarpal joints with relative sparing of the metacarpo-phalangeal and proximal interphalangeal joints. Jaccoud's hand may be observed in the course of lupus with metacarpo-phalangeal dislocation of capsulo-ligamentous origin without cartilaginous destruction. Wrist involvement is often asymmetrical in ankylosing spondylitis. In psoriatic rheumatism, arthritis of the wrist is similar to that observed in rheumatoid arthritis, but demineralization is less common and occurs later and constructive lesions are associated with pinching.
A woman, who had intensively used an abrasive cleansing powder rich in silica for 12 years, developed a connective tissue disease consisting of destructive inflammatory oligoarthritis, Gougerot-Sjögren's syndrome, Raynaud's syndrome, anti-nuclear antibodies and hypocomplementemia. Another woman, who had sprayed silicone-glaze on cables for 15 years, suffered from edematous scleroderma. A man, who had sprayed the same silicone-glaze on cables over a 20 year period, had a subluxing arthropathy. These autoimmune syndromes were compared with diseases classically induced by exposure to silica and silicone. To the best of our knowledge, these means of exposure have not been reported previously and merit being known in the aim of prevention.

We describe a case of spondylodiscitis caused by Campylobacter fetus in a previously healthy patient. This is the first report of this agent causing discovertebral infection, which generated cellular immune deficiency, reversible with the cure.
After 5, 16 and 21 years of occupational exposure to silica, 2 men and 1 woman, respectively, were diagnosed as having dermatopolymyositis. Pulmonary impairment was always present and took the form of diffuse interstitial fibrosis in 2 patients, whose lung biopsies revealed high particulate silica contents. The role of silica in the induction of dermatopolymyositis is discussed in light of published information on silica-induced connective tissue diseases.
We describe avascular necrosis at multiple sites in 2 young adults. This is a condition rarely seen except in hypercortisonism or hemoglobinopathies. In one case, it occurred in the first trimester of pregnancy. No obvious cause of osteonecrosis could be found but both patients were infected by the HIV. The role of the latter in the bone necrosis is questionable.

The authors briefly remind of the broad outline of their therapeutic approach regarding rheumatoid polyarthritis, specifying their attitude towards the main medications currently available. They mainly stress the mistakes too often mad in the long-term treatment of the disease.
The authors report two cases of muscular tuberculosis in patients with dermatomyositis treated with steroids. Antituberculous therapy with Rifampicin led to a severe relapse of the dermatomyositis in both cases, suggesting enzymatic induction inhibiting the action of the steroids.

After reviewing the history of the discovery of the action of D-penicillamine in rheumatoid arthritis, the authors define the practical modalities for the management of treatment based on their own experience and on the data from the literature. The therapeutic score remains relatively low because of the frequency of side effects and therapeutic escapes, even when an encouraging result has been obtained during the first years of treatment.
