Small mammals naturally infected with Schistosoma mansoni were studied in the valley of the Paraíba do Sul river (São Paulo State, Brazil). 192 animals of 12 species were examined post mortem. Cavia aperea, Holochilus brasiliensis leucogaster, Nectomys squamipes squamipes, Oryzomys nigripes eliurus and Zygodontomys brachyurus were found infected. Most schistosome eggs found in the faeces were mature but no eggs were found in the faeces of C. aperea although adults were recovered. In the other infected animals the number of eggs per gram of faeces showed a marked daily variation. Perfusion of the portal system demonstrated that in H. b. leucogaster worms usually remained in the mesenteric veins. In C. aperea 75.7% of worms were found in the portal and intra-hepatic veins. H. b. leucogaster seems to be the rodent most likely to play a role in the epidemiology of schistosomiasis mansoni in the valley of the Paraíba do Sul river.
The usefulness of selective transplenic decompression of oesophageal varices by distal splenorenal shunt and splenocaval shunt was evaluated in the control of gastrointestinal haemorrhage in patients with portal hypertension of varied aetiology. (Decompression was successful in 69 out of 78 cases.) It was shown that it is superior to total portosystemic shunts, as the incidence of encephalopathy was very low compared with the data from our series of portocaval shunts. The operative mortality has been progressively lowered and has now reached levels comparable with portocaval shunt. Distal splenorenal shunt when performed as an emergency procedure to arrest bleeding has limited usefulness but when performed as an elective or prophylactic procedure its results are comparable with those of portocaval shunt without the untoward complications such as encephalopathy. A modified selective decompression of varices has been described in which the distal end of the splenic vein is anastomosed to the inferior vena cava. Though no long term follow-up studies are available, we believe that this shunt is likely to prove superior to distal splenorenal shunt as it has both the advantages of the distal splenoral and the haemodynamic advantage of end-to-side portocaval shunt. We conclude that in patients with portal hypertension of varied aetiology, who have not had a haemorrhagic episode but in whom varices have been demonstrated or who have had one episode of haemorrhage from varices, the splenocaval shunt when feasible or the distal splenorenal shunt offers the optimal method of management at present in India.

Painful shoulders are a frequent pathology and its specific diagnosis and therapeutic control are common challenges to medical professionals. To solve these problems a non invasive morphologic method of study--Real Time Ultrasonography--is presented. A standardized routine ultrasonographic technique using static and dynamic imaging was applied to the analysis of fifty patients whose clinical data suggested soft tissue pathology of that joint. Initial sonographic diagnostic data were compared with clinical signs and x-ray information and in some cases arthrographic and surgical observation as well. The utility of sonography in the therapeutic control was specially evaluated in a sub-group of twenty patients with clinical and ultrasonographic evidence of tendinitis or bursitis at that location and submitted to physiatric treatment. Sonographic criteria established in terms of echogenicity, dimensions and dynamic scanning performance are described and its value as diagnostic and therapeutic control tools are discussed. Our results suggest that real time ultrasonography is a useful method to confirm the diagnosis and could be indispensable to follow and control the therapeutic results in Painful Shoulders.
