[Clinical study on the contraceptive effectiveness of trisiston from the Jenapharm firm].

Now triphasic contraceptive preparations are used widely. Some news low dose formulations are available on the market, among them Trisiston. The study was conducted to confirm clinically acceptance and effectiveness of Trisiston. There were 437 volunteers mostly multiparous, with a mean age of +/- 25 years who participated in a multi center study. They were taking pills for a 3 to 12 cycles. Cumulatively 4026 cycles were observed. The investigation was conducted according special protocol. At the admission a complete medical history was obtained and a general physical and gynecological examination was performed. All subject were controlled every 3rd month included laboratory tests (blood levels of Hb, WBC, RBC, ESR, carbohydrate metabolism, clotting factors, bilirubin, creatinine, protein, glucose, albumin, LDL, HDL). There was no case of pregnancy. Pearl index = 0. The Trisiston was well tolerated by most of women (80%). Some effects like headache, vertigo, mastalgia, spotting and breakthrough bleeding, loss of libido, premenstrual syndrome were observed in 20% patients, soma of our patients (20%) discontinued contraceptive.

Cholesterol palmitate in amniotic fluid: confirmation of a simple, rapid, inexpensive, and reliable indicator of fetal pulmonary maturity.

The cholesterol test was previously described and preliminary experience suggested it might be a rapid, inexpensive, and reliable indicator of fetal lung maturity. In this expanded series of 1342 patients delivered of infants within 72 hours of amniotic fluid analysis, the predictive value for mature test was 98.6%. Predictive value for immature test was 55.4%. For the phospholipid profile the predictive value was 97.0% for a mature test result, 31.2% for an immature result. These observations, coupled with its methodologic simplicity, make the cholesterol palmitate test a good first step in a cascade scheme of tests for fetal lung maturity and a valuable test in a facility where the phospholipid profile is not available 24 hours a day.

The author points to the need of further training gynecologists in subspecialties resulting from the progress in this basic branch of medicine. It is particularly important to organize further training in gynecological oncology, reproductive endocrinology, maternal-fetal medicine (perinatology) and gynaecological urology. Accepting training in subspecialties, preparing programs and tests of skills and knowledge will give better results in specialist therapy of patients.

[General proposals of the training programs in subspecialties in gynecology and obstetrics].

The article presents the outlines of training programs in subspecialties in gynecology and obstetrics as gynaecological oncology, reproductivity and gynaecological endocrinology, maternal-fetal medicine and gynaecological urology. The program should be discussed and evaluated by scientific gynaecological associations, the Centre for Postgraduate Training and approved of by the ministry.

Influence of head-out water immersion on plasma renin activity, aldosterone, vasopressin and blood pressure in late pregnancy toxaemia.

In 39 healthy pregnant women and 45 women with mild or moderate late pregnancy toxaemia (LPT) the influence of head-out water immersion on blood pressure, the renin-aldosterone system, vasopressin and plasma osmolality was examined. Water immersion induced a prompt and marked fall in systolic and diastolic blood pressure, which was significantly higher in LPT women. Simultaneously a significant decrease of plasma renin activity, aldosterone and vasopressin was noted both in healthy and toxaemic pregnant women. In contrast to healthy pregnant women in LPT haemodilution was not observed in the early phase of water immersion.
