Size and shape evaluation of astrocytoma nuclei with the shape analytical morphometry software system.

Eighteen cases of astrocytomas diagnosed as grades 1-3 of malignancy were studied with analytical morphometry to determine if different grades can be discriminated by size and shape parameters related to the nuclei. The shape analytical morphometry system was used to calculate dimensional measurements and express shape irregularities through quantitative parameters. With the shape asymmetry evaluator procedure, nuclear symmetry was evaluated. All parameters subjected to univariate and multivariate strategies were the same in easily distinguishing grade 1 from the others. Grades 2 and 3 could not be discriminated completely by either dimensional or with analytical parameters revealing strong similarities of nuclear shape and dimensions.

Computer-aided morphometric analysis of lymph nodes draining laryngeal carcinomas.

A new method of morphometric evaluation of nodal patterns in patients with laryngeal cancer was developed. In 8 selected cases with median tumor and unilateral metastasis, the morphometric data about the perimetastatic nodal tissue and contralateral node in the same patient were compared. The results showed the paracortical area to be consistently larger in lymph nodes without metastasis than in those containing metastases. The mean number of blasts, mitoses and postcapillary venules was also increased in the paracortical area in the non-metastasis-containing nodes. These data support the idea that delayed immunity may inhibit development of metastases. The need for objective morphometric methods in lymph node examination is emphasized.

Prognostic value of the prelaryngeal node in laryngeal and hypopharyngeal carcinoma.

The role in tumor spread and the prognostic value of metastasis in a prelaryngeal node were investigated in 124 cases of laryngeal and hypopharyngeal carcinoma selected from about 900 total laryngectomies and pharyngolaryngectomies. Metastasis in a prelaryngeal node was noted in 26 cases: 22 without capsular rupture (N+) and 4 with capsular rupture (N+R+). Vascular embolism (E+) was never noted. Patients with a metastatic prelaryngeal node had a tumor of the piriform sinus in 13 cases (50%), a transglottic tumor in 6, glotto-subglottic in 3, glottic in 2 and supraglottic in 2. They also more frequently had involvement of the cervical nodes (81%) and of the thyroid gland. The 5-year survival in these patients is very poor: only 3 patients (11.5%) were alive and free of disease. These data demonstrate the role of prelaryngeal nodes in tumors that affect the hypopharynx and the antero-inferior portion of the larynx and that directly reach this node through the anterior lymphatic peduncle.

RNA expression bcl-w, a new related protein Bcl-2 family, and caspase-3 in isolated sertoli cells from pre-pubertal rat testes.

Apoptosis has a major role in molding the embryo, in the maintenance of tissue homeostasis, and in the defense against pathogens, while its disgregulation is strongly implicated in cancer as well as in autoimmune and degenerative diseases. The opposite action of anti-apoptotic proteins (Bcl-2 family) and pro-apoptotic proteins (p53, Bax, Bak) regulates the activation of caspases that are the effectors proteases of the cell suicide. Bcl-W is a pro-survival protein, recently discovered, related to the Bcl-2 family. The presence of Bcl-W is fundamental for spermatogenesis in rats. Caspases are cysteine-dependent aspartate-specific proteases, and their over-expression can result in apoptotic cell death. Normally, caspases exist in cells as inactive pro-enzymes and can be activated by 2 distinct mechanisms: the FADD/caspase 8 cascade, and the Apaf-1/caspase 9 cascade. These 2 mechanisms are used extensively by cells for the activation of the effectors caspases: caspase 3, caspase 6, and/or caspase 7. Bcl-W and caspases might have a pivotal role in maintenance of Sertoli cells integrity. In this study, we demonstrate that both Bcl-W mRNA and caspase 3 mRNA are expressed in isolated Sertoli cells of pre-puberal rat testes. This finding might be crucial in clarifying whether Sertoli cells die by an apoptotic mechanism. Further studies are required to understand whether the expression of Bcl-W and caspases is different before and after puberty in rat testis and/or in pathological conditions, that lead to an increased cell apoptosis.

Multiple osteoid osteoma. A clinical case.

Osteoid osteoma is a benign tumor occurring frequently during the growth years. It may be found in any of the skeletal segments, and it is often present as an isolated neoformation. In some rare cases it may involve more than one segment or it may be multicentric. The authors report their experience with a case of double localization of metachronous osteoid osteoma.

[Petit's lumbar hernia: about a case].

The authors are surgeons of Moscati Hospital in Aversa (CE)-Italy, and one of them is a Radiologist in the same hospital; they describe a rare case of lumbar hernia, the Petits hernia and describe the anatomical aspects of parietals hernias. Subsequently they describe the diagnostics aspects and then the surgicals treatments. In this case-report the authors describe the surgical operation to use one prolene-net to close the parietal breach. The most important aspect, that the authors emphsasize, is the absolute importance of careful clinic examination to diagnose it. This work is completed with some pictures that show it; some pictures are made in operatory-room during the surgical treatment and some of them are TC images.

Non-Hodgkin's lymphoma in the gluteal region: a case report.

The infiltration of the skeletal muscles on the part of lymphomatous cells is rarely reported and only histological examination can reveal it. Based on a review of the literature, it is the purpose of this study to describe a rare case of non-Hodgkin's lymphoma with muscular invasion, characterized by voluminous swelling in the left gluteal region, in a female aged 60 years who was completely asymptomatic. Biopsy and immunohistochemistry alone allowed us to diagnose non-Hodgkin's lymphoma. It is the purpose of this study to document the existence of NHL with musculoskeletal localization, emphasizing the morphological features and the site, as well as hypothesizing that wide-margin surgery can constitute a valid therapeutic aid for such extralymphonodal forms, that are particularly large and with a single localization.

[What is the prognostic significance of histomorphology in small cell lung carcinoma?].

What is the prognostic significant of the histomorphology in the small cell carcinomas of the lung? After the WHO classification of the lung cancer (1981), several studies criticized the subdivision of the small cell carcinoma in three sub-types (oat-cell, intermediate cell and combined types). The role of histology in the prognostic predition has been devaluated. In order to verify the prognostic value of the morphology of the small cell types of lung cancer, we performed a multivariate analysis in 62 patients. The survival rate was analytically compared with the following parameters: nuclear maximum diameter, nuclear form, nuclear chromatism, chromatine distribution, presence of nucleolus, evidence of cytoplasm. The results showed that none of these parameters are able to express a prognostic value. According to the recent studies, we think that the small cell carcinoma of the lung is a neoplasia with a multiform histologic pattern. Differences observed in clinical management are not correlate with the morphology, but with other biological parameters still unknown.

Recurring intracranial meningiomas. Morphometrical evaluation of nuclear pleomorphism by S.A.M. (shape analytical morphometry) system.

Meningiomas are the most common neoplasms of the central nervous system and their biological behavior is not always predictable from the histologic appearance of the tumors. The nuclear pleomorphism seems to be one of the most important morphological features in the prediction of recurrence. By using analytical morphometric methods it is possible to quantify nuclear atypias and to obtain parameters describing nuclear contour irregularities and distortions of the figure. Moreover the amount of information obtained from analytical procedure allowed to discriminate, by multivariate discriminant analysis recurrent or no-recurrent meningiomas (5% of error).
