Serine 257 phosphorylation regulates association of polyomavirus middle T antigen with 14-3-3 proteins.

Polyomavirus middle T antigen (MT) is phosphorylated on serine residues. Partial proteolytic mapping and Edman degradation identified serine 257 as a major site of phosphorylation. This was confirmed by site-directed mutagenesis. Isoelectric focusing of immunoprecipitated MT from transfected 293T cells showed that phosphorylation on wild-type MT occurred at near molar stoichiometry at S257. MT was previously shown to be associated with 14-3-3 proteins, which have been connected to cell cycle regulation and signaling. The association of 14-3-3 proteins with MT depended on the serine 257 phosphorylation site. This has been demonstrated by comparing wild-type and S257A mutant MTs expressed with transfected 293T cells or with Sf9 cells infected with recombinant baculoviruses. The 257 site is not critical for transformation of fibroblasts in vitro, since S257A and S257C mutant MTs retained the ability to form foci or colonies in agar. The tumor profile of a virus expressing S257C MT showed a striking deficiency in the induction of salivary gland tumors. The basis for this defect is uncertain. However, differences in activity for the wild type and mutant MT lacking the 14-3-3 binding site have been observed in transient reporter assays.

HIV, hepatitis B and sexual practices in the street-recruited injecting drug users of Calcutta: risk perception versus observed risks.

Injecting drug users (IDUs) were recruited from the streets of Calcutta to obtain a baseline biological and behavioural data on risk practices. One-fifth of them (mostly using buprenorphine) tested positive for hepatitis B surface antigen (HBsAg); 4% were reactive to serologic test for syphilis (VDRL: Venereal Disease Research Laboratory). Condom use was insignificant while 74% reported sex with female sex workers and 15% of male IDUs also reported having sex with men. Although, sharing of injecting equipment ('works') was perceived as dangerous by the IDUs, majority of them (90/103) reportedly shared it; cleaning of works before sharing was a concern for intravenous but not for intramuscular drug injecting. Half of the IDUs reported suffering ever from abscess; a proportion (12%) of which had had superadded attack of maggots in it. They were also found to be infected with HIV (1%, 95% CI 0.028-5.97%) at a low prevalence that prompted subsequent launching of needle syringe exchange programme, establishment of cleaning norms before sharing of works, cleaning of injecting site on the body and condom promotion.

Randomized placebo-controlled trial of 2,3-dimercaptosuccinic acid in therapy of chronic arsenicosis due to drinking arsenic-contaminated subsoil water.

INTRODUCTION: Chronic arsenic toxicity producing various clinical manifestations is currently epidemic in West Bengal, India, Bangladesh, and other regions of the world. Animal studies have indicated that 2,3-dimercaptosuccinic acid can be used as an oral chelating agent. A prospective, double-blind, randomized controlled trial was carried out to evaluate the efficacy and safety of 2,3-dimercaptosuccinic acid for chronic arsenicosis due to drinking arsenic-contaminated (> or = 50 micrograms/L) subsoil water in West Bengal. METHOD: Twenty-one consecutive patients with chronic arsenicosis were individually randomized (random number; assignment made by individual not evaluating patients) into 2 groups: 11 patients (10 male, age 25.5 +/- 8 years) received 2,3-dimercaptosuccinic acid 1400 mg/d (1000 mg/m2) in the first week and 1050 mg/d (750 mg/m2) during the next 2 weeks with a repeat course 3 weeks later. The other 10 patients (all male, age 32.2 +/- 9.7 years) were given placebo capsules for the same schedule. The clinical features were evaluated by an objective scoring system before and after treatment. Routine investigations including liver function tests, arsenic concentrations in urine, hair, and nails, and skin biopsy evaluations were also completed. RESULTS: Though there was improvement in the clinical score of 2,3-dimercaptosuccinic acid-treated patients, similar improvement was observed in the placebo-treated group. There were no statistical differences in the clinical scores between the 2 groups at the beginning and at the end of treatment. Similarly, no differences were found for the other investigated parameters. CONCLUSION: Under the conditions of this study, 2,3-dimercaptosuccinic acid was not effective in producing any clinical or biochemical benefit or any histopathological improvement of skin lesions in patients with chronic arsenicosis.

Efficacy of topical nonsteroidal drops as pain relief after excimer laser photorefractive keratectomy.

PURPOSE: To compare the efficacy of topical aqueous indomethacin and ketorolac for pain relief following excimer laser photorefractive keratectomy (PRK). METHODS: We conducted a double masked, randomized trial involving 120 patients. All patients received a standard regimen of one of the trial drugs and chloramphenicol ointment to be used four times daily for 2 days. Pain levels and quality were assessed on a 10 cm linear analogue scale prior to each instillation of the topical medication with the McGill pain questionnaire. Pain threshold was evaluated using a modified State Trait Anxiety Inventory questionnaire. Patients were advised to use paracetomol for additional pain relief but were allowed to use any oral analgesic (except nonsteroidal agents) which they documented (type and quantity). RESULTS: There was no significant difference in pain scores between the groups using indomethacin and ketoralac. CONCLUSION: We found no significant difference between topical ketoralac and aqueous indomethacin as pain relief following excimer laser PRK. Both drugs seemed to significantly reduce pain.

Comparison of 18% ethanol and mechanical debridement for epithelial removal before photorefractive keratectomy.

PURPOSE: To assess the safety of alcohol compared to mechanical debridement for epithelial removal prior to photorefractive keratectomy (PRK). METHODS: Forty-one second eyes of 41 patients underwent epithelial removal using alcohol prior to PRK with a Nidek EC-5000 excimer laser. The results were compared with the results of the 41 first eyes of the same patients that had mechanical debridement. RESULTS: Initial results showed that the alcohol treated group tended to stay slightly hyperopic but had less haze compared to the mechanical debridement group. The treating surgeons favored alcohol as the preferred method of epithelium removal. CONCLUSION: Alcohol debridement is an effective procedure although algorithms may have to be altered to take into account the slightly different healing process postoperatively.

Satisfaction after photorefractive keratectomy.

PURPOSE: To evaluate patient satisfaction one year after photorefractive keratectomy (PRK) for myopia and myopia astigmatism using the Nidek EC-5000 excimer laser. METHODS: This study assessed the level of satisfaction of 6280 patients who underwent PRK after a minimum follow-up of 1 year. All patients were asked to enter their level of satisfaction as excellent, good, worthwhile, or disappointed into a networked computer database. Their responses were subsequently analyzed according to baseline and postoperative refractive data. RESULTS: Patients considered their results to be as follows: 82.3% good or excellent, 14.9% worthwhile, and 2.8% were disappointed with the outcome. CONCLUSION: A high percentage of patients were satisfied with the outcome following PRK. However, a fair number (14%) of patients considered the treatment to be worthwhile but it did not meet their expectations. Extensive counseling and inculcating realistic expectations before treatment is essential in achieving better patient satisfaction.

Photorefractive keratectomy for myopia with -8.00 diopter cylindrical correction.

PURPOSE: A colleague requested PRK. The last time the patient was seen, no spectacles or contact lenses were worn. However, subjective refraction was -7.00 -5.50 x 165 degrees. The original Nidek nomogram would have suggested setting the laser at the actual refraction. Our algorithm adjustments suggested a 40% extra correction of cylinder would be required. The laser was therefore set to -3.50 -8.00 x 165 degrees aiming for a -1.00 -2.00 x 165 degrees outcome to allow for possible retreatment. METHODS: A 165 micron depth ablation was performed. RESULTS AND CONCLUSION: At 6 weeks after surgery, visual acuity was 0.63 unaided with a refraction of +1.50 -1.75 x 170 degrees on pred forte.

The proofreading domain of Escherichia coli DNA polymerase I and other DNA and/or RNA exonuclease domains.

Prior sequence analysis studies have suggested that bacterial ribonuclease (RNase) Ds comprise a complete domain that is found also in Homo sapiens polymyositis-scleroderma overlap syndrome 100 kDa autoantigen and Werner syndrome protein. This RNase D 3'-->5' exoribonuclease domain was predicted to have a structure and mechanism of action similar to the 3'-->5' exodeoxyibonuclease (proofreading) domain of DNA polymerases. Here, hidden Markov model (HMM) and phylogenetic studies have been used to identify and characterise other sequences that may possess this exonuclease domain. Results indicate that it is also present in the RNase T family; Borrelia burgdorferi P93 protein, an immunodominant antigen in Lyme disease; bacteriophage T4 dexA and Escherichia coli exonuclease I, processive 3'-->5' exodeoxyribonucleases that degrade single-stranded DNA; Bacillus subtilis dinG, a probable helicase involved in DNA repair and possibly replication, and peptide synthase 1; Saccharomyces cerevisiae Pab1p-dependent poly(A) nuclease PAN2 subunit, required for shortening mRNA poly(A) tails; Caenorhabditis elegans and Mus musculus CAF1, transcription factor CCR4-associated factor 1; Xenopus laevis XPMC2, prevention of mitotic catastrophe in fission yeast; Drosophila melanogaster egalitarian, oocyte specification and axis determination, and exuperantia, establishment of oocyte polarity; H.sapiens HEM45, expressed in tumour cell lines and uterus and regulated by oestrogen; and 31 open reading frames including one in Methanococcus jannaschii . Examination of a multiple sequence alignment and two three-dimensional structures of proofreading domains has allowed definition of the core sequence, structural and functional elements of this exonuclease domain.

Statistical modeling and analysis of the LAGLIDADG family of site-specific endonucleases and identification of an intein that encodes a site-specific endonuclease of the HNH family.

The LAGLIDADG and HNH families of site-specific DNA endonucleases encoded by viruses, bacteriophages as well as archaeal, eucaryotic nuclear and organellar genomes are characterized by the sequence motifs 'LAGLIDADG' and 'HNH', respectively. These endonucleases have been shown to occur in different environments: LAGLIDADG endonucleases are found in inteins, archaeal and group I introns and as free standing open reading frames (ORFs); HNH endonucleases occur in group I and group II introns and as ORFs. Here, statistical models (hidden Markov models, HMMs) that encompass both the conserved motifs and more variable regions of these families have been created and employed to characterize known and potential new family members. A number of new, putative LAGLIDADG and HNH endonucleases have been identified including an intein-encoded HNH sequence. Analysis of an HMM-generated multiple alignment of 130 LAGLIDADG family members and the three-dimensional structure of the I- Cre I endonuclease has enabled definition of the core elements of the repeated domain (approximately 90 residues) that is present in this family of proteins. A conserved negatively charged residue is proposed to be involved in catalysis. Phylogenetic analysis of the two families indicates a lack of exchange of endonucleases between different mobile elements (environments) and between hosts from different phylogenetic kingdoms. However, there does appear to have been considerable exchange of endonuclease domains amongst elements of the same type. Such events are suggested to be important for the formation of elements of new specficity.

Conformational changes in plant Ins(1,4,5)P3 receptor on interaction with different myo-inositol trisphosphates and its effect on Ca2+ release from microsomal fraction and liposomes.

The interaction of the only reported plant inositol trisphosphate receptor with different myo-inositol trisphosphates (InsP3 species), namely Ins(1,4,5)P3, Ins(1,3,4)P3, Ins(1,5,6)P3, and Ins(2,4,5)P3, were studied to assess the extent of Ca2+ mobilization from microsomes/vacuoles as well as liposomes in vitro. Ins(1,4,5)P3 and Ins(2,4,5)P3 bind with the receptor with comparable affinities, as evidenced from their dissociation constants (Kd approx. 100 nM at 5 degrees C), whereas the interaction between Ins(1,3,4)P3/Ins(1,5,6)P3 and the receptor was not detected even with these ligands at 5 microM. Ins(1,3,4)P3/Ins(1,5,6)P3 isomers also do not elicit Ca2+ release from liposomes or microsomes/ vacuoles. The ability of any InsP3 to bind the receptor for Ins(1,4,5)P3 is a prime requirement for Ca2+ release. However, the comparison of binding affinities at a single temperature does not help to correlate it directly with the extent of Ca2+ release from the intracellular stores because the concentration of Ca2+ released by Ins(1,4,5)P3 as estimated over a period of 20 s is 3500 +/- 200 nM/mg of protein and is about 4-fold higher than that by Ins(2,4,5)P3 under identical conditions. To understand the role of the receptor conformation in Ca2+ release by different isomers, we have probed the conformational change of the receptor when the different isomers bind to it. Accessibility of the tryptophan residues in the free and Ins(1,4,5)P3/Ins(2,4,5)P3-bound receptor was monitored by a neutral fluorescence quencher, acrylamide. The resulting Stern-Volmer-type quenching plots of the internal fluorescence indicate a change in the conformation of the receptor on binding to Ins(1,4,5)P3 and Ins(2,4,5)P3. It is also detected when far-UV CD spectra (205-250 nm) of the free and ligand [Ins(1,4,5)P3/Ins(2,4,5)P3]-bound receptor are compared. The results from CD spectroscopic studies further indicate that the conformational changes induced by the two isomers are different in nature. When thermodynamic parameters, such as enthalpy (delta H), entropy (delta S) and free energy (delta G), for the formation of the two InsP3-receptor complexes are compared, a major difference in the extent of changes in enthalpy and entropy is noted. All these findings taken together support the proposition that it is the overall interaction leading to the requisite conformational change in the receptor that determines the potency of the InsP3 isomers in their abilities of Ca2+ mobilization from the intracellular stores or reconstituted liposomes.

Stiff-persons' syndrome associated with thymoma and subsequent myasthenia gravis.

We report the first case of stiff-persons' (-man) syndrome in the setting of a histologically proven thymoma. Muscular hyperactivity was abolished under general anesthesia and the symptoms of stiffness resolved after thymectomy and three courses of intravenous immunoglobulins. After thymectomy, the patient developed ocular myasthenia gravis which later resolved spontaneously. We suggest that thymoma be sought for in cases with neuromuscular hyperactivity syndromes. Myasthenia gravis may develop subsequently in these cases.

Thalamic thought disorder: on being "a bit addled".

Humans can generate and maintain relatively coherent trains of thought in natural discourse. The neural mediation of this ability and the phenomenology of its breakdown are not well understood. We report a case of a woman with paramedian thalamic strokes involving the mammillothalamic tract, intralaminar nuclei, parts of the dorsomedial and ventral lateral nuclei bilaterally. She presented with a dense amnesia and confusion typical of the syndrome of bilateral paramedian thalamic infarcts. Her Tc-99m HMPAO brain SPECT scan showed decreased thalamic and basal ganglia blood flow. General diminution of cerebral blood flow and areas of further diminution in the right frontal, left temporal and left temporoparietal regions were also observed. Although her amnesia was characteristic of diencephalic amnesia, her most striking clinical feature was a bizarre, disconnected and at times incoherent speech output. Analysis of her speech revealed relatively preserved lexical and morpho-syntactic linguistic production. By contrast, analysis of the macrostructure of her discourse revealed frequent unpredictable topic shifts that were completely unconstrained by contextual factors. Many of her shifts were intrusions from previous topics. We interpret her severely disordered speech output as representing the surface manifestations of a thought disorder (rather than as a language disorder per se) characterized by an inability to maintain and appropriately shift themes that normally guide discourse. Median and intralaminar thalamic nuclei appear to be critical for the neurophysiologic regulation of thalamocortical and striatocortical circuits, which in turn may be critical for the functional regulation of contextually appropriate transitions of thought.

Contributions of the left and right cerebral hemispheres to line bisection.

Thirty-one right-hemisphere lesioned (RHL) patients, 11 left-hemisphere lesioned patients (LHL) and 10 normal controls (NC) bisected lines in three spatial location and four directional cuing conditions. The error direction and error size were analyzed as separate and combined variables. Seventy-seven percent of RHL patients and 45% of LHL patients made abnormally large errors in line bisection. Right-hemisphere lesioned patients were more sensitive to spatial location and directional cuing than NC subjects. In contrast, LHL patients were less sensitive to either condition than NCs. The error direction and error size emerged as dissociable components of line bisection. Right-hemisphere lesioned patients and NC subjects bisected lines consistently to one side of the true center. Left-hemisphere lesioned patients bisected lines equally often on both sides of the true center. Both RHL and LHL patients made larger absolute bisection errors than NC subjects, but the RHL patient's errors were larger than those of the LHL patients. We propose that the greater sensitivity of RHL patients to spatial location and directional cues and the directional consistency of their bisection errors represent contributions of the intact left cerebral hemisphere to line bisection. In contrast, the LHL patient's unrestrained ability to orient to both ends of the line reflects a contribution of the intact right cerebral hemisphere to line bisection. The failure of both groups to accurately bisect lines reflects a common visuospatial processing deficit that is more pronounced following RHLs than LHLs.

Results of excimer laser retreatment of residual myopia after previous photorefractive keratectomy.

BACKGROUND: Nine percent to 30% of all patients who undergo a single excimer laser photorefractive keratectomy (PRK) do not achieve an unaided visual acuity of 20/ 40 or better and may require optical correction to obtain adequate vision. METHODS: The authors performed a retrospective analysis of the records of 164 patients who had undergone retreatment with the excimer laser for residual myopia after a previous PRK. Mean follow-up was 35.5 +/- 15.2 weeks (range, 26-104 weeks). RESULTS: The mean spherical equivalent (MSE) before retreatment was -2.59 +/- 1.36 diopter (D) (range, -0.50 to -7.75 D). The final MSE after reablation was -0.52 +/- 1.36 D (range, 2.50 to -5.50). Of the 164 patients, 107 (65.2%) obtained a final refraction within 1.00 D of emmetropia and 111 (67.3%) achieved an unaided visual acuity of 20/40 or better. Only 10 patients (6.1%) lost more than one Snellen line of best-corrected visual acuity. The final MSE result for the subgroup of patients who had a pre-retreatment myopia of between -0.50 and -1.90 D (-0.31 +/- 1.09 D) was significantly closer to emmetropia than that of the subgroup with a residual myopia of -4.00 to -7.75 D (-1.62 +/- 1.94 D). CONCLUSIONS: Excimer laser retreatment may provide a relatively safe and predictable method of correcting residual myopia after an earlier PRK with a 25% extra correction recommended for residual myopia.

Astigmatism induced by spherical photorefractive keratectomy corrections.

PURPOSE: The purpose of the study is to evaluate the induced astigmatism after spherical photorefractive keratectomy on the Summit Omnimed (Summit Instruments, Waltham, MA) and the Nidek EC-5000 (Nidek Co. Ltd, Aichi, Japan) excimer lasers. METHODS: A total of 4269 eyes of 3289 patients were treated with a 5-mm optical zone using the Summit Omnimed excimer laser and 1825 eyes of 1303 patients treated with the Nidek EC-5000 excimer laser. The final astigmatic refractive outcome was compared with the initial refraction by vector analysis (Alpin and Jaffe method). RESULTS: Subjective astigmatic refraction for the Summit laser reduced from a mean of -0.39 diopter (D) +/- standard deviation (SD) 0.33 D (range, 0 to -2.50 D) to -0.33 D +/- SD 0.41 D (range, 0 to -3.00 D). Surgically induced astigmatism (SIA) had a mean of 0.42 +/- SD 0.34 D (range, 0 to 2.89 D). Mean SIA increased with increasing preoperative astigmatism by 0.60 D SIA for every 1.00 D of preoperative cylinder. For the Nidek laser, subjective astigmatic refraction changed from a mean of -0.18 D +/- SD 0.21 D (range, 0 to -1.25 D) to -0.30 D +/- SD 0.33 D (range, 0 to -3.00 D). Surgically induced astigmatism had a mean of -0.32 D +/- SD 0.29 (range, 0 to 3.05 D). Mean SIA increased with increasing preoperative astigmatism by 0.47 D SIA for every 1.00 D of preoperative cylinder. CONCLUSIONS: The authors show that spherical photorefractive keratectomy corrections can induce significant astigmatic change, particularly if a large amount of preoperative astigmatism is present.
