Clinical experience with polyurethane-covered gel-filled mammary prostheses.

We have used 104 low-profile polyurethane-covered gel-filled mammary prostheses in 54 patients. Only one patient has developed capsular contracture. Foreign-body reaction to polyurethane was clinically significant in only one patient. Other complications have been few and of the type that can accompany any type of mammary prosthesis. Of the 54 patients studied, 51 still have their implants in place and have soft, compressible breasts. The superior wrinkling and palpable implant edge that is occasionally present when the implant is placed subcutaneously has been of some concern to patients and can be eliminated by submuscular placement. The use of polyurethane-covered low-profile gel-filled prostheses for breast reconstruction has greatly improved our results, and the satisfaction of both surgeon and patient has been gratifying.

Breast

4'-Methylangelicin derivatives: a new group of highly photosensitizing monofunctional furocoumarins.

The photobiological properties of three new angelicin derivatives carrying a methyl-group in 4' position at the furanic ring have been studied. In double-irradiation experiments on E. coli cells, they appeared to behave as monofunctional reactives towards DNA, similarly to the other angelicin derivatives previously studied. In short-term experiments such as the studies on macromolecular synthesis in Ehrlich ascites cells, in which the assay is performed that after irradiation, before DNA repair an intervene significantly, 4'-methylangelicin derivatives produced an inhibition of DNA and RNA synthesis 10 times higher than angelicin, the parent compound, and about 3.5 times higher than 4,5'-dimethylangelicin and psoralen, all assumed as reference compounds. In long-term experiments such as the studies on tumor transmission to healthy mice by injection of sensitized Ehrlich ascites cells, or on the survival of E. coli cells, in which DNA repair is active, 4'-methylangelicin derivatives appeared to be several times more active than angelicin and 4,5'-dimethylangelicin, and sometimes also more active than psoralen, a well-known, effective and cross-linking furocoumarin. While, as already observed, angelicin and 4,5'-dimethylangelicin failed to induce erythema on guinea-pig skin, 4'-methylangelicin derivatives proved to be phototoxic; however in comparison with the cross-linking psoralen, much higher amounts of substance and radiation doses were required. The erythema produced by 4'-methylangelicin derivatives presents some typical features of cross-linking furocoumarins, such as a long latency period and insensitivity to the anti-inflammatory drugs.

Smoke inhalation study.

An aggressive approach to the treatment of patients sustaining combined thermal and inhalation injuries is presented. Early and repeated bronchoscopy for diagnosis and treatment has yielded improved results.

Evaluation of biological dressings.

The use of biological dressings in the treatment of burns is reviewed. Many theoretical advantages were initially proposed, but time and controlled studies have disproved some of them. Biological dressings are nevertheless helpful in relieving pain, protecting exposed vital structures, reducing exudative protein and erythrocyte loss and evaporative water loss, protecting granulation tissue, antibacterial activity, and testing a recipient bed's readiness for grafting. They probably offer no benefit in final wound debridement and primary dressings for partial thickness burns or split skin donor sites. There is no conclusive evidence that biological dressings alter epithelialization or the rate of formation of granulation tissue. A review of our last 100 cases at the Bothin Burn Center generally supports these conclusions with evidence of the superiority of homografts over heterografts.

Bandages

Xenon ventilation-perfusion lung scans. The early diagnosis of inhalation injury.

The use of xenon Xe 133 ventilation-perfusion lung scans for the early diagnosis of inhalation injury was evaluated in 67 patients with acute thermal burns. Study results were interpreted as normal if there was complete pulmonary clearance of the radioactive gas by 150 seconds. Thirty-two scans were normal, 32 abnormal, and three technically inadequate. There were three true false-positive study results and one false-negative study result. Good correlation was found between the scan results and various historical, physical, and laboratory values currently used to evaluate inhalation injury. The scans appeared to be the most sensitive method for the detection of early involvement, often being abnormal several days before the chest roentgenogram. Xenon lung scanning is a safe, easy, accurate, and sensitive method for the early diagnosis of inhalation injury and has important therapeutic and prognostic implications as well.

Distant migration of silicone gel from a ruptured breast implant. Case report.

A complication of augmentation mammaplasty is presented in which the bag of a gel-filled implant ruptured and the thin gel migrated through subcutaneous planes as far as the groin. We feel that when a rupture of a gel-filled prosthesis becomes evident, it should be replaced as soon as possible if such migration is to be prevented (particularly if the gel is of the thinner and less-cohesive variety).

Subcutaneous mastectomy data: a preliminary report.

This is a preliminary report on data accumulated in the first 12 months by the Subcutaneous Mastectomy Data Evaluation Center at Saint Francis Memorial Hospital in San Francisco, California. We present some statistics on 419 subcutaneous mastectomies performed by 105 plastic surgeons.

Adenocarcinoma

[Ultrasonics in diagnosis of the upper abdomen].

On the basis of seven cases of suspected liver pathology, an albeit modest contribution has been made to the significance and diagnostic possibilities of ultrasonographic investigation of the upper abdomen. In four cases, ultrasound findings were confirmed by scintigraphy and in three cases by necropsy and histological reports.

The incidence of obscure carcinoma in subcutaneous mastectomy. Results of a national survey.

A national survey of plastic surgeons has been conducted to determinr the incidence of occult breat carcinoma in patients having a subcutaneous mastectomy. The results are reported herein. We believe subcutaneous mastectomy is the most effective prophylactic procedure presently available that is acceptable to women who have a high risk of developing breast cancer.

Determination of fetal maturity and of development of the fetal lungs by means of combined methods.

The validity of the assessment of the fetal age and pulmonary maturity was evaluated on 369 patients at various stages of normal pregnancy and complicated by diabetes, toxemia and fetal anencephaly. Gestational maturity was assessed by ultrasonic measurement of the fetal biparietal diameter and by analysing constituents of amniotic fluid (creatinine, bilirubin, fat cells) while pulmonary maturity was assessed from the L/S ratio in the amniotic fluid. Each individual method has a wide margin of error, however, ultrasound cephalometry gave the best prediction for determination of fetal age. In 135 selected cases lung maturity was assessed by using two techniques, a thin layer chromatographic determination of the lecithin/sphingomyelin ratio in the amniotic fluid and a rapid semiquantitative test to determine the titre of surfactant which generates stable foam in presence of ethanol. The results obtained by the two methods correlated well and were reliable in predicting the respiratory distress syndrome. It appears that these methods make it possible to obtain greater margins of reliability. The role of fetal and maternal factors in the maturation of fetal lungs was also studied in several pathological conditions and it was noted that in many cases of maternal diabetes mellitus lung maturity was reached at a later age than in normal pregnancies. It is condluded that if termination of pregnancy is indicated, the physician should rely more on the assessment of lung maturity than on indicators of gestational age.
