Growth after intestinal resection for Crohn's disease in children, adolescents, and young adults.

OBJECTIVE: Growth before and after intestinal resection for Crohn's disease (CD) was examined in a group of children, adolescents, and young adults. METHODS: Retrospective chart review of patients who had intestinal resections as clinical management of complications of CD between 1985 and 1996. Pre- and postoperative measurements of weight and height were reviewed. Z-scores were computed for weight-forage (WAZ), height-for-age (HAZ), and weight-for-height (WHZ). Two tailed t tests were used to compare postoperative growth patterns. Significance was defined as p < 0.05. RESULTS: Twenty-five subjects (8 females, mean age 16.2+/-2.8 years with one operation, and 3 males, mean age 15.7 years with multiple operations) were identified. There were significant improvements in the postoperative growth patterns of subjects who had one operation: HAZ (-1.28+/-1.45 versus -0.98+/-1.37, p = 0.041), WAZ (-1.35+/-1.02 versus -0.74+/-0.93, p = 0.0006) and WHZ (-0.64+/-0.95 versus -0.23+/-0.81, p = 0.036). Furthermore, the magnitude of postoperative weight gain directly correlated with the age at CD diagnosis, R2 = 0.16, p = 0.046. Trends towards improved postoperative WAZ (-0.83 versus -0.49) and HAZ (-0.47 versus -0.27) were also observed in the three subjects who had multiple operations. CONCLUSION: The pattern of weight and height growth was improved after intestinal resection for CD. Nonetheless, close monitoring of postoperative growth is necessary especially in children diagnosed with CD at a young age.

Differential gene expression of mammalian SPO11/TOP6A homologs during meiosis.

As the initiator of DNA double-strand breaks during meiosis in Saccharomyces cerevisiae, the SPO11 protein is essential for recombination. Similarity between SPO11 and archaebacterial TOP6A proteins points to evolutionary specialization of a DNA cleavage function for meiotic recombination. To determine whether this extends to mammals, we isolated and characterized mouse and human SPO11 cDNAs. Mammalian SPO11 genes were found to be expressed at high levels only in testis, wherein mouse Spo11 transcript is restricted primarily to meiotic germ cells and is maximally expressed at midpachynema. Mouse Spo11 is located near the distal end of chromosome 2, while human SPO11 is found in the homologous position of chromosome 20q13.2-13.3, a region that is amplified in some breast cancers. Sequence homology and differential expression together support a highly conserved role for SPO11 in the enzymatic cleavage of DNA that accompanies meiotic recombination.

A versatile image analysis approach for simultaneous chromosome identification and localization of FISH probes.

Modern cytogenetic techniques, such as comparative genomic hybridization (CGH) and the multi-color fluorescence in situ hybridization (FISH) techniques of multiplex fluorescence in situ hybridization (M-FISH) and spectral karyotyping (SKY), require a coordinated banding analysis to maximize their usefulness. All of the methods currently used, including Giemsa (G-) banding, Alu banding, and 4',6-diamidino-2-phenyl-indole (DAPI) banding, have serious drawbacks. A simple and effective method to band chromosomes concurrently with FISH is needed. To address this problem, we stained chromosomes with DAPI and chromomycin A3, and then used an image analysis program to generate banding by dividing the image taken with a DAPI excitation filter by the image taken with a chromomycin A3 excitation filter. The result was a metaphase spread in which the chromosomes possessed a banding pattern characteristic of R-banding. The image analysis program was then used to generate linescans of pixel intensity versus relative position along the length of chromosomes that were banded using this technique, which we have called D/C R-banding. Each chromosome in a genome was represented by a characteristic scan profile, which was unaffected by FISH signals. Reference linescans were prepared by karyotyping D/C R-banded chromosomes for a given species, and then drawing lines along the length of the known chromosomes. The linescans were combined into a spreadsheet database, which was linked by dynamic data exchange to the image analysis program and normalized for length and intensity. The linescan of an unknown chromosome was then transferred to the spreadsheet, where it was normalized for length and intensity and overlaid on the linescans of each chromosome in the genome. Unknown chromosomes were identified by comparison of their graphs with graphs in the standardized reference genome. We have used this approach to create reference linescan karyotypes of several species, and to identify chromosomes on which FISH was performed.

Size dependence in prosimian locomotion and its implications for the distribution of body mass.

The mechanical requirements for arboreal life are reviewed and the constraints which these requirements impose on the body of a prosimian are defined. The mechanical necessities can be fulfilled only by animals which possess the appropriate morphological characters. It is incorrect to refer to these morphological traits directly as 'adaptations'. Instead their a priori existence must be considered as the precondition for the acquisition of a certain life-style. Once such a life-style has been acquired, a strong selective pressure acts towards a further refinement of such 'adaptations' or 'pre-adaptations'. Postcranial morphology must be seen in a context of following natural laws and is strictly related to the mechanics of posture and locomotion. The traits emphasised and explained here are body proportions--specifically the relative lengths of body segments and the distribution of (muscle) mass on these segments.

Cholesterol metabolism and efflux in human THP-1 macrophages.

This study has investigated in detail factors regulating accumulation, esterification, and mobilization of cholesterol in human THP-1 macrophages. Human THP-1 monocytes were differentiated into macrophages and then cholesterol enriched by exposure to acetylated LDL (AcLDL), together with [3H]free cholesterol (FC). Although THP-1 macrophages accumulated FC and esterified cholesterol (EC), assessed by both mass and radioactivity, cellular EC always demonstrated a much lower specific activity (cpm/ microg) than did cellular FC, and several potential causes of this finding were investigated. Inhibition of acyl-CoA:cholesterol acyltransferase (ACAT) during loading decreased cell [3H]EC by 95+/-1.4% but decreased cell EC mass by only 66.0+/-4.0%, indicating that some intracellular undegraded AcLDL-derived EC was present in these cells. Esterification of [3H]oleate to EC in THP-1 cells loaded with AcLDL was 2.0 nmol x mg-1 x h-1, consistent with previous literature. However, EC, triglyceride, and phospholipid fractions respectively contained 1.0+/-0.07%, 80.0+/-0.5%, and 18.9+/-0.3% of cell [3H]oleate, indicating triglycerides were much more metabolically active than EC. In addition, the mass of triglyceride in THP-1 macrophages exceeded that of EC both before and after exposure to AcLDL. Esterification of nonlipoprotein-derived cholesterol was compared in THP-1 cells and nonhuman Fu5AH, CHO, and RAW macrophage cells. Whereas the nonhuman cell lines all esterified over 30% of 2-hydroxypropyl-beta-cyclodextrin (hp-ss-CD)-delivered cholesterol within 6 hours, THP-1 cells esterified <8.0% of incorporated cholesterol. Kinetics of cholesterol efflux from AcLDL-loaded THP-1 cells were first investigated after loading with only FC, and interactions between efflux and EC hydrolysis were further assessed after loading cells with both EC and FC. Over 24 hours, human apolipoprotein (apo) A-I, apoHDL reconstituted with phosphatidylcholine, and HDL3 respectively removed 46.6+/-3.7%, 61. 3+/-3.4%, and 76.4+/-10.1% of [3H]FC from FC-enriched THP-1 cells. Cholesterol efflux to apoA-I was saturated by 24 hours and was enhanced by using apoA-I-phospholipid instead of pure apoA-I. Kinetic modeling identified that 97% of effluxed FC derived from a slow pool, with a T1/2 ranging from 27.7 hours for HDL to 69.3 hours for apoA-I. Although efflux enhanced net clearance of EC, hydrolysis of EC during concurrent inhibition of ACAT was unaffected by cholesterol efflux. Supplementation of THP-1 cultures with cAMP to stimulate hormone-sensitive lipase did not significantly enhance net hydrolysis of EC or cholesterol efflux. In conclusion, human THP-1 macrophages contain a large and metabolically active pool of triglyceride and a relatively inactive pool of EC. The low specific activity of EC relative to FC is contributed to by reduced esterification of FC, slow hydrolysis of EC, and accumulated lipoprotein EC. The relative inactivity of the EC pool may further contribute to already impaired cholesterol efflux from these cells. Net cholesterol efflux from human macrophages is achieved by pure apoA-I and is substantially further enhanced by the presence of phospholipid in acceptor particles.

Clinically insignificant (natural) autoantibodies against acetyl cholinesterase in the sera of patients with a variety of neurologic, muscular and autoimmune diseases.

Acetyl cholinesterase (AChE) antibodies were shown to be associated with myasthenia-like neuromuscular disease. However, it is not clear whether they cause the disease, or their presence is secondary to the disease or an unrelated epiphenomenon. Therefore, AChE antibodies were studied in the sera of 135 patients with neurologic, muscular and autoimmune diseases, using enzyme linked immunosorbent assay (ELISA), immunoblotting and enzyme inhibition assay. In 12 sera the AChE binding by ELISA was greater than 2 standard deviations (SDs) above the mean value of the 20 healthy controls. However, this increased binding was not disease-specific, had no clinical correlates and could not be demonstrated using Western blotting and AChE enzyme inhibition assay, suggesting that these antibodies are naturally occurring, pathogenically unimportant autoantibodies. The finding also supports a possible pathogenic role for the previously reported, high titer, high affinity, inhibitory AChE antibodies in the neuromuscular disease.

Clinical nurse specialists: creating new programs for neonatal home care.

Specialty home care programs are making headlines in nursing. Clinical nurse specialists are taking the lead in designing and directing follow-up home care for infants in neonatal intensive care units. The article describes a home care program in a Texas community that utilizes neonatal intensive care unit nursing staff as the home care providers for these infants.

Size influences on primate locomotion and body shape, with special emphasis on the locomotion of 'small mammals'.

The mechanical laws which make possible several characteristic and well-known modes of primate locomotion are reviewed. Biological requirements are fulfilled in small and in large primates by utilizing different mechanical principles. On the basis of the mechanics, special morphological traits can be identified which are advantageous for performing these locomotor modes, and which determine different life-styles. These morphological 'adaptations' consequently are different in larger and smaller primates. The divergence between large and small forms is clarified by the inclusion of non-primate mammals into the comparisons.

Reconstitution of a group I intron self-splicing reaction with an activator RNA.

The self-splicing rRNA intron of Tetrahymena thermophila belongs to a subgroup of group I introns that contain a conserved extra stem-loop structure termed P5abc. A Tetrahymena mutant precursor RNA lacking this P5abc is splicing-defective under standard conditions (5 mM MgCl2/200 mM NH4Cl, pH 7.5) in vitro. However, the mutant precursor RNA by itself is capable of performing the self-splicing reaction without P5abc under different conditions (15 mM MgCl2/2 mM spermidine, pH 7.5). We have investigated the functional role of the P5abc in the mechanism of the self-splicing reaction. When an RNA consisting of the P5abc but lacking the rest of the Tetrahymena intron is incubated with the mutant precursor, the self-splicing reaction proceeds highly efficiently under standard conditions (5 mM MgCl2/200 mM NH4Cl, pH 7.5). Two steps of the bimolecular self-splicing reaction can be performed accurately by a shortened precursor RNA containing all essential components required in the self-splicing reaction and an activator RNA consisting of the P5abc. Gel-mobility-shift assays suggest that two molecules associate by a direct RNA-RNA interaction during the splicing reaction. The results imply that there might exist other small RNAs whose role is to activate ribozymes.

[Feed conversion and the energy requirement of pregnant heifers].

Eight Holstein heifers were subjected to experiments running over the entire pregnancy period to investigate the conversion of rations mainly made up of maize silage (85% of the ration's dry matter) by the experimental animals. Both digestible and metabolizable energy (ME) and the energy requirement were studied. The live weight (LW) of the heifers was found to rise linearly during the entire pregnancy period being 422 kg at service and reaching 565 kg at calving. Feed intake, referred to 1 kg live weight0.75, was almost constant until the 7th month of gestation and declined gradually until parturition. The degree of digestibility and metabolizability of the energy contained in the silage-bases rations prove equal over the whole gestation period. The energy required for maintenance and gravidity (y) was found to rise exponentially as pregnancy advance (t). This relationship is reflected by the equation: ME, kcal/kg LW0.75y=135+0.2590.0206t range of significance=+/-0.07 During the last two months of gestation, a negative energy balance was found. This negative energy balance corresponds with a negative fat balance and a reduction, of the respiratory quotient, a fact suggesting to the mobilisation of body fat reserves by the mother to cover the increased energy requirement during pregnancy. The protein balance was found continuously positive during the entire gestation period. The conversion of metabolizable energy for fetus development was found to be 8.13%.

The relationship between women's symptoms of endometriosis and self-esteem.

OBJECTIVE: To determine the relationship between women's symptoms of endometriosis and self-esteem. DESIGN: Descriptive. SETTING: Private practice. PARTICIPANTS: Twenty-three mostly middle class, employed women who had been diagnosed with endometriosis and had received treatment from a physician. METHOD: The volunteer subjects completed a two-part questionnaire. One part consisted of questions on demographic characteristics, health history, menstrual history, and symptoms of endometriosis, and the other part consisted of the Rosenberg Self-Esteem Scale. MAIN OUTCOME MEASURES: The relationship between women's self-esteem and perceptions of the number, frequency, and severity of their symptoms. RESULTS: No significant correlation was found between self-esteem and the number, frequency, and severity of endometriosis symptoms. Significant correlations were found between the number/frequency of symptoms and the severity of pain and between the frequency and number of symptoms. CONCLUSION: Although endometriosis symptoms did not influence self-esteem, they interfered with women's lives. Assessment of psychosocial status and nursing interventions designed to relieve pain are important factors in assisting women with endometriosis.

Home care of the battered pregnant woman: one battered woman's pregnancy.

Perinatal nurses in the home care setting should screen all women for battering. Pregnant women who are battered need nursing education and intervention to help break the cycle of abuse and promote optimal maternal and fetal outcomes. This case report describes nursing care in the home for a battered pregnant patient at high risk.
