A technical report reviewing the use of the 11-mm Gamma nail: interoperative femur fracture incidence.

Two years after the first large series on the Gamma nail, the authors analyze 119 surgical procedures for proximal femur fractures performed using the 11 mm Gamma nail. The study was performed by 24 surgeons (from six orthopedics departments) at different levels of learning, as in the previous study. The small-sized nail allows an easier surgical procedure and a manual introduction of the nail even in small femurs, as suggested by the previous experience. A careful operative technique and a 2 mm overreaming, which was almost always adopted, explains the decrease in interoperative and postoperative complications (evaluated in 88 cases), compared to the last case report published in 1991. No interoperative shaft fractures occurred in 119 nailings.

The importance of clinical semiotics and instrumental investigations in the diagnosis and surgical treatment of osteoid osteoma. A report of 6 cases.

The authors emphasize the importance of a complete examination, including history, symptomatology and instrumental tests in the diagnosis of osteoid osteoma (O.O.). This lesion is often difficult to diagnose, particularly at unusual sites. However, by the use of accurate radiography, stratigraphy, bone scan and CAT, a pre-operative accurate diagnosis is usually possible. The authors emphasize the importance of the least invasive surgical approach and the complete removal of the lesion with generous curettage. This is not always easy, as illustrated in the present study, which includes atypical sites, sometimes in delicate anatomical situations. They also illustrate a particular method used for removal.

Grice's operation for spastic flat foot.

The authors report 24 cases of severe spastic pes valgus in children affected by cerebral palsy, treated surgically by a technique derived from that of Grice - taking the transplant from the calcaneus itself instead of from the usual sites. They discuss the parameters usually considered in assessing the results and describe their own method of interpreting the measurements derived from the angles usually employed in the evaluation of pes valgus.

Vertebral compression syndrome in multiple exostoses (case report).

The authors present a case, the second in the world literature, of Spinal Compression Syndrome in Multiple Exostoses. The diagnosis, not revealed on standard radiographs of the vertebral column, was suspected on radiculography and confirmed only by oblique stratigraphic projection. These demonstrated bone formation on the right vertebral lamina of L4 encroaching considerably on the spinal canal. Surgical laminectomy was followed by complete remission of neurological symptoms.
