Role of hyperthermic perfusion in the treatment of limb osteogenic sarcoma.

The role of hyperthermic perfusion (HP) for the treatment of limb osteogenic sarcoma was evaluated in 76 patients. Twelve patients were treated with HP alone, and a 5- and 10-year survival rate of 25.5% was obtained. When amputation was systematically associated to HP (12 patients), a 5- and 10-year survival rate of 50.9% was achieved. Twenty-seven patients were treated with hyperthermic antiblastic perfusion (HAP) followed by amputation 4 weeks later. With this treatment, the 5- and 10-year actuarial rates rose to 71.4 and 65%, respectively, with a 5-year disease-free survival rate of 67.4%. Fifteen patients were treated with HAP followed by an 'en bloc' resection and bone reconstruction. The 5- and 10-year actuarial survival rate obtained was 63.5% with a 5-year disease-free survival rate of 53.6%. These results seem to indicate that HAP permits carrying out conservative rather than ablative surgery for the treatment of limb osteogenic sarcoma.

Diagnostic approach to hepatic hemangiomas detected by ultrasound.

Eighty cases of hepatic hemangioma were studied using ultrasound. In one group consisting of 28 subjects a final diagnosis of hepatic hemangioma was supported by arteriography (21 cases) or surgery (7 cases). In the remaining 52 cases, the diagnosis was uncertain and the normal clinical and biochemical findings with the ultrasound follow-up studies at intervals of 3, 6 and 12 months, made a diagnosis of hemangioma highly probable. Twenty-nine cases (9 cases of the first group and 20 of the second) were also evaluated by Tc-99m colloid and in vivo Tc-99m-labelled red blood cell scintigraphy. On the basis of ultrasound appearance and internal structure, hemangiomas may be divided into three groups: hyperechoic pattern (of which there were 16 cases in our study), cystic or anechoic pattern (5 cases), and complex pattern (7 cases). Fifty-two cases of uncertain diagnosis showed hyperechoic focal lesions with rounded, well-defined margins and no clinical or biological abnormalities. Differentiation from malignant forms must be approached according to the specific ultrasound pattern observed and the presence or absence of symptoms. Only in cases of hyperechoic, well-defined lesions detected in asymptomatic patients may a reliable diagnosis of hemangioma be made. The use of in vivo Tc-99m-labeled red blood cell scintigraphy (Tc-99m RBC scintigraphy) is useful in hyperechoic and cystic forms having a diameter greater than 3 cm. Complex forms invariably require additional studies, using complementary procedures (angiography, angio-CT) to confirm diagnosis.

[Effect of sulodexide on hemorheological parameters in a group of patients with peripheral arteriosclerotic vascular disease].

The aim of this study was the evaluation of some hemorheological and metabolic parameters in a group of patients affected by peripheral artery disease treated with sulodexide. A double-blind with cross-over treatment was used. The results demonstrate that sulodexide reduces blood viscosity and triglyceride levels. This action may have a role in order to modify clinical symptoms and evolution of atherosclerosis.

Calcitonin increases peripheral plasma somatostatin-like immunoreactivity levels in humans.

Even though the inhibitory effects of CT on both hormone secretion and gastrointestinal functions have been well established, the exact mechanism of action still remains unclear. Since the effects of CT can be reproduced by somatostatin, we studied in man the effect of SCT on peripheral plasma SLI levels. Immediately after the onset of CT infusion SLI rose from its mean basal value of 45 +/- 5.5 pg/ml to a peak value of 91 +/- 11 pg/ml (p less than 0.005). SLI levels were still significantly elevated at 30 (p less than 0.05), 45 (p less than 0.05), 90 (p less than 0.005) and 120 min (p less than 0.02). Our results, in good agreement with the previous report by Chiba et al. on isolated perfused rat stomach, suggest that CT effects may, at least in part, be mediated by endogenous somatostatin release.

Effect of PGE2 and indomethacin on human fibroblast collagen production.

Fibroblasts can synthetize prostaglandins (particularly PGE2) "in vitro" but it still remains unclear what role they play in the regulation of fibroblast proliferation and collagen production. We report here the effect of PGE2 and indomethacin on collagen synthesis by cultured human dermal fibroblasts. PGE2 (range: 1-300 pmoles/ml) and indomethacin (range: 0.0025-1.0 micrograms/ml) did not significantly affect fibroblast collagen production, when added for 24 hours at 37 degrees C to the cultures, in comparison to controls (fibroblasts incubated for 24 hours at 37 degrees C in medium only). Prostaglandins probably modulate collagen synthesis, as described in a previous report, by means their effect on cell proliferation. It appears they do not affect the intracellular mechanism of collagen production.

Diagnosing metastases of differentiated thyroid carcinoma with 201 thallium scan: a case report.

A case of supraclavicular metastatic follicular carcinoma which occurred 7 years after the patient had undergone total thyroidectomy is presented. By means of whole body 210-thallium scan, a precise diagnosis was made, while the whole body 131-I scan was negative. The thyroglobulin serum levels were high before surgery but significantly decreased after lymphadenectomy. The clinical use of this tracer to detect functioning and non-functioning metastases of thyroid carcinomas is discussed.

[Blood hyperviscosity syndromes].

Hyperviscosity syndromes are characterized by altered blood flow properties; the rheological properties of blood depend from cellular or plasmatic factors. In this study we have evaluated the role of some high molecular weight proteins (IgG, IgA, IgM, fibrinogen) and of some triglyceride-rich lipoprotein fractions. In Waldenström's disease a significant increase of apparent blood viscosity and plasma viscosity was observed; erythrocyte filterability was reduced. In patients with IgG and IgA myeloma just a slight increase of viscosity was observed. As for lipoprotein role, patients with hypertriglyceridemia type IV and V of Fredrickson have shown a significant increase of plasma viscosity; in these hyperlipidemias triglyceride levels were significantly correlated with both apparent blood viscosity and plasma viscosity. No correlation was observed with erythrocyte filterability. These data suggest that serum proteins and high levels of triglycerides can modify blood rheological properties with both increased viscosity and reduced erythrocyte filterability.

[Correlation of metabolic and hemorrheological parameters in diabetes and hyperlipidemia].

Blood viscosity, plasma viscosity and erythrocyte filterability were studied in 46 diabetic and in 24 hyperlipidemic patients and were compared with those of a group of normal controls; 35 diabetics were type I (IDDM, 13 of whom complication-free and 22 with micro- and macroangiopathy), 11 diabetics were type II (NIDDM, 5 of whom complication-free and 6 with micro- and macroangiopathy). Erythrocyte filterability was lower in type I and II diabetes than in controls, and was also lower in diabetics with vascular complications than in complication-free patients. Erythrocyte filterability inversely correlated with HbA1c and with plasma uric acid levels. The hyperlipidemic group included 6 type IIa, 6 type IIb, 8 type IV and 4 type V patients. All hyperlipidemic patients were examined before and during diet and drug therapy. In all the hyperlipidemic patients blood viscosity inversely correlated with HDL-cholesterol, while plasma viscosity directly correlated with plasma uric acid. All these findings suggest that some hemorheological factors can play a role in the onset and progression of vascular complications of diabetes mellitus and hyperlipidemias, and that the hemorheological changes correlate with the severity of the metabolic abnormalities.

High resolution CT spinal scanning with Acta 0200 FS.

The diagnostic features of CT images of the spine are constantly influenced by some limitations of the method because of the peculiar configuration of the human spine. Recent generations of CT scanners greatly improve the results but with the second generation (Acta 0200 FS Pfizer Scanner) we have been able to develop a modified scan reconstruction method to maintain the quality of our spinal images, thanks to the very flexible software of our CT scanner.

Applications of dynamic-CT, or angio-CT in neuroradiology. A disappointing experience.

The authors expose their experience in neuroradiological applications of dynamic-CT (D-CT) or angio-CT. The examinations are performed by means of a G.E. 8.800 CT/T. A special software offers automatic analysis of the region of interest. Vascular and tumoral patients are examined trying to evaluate: (a) the blood flow in patients suffering from transient ischaemic attacks and (b) to estimate the possibilities of D-CT in the evaluation of tumoral vascularity and in the identification of characteristic curves for different kinds of tumors.
