Thyroid associated components in serum during normal pregnancy.

Thyroxine-binding globulin (TBG(, thyroxine-binding pre-albumin (TBPA), thyroxine (T4), free-T4, triiodothyronine (T3), and thyroid-stimulating hormone (TSH) were studied throughout apparently normal pregnancy in 290 cases grouped in 2 week intervals. TBG increased to a plateau level reached in the 24th week and was hereafter unchanged until term. T4 showed an increase until the 16th week of pregnancy and levelled off to a constant level for the rest of the pregnancy. Free-T4 declined to almost subnormal values for the non-pregnancy state which was reached around the 20th week of pregnancy. T3 showed a slight and definite increase in the beginning of pregnancy and levelled off to a constant value after the 14th week. TSH increased towards the end of pregnancy also showing its maximum during the last 4 weeks. TBPA showed constant values throughout pregnancy. The results are discussed in view of the use of modern techniques and previously missing data in literature (TBPA).

Placental transfer of maternal anti-rabbit IgG causing falsely elevated TSH levels in neonates.

Two infants were found to have markedly increased TSH levels, 104 and 154 mU/l of plasma, respectively, in a routine screening programme for congenital hypothyroidism. THe recall limit used was 50 mU/l of plasma. On follow-up, both infants were clinically euthyroid and had normal serum T4 and T3. The elevated TSH levels were confirmed only with some commercial radioimmunoassay kits--but not with others. Similar results were obtained in TSH assays of samples from their mothers, who had no other biochemical or clinical evidence of thyroid dysfunction. Both mothers had intense contact with rabbits over long periods. The apparent TSH activity was found to be associated with the IgG fraction. It was neutralized by the addition of normal rabbit serum to the samples and was caused by antibodies to rabbit immunoglobulin. The activity was eliminated from the circulation of both infants with a half-life of approximately one month. Apparently, the heterophilic antibodies were of maternal origin and were transferred to the foetus via the placenta. Infants with so-called transient hyperthyrotropinaemia identified in screening programmes have to be reevaluated to exclude false TSH elevations of this type.

Herpes simplex encephalitis: A serological follow-up study. Synthesis of herpes simplex virus immunoglobulin M, A, and G antibodies and development of oligoclonal immunoglobulin G in the central nervous system.

A solid-phase radioimmunoassay method was used for the detection of herpes simplex virus (HSV) immunoglobulin M (IgM), IgA, and IgG antibodies within the central nervous system in 11 patients with acute HSV encephalitis. Serial cerebrospinal fluid (CSF) and serum specimens were sampled during the observation periods, extending up to 43 months after onset. The clinical diagnosis of HSV encephalitis was confirmed demonstrating virus or virus antigen in the central nervous system in four patients and with significant HSV antibodies in CSF in all the patients. In acute stage CSF HSV antibodies of a significant level were demonstrated in one of four samples taken on days 3--4 after onset, and in samples taken on days 6--8 in five of nine patients. CSF HSV antibodies of a significant and high level were detected in all samples taken from day 10 after onset. Intrathecal production of HSV IgM and IgA antibodies lasted from 7 weeks to 43 months during the observation periods. All patients had persistent intrathecal production of HSV IgG antibodies as well as of oligoclonal IgG during the total observation periods up to 43 months.

2 translocations, t(11;14) and t(1;6), in a patient with plasma cell leukaemia and 2 populations of plasma cells.

2 translocations, t(11;14) and t(1;6), were found in 24 out of 46 metaphases in bone marrow cells from an untreated patient with plasma cell leukaemia. The predominating cell population produced only kappa chains while a minute population produced IgG kappa. All serum Ig's were low and only minimal amounts of monoclonal IgG kappa were found in the serum and very small amounts of kappa light chains in the urine. Analyses of ours and 2 other reported patients indicated the possibility that the location of breakpoints on the chromosomes may be of etiologic importance for the type of light chain and perhaps for the class of heavy chain.

Use of electrophoresis in the identification and quantitation of antibiotics administered in combinations.

The investigation presents a method of electrophoretic separation of antibacterial drugs which are used in combinations in clinical medicine. Subsequent to electrophoresis in agarose gel, a microbiological assay was performed. This technique permitted the determination of the concentrations of beta-lactam antibiotics, rifampicin, and clindamycin in the presence of aminoglycosides. In therapeutic combinations of fusidic acid and clindamycin, the concentrations of each drug could be determined.

Immunologic investigation in children with recurrent pneumonia.

In vitro lymphocyte stimulation by phytohemagglutinin (PHA) and pokeweed mitogen (PWM) and tests for granulocyte function were performed in 12 children with recurrent pneumonia. Reduced lymphocyte stimulation was found at repeated tests in 2 children and transiently decreased lymphocyte stimulation in 6 more children. In 3 patients, reduced bactericidal capacity of granulocytes was demonstrated, and in 1 cast stimulation showed decreased NBT activity of granulocytes. One patient examined had immunoglobulin deficiency.

Transient appearance of oligoclonal immunoglobulins and measles virus antibodies in the cerebrospinal fluid in a case of acute measles encephalitis.

Acute measles encephalitis with severe sequelae in a 25-year-old man was studied. A transient appearance of oligoclonal IgG in cerebrospinal fluid and of intrathecally produced measles antibodies was found during 2 months after the onset of the disease. On the basis of this finding of local hyperimmunization it is proposed that in the case studied the measles virus infection may have been directly responsible for the disease process in the central nervous system.
