Double-blind randomized trial of ACTH versus dexamethasone versus methylprednisolone in multiple sclerosis bouts. Clinical, cerebrospinal fluid and neurophysiological results.

30 patients with acute exacerbations of multiple sclerosis were treated by ACTH, dexamethasone or methylprednisolone in a double-blind randomized study. Clinical parameters were assessed; cerebrospinal fluid and neurophysiological parameters (visual- and brainstem-evoked potentials) were evaluated at the beginning and at the end of treatment. Dexamethasone was more effective than ACTH and 6-methylprednisolone in shortening bout duration. Neither CSF nor neurophysiological parameters were significantly affected by therapy.

Double blind controlled randomized study on azathioprine efficacy in multiple sclerosis. Preliminary results.

The preliminary results of a double blind controlled prospective randomized trial of Azathioprine for therapeutic efficacy in 38 patients with Multiple Sclerosis are reported. Progression of the disease is significantly reduced in Azathioprine-treated group in comparison to Placebo-treated patients, but this effect is reached only after 3 years of treatment (P less than 0.025) regardless of the clinical course and without affecting relapse rate.

Magnetic resonance imaging in multiple sclerosis: diagnostic value and clinical correlations.

101 patients with multiple sclerosis (MS) (53 definite, 22 probable, 8 possible and 18 suspected according to the McDonald and Halliday criteria) were investigated by magnetic resonance imaging (MRI), to evaluate its diagnostic sensitivity compared to other diagnostic parameters such as cerebrospinal fluid (CSF) findings and auditory brainstem responses (ABR). In addition, correlations between severity of lesions on MRI and clinical data (disability duration of the disease and progression rate) and CSF immunoglobulin abnormalities were evaluated. MRI detected multiple demyelinating lesions in 96% of definite, 77% of probable, 50% of possible and 56% of suspected MS. In the same groups CSF was positive for oligoclonal banding in 85%, 68%, 50% and 37% and ABR in 82%, 50%, 80% and 28% respectively. Severity of MRI was found to correlate with disability and duration of the disease, but not with progression rate. The presence of CSF oligoclonal bands and a raised and IgG index were usually associated with more extensive white matter involvement as evaluated by MRI.

Clinical experience with urokinase in intracoronary thrombolysis.

A prospective nonrandomized study of the thrombolytic efficacy and dose-response effect of a high-molecular-weight urokinase, administered into the coronary artery, was conducted in 63 patients with acute myocardial infarction. Urokinase was infused (up to 180 min) at rates of 2000, 4000, 6000, and 10,000 IU/min in four consecutive groups of patients within 184 +/- 70 min following onset of chest pain. Of 54 patients with complete occlusion of the infarct-related vessel, 48 (89%) exhibited complete reperfusion. In 9 patients with incomplete occlusion, the degree of coronary stenosis was reduced with concomitant improvement in antegrade flow. The median effective dosage requirement of urokinase to reperfuse 50% of the treated patients was 180,000 IU. A relationship between the four infusion regimens and successful reperfusion was not found. The time to reperfusion, however, ranging from 42 +/- 30 to 60 +/- 41 min, appeared to be dose dependent. The reocclusion rate at follow-up (10-14 days) was 18%. Ejection fraction improved (40 +/- 8 vs. 47 +/- 8%, p = 0.002) in patients with low pretreatment values and in those treated within 2 h of the onset of symptoms. In-hospital mortality was 9%. Hemorrhage requiring transfusion occurred in 8% of the patients. None of the patients had levels of circulating fibrinogen inferior to 100 mg/dl. We conclude that urokinase can induce timely coronary reperfusion in patients with evolving myocardial infarction, at moderate infusion rates, and with concomitant induction of an only mild systemic lytic state.

Prognostic value of cerebrospinal fluid electrophoresis in optic neuritis and suspected multiple sclerosis.

The prognostic significance of cerebrospinal fluid electrophoresis and IgG-index has been investigated in 44 patients, of whom 10 had optic neuritis and 34 suspected, possible or probable multiple sclerosis. The predictive value of CSF oligoclonal banding for future disease dissemination is very high in the suspected multiple sclerosis subgroup, where after a follow-up of 4.4 years, 7 out of 7 band-positive patients displayed further disease activity. The absence of CSF oligoclonal bands seems to carry a relatively benign short-term (follow-up 3.4 years) prognosis in suspected MS cases.

Antihypertensives. N-1H-Pyrrol-1-yl-3-pyridazinamines.

The hypothesis that the side effects of hydralazine, such as mutagenicity and lupus erythematosus like syndrome, might be due to the NHNH2 group prompted us to incorporate part of this moiety into a pyrrole ring. Therefore, we prepared a series of N-1H-pyrrol-1-yl-3-pyridazinamines and a limited number of N-1H-pyrrol-1-yl-1-phthalazinamines by reaction of 3-hydrazinopyridazines and 1-hydrazinophthalazines with gamma-diketones. Most of these compounds, especially in the pyridazine series, showed moderate to strong antihypertensive activity in spontaneously hypertensive rats. The decrease in blood pressure generally had a slow onset after either oral or intravenous administration. N-(2,5-Dimethyl-1H-pyrrol-1-yl)-6-(4-morpholinyl)-3-pyridazinamine hydrochloride (30) (MDL 899) showed no mutagenic activity in several tests and is now in clinical trials in patients.

Efficacy of intracoronary and intravenous urokinase in acute myocardial infarction.

The efficacy of intracoronary and intravenous urokinase was studied in 83 patients with acute evolving myocardial infarction. Urokinase was administered intracoronary in 48 patients with a success rate of 89% obtained after 47 +/- 32 minutes of infusion of a dose of 255.000 +/- 224.000 IU. In-hospital mortality in this group of patients was 10%. Severe arrhythmias were observed in 33% of the patients and the reocclusion rate at the re-study was 16%. Intravenous urokinase was administered as 200.000 IU bolus followed by 1.220.000 IU infusion in 21 patients. Angiography performed in this group of patients 48 hours after therapy showed a patency rate of 66%. A single intravenous bolus of 500.000 IU of urokinase was administered to 14 patients. At angiography all patients but one were found reperfused. The value of intravenous low-dose bolus injection of urokinase in acute myocardial infarction needs hower to be assessed with a properly designed clinical trial.

[Intracoronary thrombolysis].

Intracoronary Urokinase administration was attempted in 42 patients with acute myocardial infarction. One patient died before Urokinase infusion could be started, two patients during treatment. 34 patients (83%) had total occlusion of a coronary vessel, 7 patients (17%) had a subtotal occlusion. Urokinase was administered at a rate of 10.000 IU/min in 11 consecutive patients (Group I), 6000 IU/min in 11 patients (Group II), 4000 IU/min in 11 patients (Group III), 2000 IU/min in 8 patients (Group IV). Recanalization of the occluded vessel or amelioration of the subocclusion was obtained in 36 patients (88%). 35 patients were controlled 12 days after the procedure, with coronary-angiography and left ventriculography. Patency persisted in 31 patients and successful thrombolysis was associated with significant improvement of left ventricular ejection fraction in those patients with depressed ventricular function before Urokinase treatment.

Antero-lateral rotatory instability of the knee.

Acute or chronic antero-lateral capsulo-ligamentous laxity of the knee is not always detected on clinical examination and is therefore wrongly thought to be uncommon. However, even in moderately mild lesions, the functional deficit is often quite severe, particularly in athletes. After discussing the aetiopathogenesis and clinical features of this condition, the authors deal in detail with the surgical treatment, and describe the Andrews technique which they normally use. The long-term results in 15 acute and chronic cases treated by this method confirm the effectiveness of this procedure.

Intracoronary thrombolysis with urokinase in acute myocardial infarction. Effects on reperfusion and left ventricular wall motion.

Intracoronary Urokinase administration was attempted in 20 patients (mean age 56 years) during the early phase of an acute evolving myocardial infarction. One patient died before urokinase infusion could be started. Out of the remaining nineteen patients, eighteen had total occlusion of a coronary vessel; one patient, in cardiogenic shock, failed to show occluding thrombi of the vessel related to the ECG site of infarction, and died during the procedure. Urokinase was administered at a rate of 4000 IU/min in 7 consecutive patients (Group I), 6000 IU/min in 5 patients (Group II), and 10000 IU/min in 6 patients (Group III). Recanalization of the occluded vessel was obtained in 16 patients (88%). The reperfusion time was 52 +/- 33 min. No significant variation in reperfusion time was noted in the three groups of patients treated. The time elapsed between the onset of symptoms and the successful recanalization was in our patients 4.3 +/- 1.8 hours. Ventricular arrhythmias, which required treatment, appeared after reperfusion in five patients. No serious hemorrhagic complications were noted. In fourteen patients ventriculography was performed before and immediately after thrombolysis: 15 patients had the study repeated 12 days after the procedure. Patency persisted in 13 patients and successful thrombolysis was associated with significant improvement in left ventricular ejection fraction and segmental wall motion. Our data demonstrate that different doses of Urokinase can be safely and successfully used to produce intracoronary thrombolysis in the majority of patients with acute myocardial infarction.

Synthesis of three metabolites of 3-hydrazino-6-[bis-(2-hydroxyethyl)amino]pyridazine. A new rearrangement of 3-[1-methylethyliden)hydrazino] derivative.

The synthesis of three metabolites (I), (II), (III) , isolated from rat urine, of the hypotensive compound 3-hydrazino-6-[bis-(2-hydroxyethyl)amino]pyridazine dihydrochloride (IV), is described. The first metabolite, 3-[bis-(2-hydroxyethyl)amino]pyridazine (I) (Scheme 1) was obtained either by reaction of 6(3)-chloro-3(6)-[bis-(2-hydroxyethyl)amino]pyridazine-N-oxide (V) with hydrazine hydrate or by catalytic hydrogenation of 3-chloro-6-[bis(2-hydrxyethyl)amino]pyridazine (VI). 3-Methyl-6-[bis-(2-hydroxyethyl)amino]-2-triazolo [4,3-b]pyridazine (II) was prepared (Scheme 2) by reaction of (IV) with acetic anhydride and subsequent hydrolysis of the resutling 6-[bis-(2-acetyloxyethyl)amino]-s-triazolo[4,3-b]pyridazine (VII). The third metabolite, 3-[bis-(2-hydroxyethyl)amino]-6-isopropyloxypyridazine (III) Scheme 3), was synthesized from 3-[(1-methylethyliden)hydrazino]-6-[bis-(2-hydroxyethyl)amino]pyridazine (VIII) which in phosphate buffer (pH 7.38) or in sodium hydrogen carbonate solution, underwent a rearrangement of new type to give (III).

Treatment of a ruptured dissecting vertebral artery aneurysm with double stent placement: case report.

A ruptured dissecting right vertebral artery aneurysm was treated by means of double stent placement with two overlapping stents. Control angiography performed 3 d after stent placement revealed beginning aneurysmal thrombosis. Substantial reduction in aneurysmal size was observed after 4 wk, whereas total occlusion was observed after 3 mo. The reduced stent porosity caused by the overlapping stents, which result in significant hemodynamic changes inside the aneurysmal sac, may accelerate intraaneurysmal thrombosis and may be helpful in achieving a more rapid complete occlusion compared with that achieved by single stent placement.
