[The presentation of viral hepatitis C infection without cryoglobulinemia with a peripheral neuropathy. Five case reports].

INTRODUCTION: Peripheral neuropathies are the most common neurological complication of viral hepatitis C infection with mixed cryoglobulinemia. CASES REPORT: We report five cases (three men, two women) of peripheral neuropathies revealing viral hepatitis C infection without cryoglobulinemia; the patients' mean age was 56 years. Paresthesias were the most frequent symptom. Electroneuromyographic examination found one case of polyneuropathy and four cases of multiplex mononeuropathies; the complement level was normal in all patients and the rheumatoid factor positive in two cases. Etiological investigations for peripheral neuropathy remained negative. Treatment and outcome were variable. DISCUSSION: Negative cryoglobulinemia in cases of VHC infection with neurological features has been described in the last few years, suggesting the possibility of other mechanisms such as direct action of the virus on the nervous system. There is no consensus on the treatment and outcome is variable. CONCLUSION: Peripheral neuropathy may reveal VHC infection, underscoring the need for VHC serology testing in etiological investigations for peripheral neuropathies.

[Thyroid tuberculosis].

OBJECTIVES: The aims of this study were to emphasize the difficulties of diagnosing thyroid tuberculosis because of its non specific symptoms and to encourage physicians to seek it more frequently. MATERIAL AND METHODS: Six cases of thyroid tuberculosis diagnosed within a 3-year period were included in a retrospective study. RESULTS: There were two men and four women, mean age 45 years. Four patients underwent lobo-isthmectomy with frozen section analysis which enabled us to rectify the diagnosis. Medical treatment was based on a 6-month course of bactericidal chemotherapy. Outcome was favorable in all the cases with 17 months average follow up. CONCLUSION: Symptoms of thyroid tuberculosis are misleading, pathologic findings are of increasing importance for diagnosis, which with the new serology techniques may improve further.

The localised aggressive periodontitis prevalence in Morocco.

This work concerns a retrospective study of prevalence records and the clinical characteristics of localised aggressive periodontitis in the breast of consultants in the periodontitis service in Dental Medicine Faculty of Rabat (Morocco) from 1997 to 1999.

Hydrothermal recharge and discharge across 50 km guided by seamounts on a young ridge flank.

Hydrothermal circulation within the sea floor, through lithosphere older than one million years (Myr), is responsible for 30% of the energy released from plate cooling, and for 70% of the global heat flow anomaly (the difference between observed thermal output and that predicted by conductive cooling models). Hydrothermal fluids remove significant amounts of heat from the oceanic lithosphere for plates typically up to about 65 Myr old. But in view of the relatively impermeable sediments that cover most ridge flanks, it has been difficult to explain how these fluids transport heat from the crust to the ocean. Here we present results of swath mapping, heat flow, geochemistry and seismic surveys from the young eastern flank of the Juan de Fuca ridge, which show that isolated basement outcrops penetrating through thick sediments guide hydrothermal discharge and recharge between sites separated by more than 50 km. Our analyses reveal distinct thermal patterns at the sea floor adjacent to recharging and discharging outcrops. We find that such a circulation through basement outcrops can be sustained in a setting of pressure differences and crustal properties as reported in independent observations and modelling studies.

[The status of carious disease in the Moroccan pediatric population (6-15 years of age)].

The objective of this study is to describe the teeth decay affect of Moroccan children. The works of reference regroup a sample of 3004 children from 6 to 15 years old. The results are the next: the teeth decay concern 3 children for 5, the DMFT is 2 (bad) and 2 children for 3 have cariogenic dental plaque which means a bad bucco-dental hygiene and a great risk of decay recurrence. One child for two hasn't a toothbrush that increases the bad bucco-dental status. It appears that there is a cause-response relationship between the rate of decay the rate of cariogenic dental plaque and missing bucco-dental hygiene and also exaggerated consumption of sugar-refineries.

[Tuberculous abscess of the liver. Apropos of 2 cases].

The authors report two cases of tuberculous liver abscess. They stress the rarity of this site, and differential diagnostic difficulties with other types of infection and liver tumors, notably amebiasis, hydatid cyst and carcinoma. Echo- or Scan-guided needle biopsy is useful for diagnosis and, in some cases, treatment by drainage of the purulent cavity.

[High incidence of sporadic non-A, non-B hepatitis in Morocco: epidemiologic study].

Non-A, Non-B hepatitis and their sequelae seem to be as frequent as HBV infections in Morocco. These diseases represent an important problem of public health because their high incidence and high fatal rate. Some aspects of the epidemiology of Non-A, Non B acute hepatitis were evoking a high incidence of enterically transmitted hepatitis E. That was confirmed by serum studies having shown that hepatitis E antibodies were detected in more than 60% of patients with acute Non-A, Non-B hepatitis. However this type of hepatitis has been recognized only as sporadic (non-epidemic), mainly transmitted by personal contacts in low hygiene conditions. Other Non-A, Non-B acute hepatitis (around 35%) were certainly due to hepatitis C virus infection, because the presence of hepatitis C antibodies in the serum of the patients. However, in our study, hepatitis C seemed to be rarely transmitted by transfusion or other blood related route. Chronic liver diseases related to Non-A, Non-B virus infection appeared to be as frequent as the ones due to hepatitis B virus. Serological studies had shown that about seventy-four per cent of the studied cases were related to an infection by hepatitis C virus (presence of hepatitis C antibodies). Among other Non-A, Non-B chronic liver diseases the possible existence of some cases due to hepatitis E virus infection cannot be ruled out but this hypothesis needs further investigations to be verified. The prevalence of the markers of past hepatitis B infection in convalescent patients from Non-A, Non-B hepatitis is comparable to the prevalence of hepatitis B infection markers in blood donors. However, chronic HBV infection could be a factor facilitating the clinical expression of the Non-A, Non-B hepatitis.

[Secondary tuberculous infection of a pilonidal sinus].

The authors report the case of a pilonidal sinus which was the site of active tuberculosis. They stress the rarity of this condition. Particular emphasis is placed upon the need for histopathological examination of all operative specimens resulting from excision of a perianal lesion.

Rioprostil in the short-term treatment of duodenal ulcer: a multicentre double-blind trial vs. cimetidine.

The efficacy and safety of the new prostaglandin E1 (PGE1) synthetic analogue, rioprostil, 300 micrograms b.d. and cimetidine, 400 mg b.d., on duodenal ulcer healing are compared in an international, multicentre, double-blind study. A total of 257 patients have entered the study; 243 are considered eligible for efficacy analysis and 207 for safety analysis. After 4 and 6 weeks of treatment, the endoscopic healing rates do not significantly differ between the two groups, being 55% and 83% respectively with rioprostil vs. 60% and 78% respectively with cimetidine. The major adverse effect attributable to rioprostil is diarrhoea, which was documented in 11% of patients compared with 1% of patients taking cimetidine. However, central nervous system complaints are twice as frequent in the cimetidine group. Monitoring of clinical laboratory tests show no significant abnormalities when compared with the baseline values during the administration of either drug. This study documents that rioprostil, at the dosage of 300 micrograms b.d., is as effective and safe as cimetidine in the short-term therapy of duodenal ulcer.

[Tuberculosis of the transverse colon. Apropos of 2 cases].

The authors report two cases of primitive tuberculosis of the transverse colon. Its rare occurrence and the importance of endoscopy are pointed out. A therapeutic test is often justified. Surgery is needed in case of acute complications or stenosis, and when a malignant lesion cannot be ruled out.
