[Echocardiographic monitoring of the left ventricle in infants and children presenting the primary form of acute cardiac failure].

A retrospective multicenter study was based on influential echocardiograms performed in 34 children, the majority (29/34) being under 2 years of age, and presenting with a primary form of acute congestive heart failure. Four died early. The average duration of serial echocardiographic monitoring was 25 months (9-72 months) in the others. During the acute stage, the left cardiac cavities were extremely dilated and the shortening fraction was reduced to 15 +/- 4%. The 4 decreased children belonged to a group of 17 children who had a shortening fraction less than 13%. From the 8th day significant improvement of the left ventricular function was observed. Recovery was faster during the first 6 months of evolution and became very low after 1 year. The age at onset, the severity of the initial left ventricular changes, and the speed of recovery during the first 15 days were not predictive of the degree of eventual recovery of left ventricular function. At the end of the study 28 of the 30 surviving children had recovered a normal shortening fraction. However 16 of 30 still showed some dilatation, albeit moderate in most cases, of this ventricle. These data show that most children presenting with a primary form of acute congestive heart failure recover satisfactory heart function and that the initial echocardiographic features are not predictive of the subsequent course.

[Suspicion of Haemophilus influenzae endocarditis of the bicuspid aortic valve in a 7-month-old infant].

The authors report a case of probable Haemophilus influenzae endocarditis in a 7 month-old infant with a bicuspid aortic valve. Precocity of the occurrence of endocarditis in such asymptomatic cardiac abnormality, scarcity of the suspected germ, and the observed mode of evolution are underlined. This case raises the problem of an endocardial involvement in an infant properly treated by adapted intravenous antibiotherapy. It can be concluded that regular clinical and echocardiographic examination is necessary for every case of severe Haemophilus influenzae infection.

[Neonatal hydropericardium caused by a silicone epicutaneo-caval catheter].

The authors describe the first case of neonatal hydropericardium involving a silicone central venous catheter which is known for its flexibility. Pericardial effusion was shown by echocardiography. The distal tip of the catheter was drawn back into the superior vena cava and the infant's condition improved quickly.

Exercise-induced asthma in schoolchildren. A pilot study in two Haifa districts.

The prevalence of exercise-induced asthma was studied in 313 Haifa schoolchildren, 160 living in an unpolluted urban area close to the sea and 153 living in a rural kibbutz (communal agricultural settlement) in the proximity of polluting industries. The overall prevalence was 8.6%; it was higher in the kibbutz group, but the difference was not significant. The prevalence tended to fall with increasing age and did not change with sex.

[Clinical evolution and cytogenetic study of two acute lymphoblastic leukemia (type L2) in the child: prognostic value of the karyotype].

Cases of two newborns with acute lymphoblastic leukemia (ALL) L2 type, are reported. In each case, some chromosomal abnormalities can be found. In the first case, a translocation t (4;11) is noticed. It has to be compared with already published patients' cases and so the non randomly occuring character of those alterations in ALL and poor pronostic factor can be confirmed. In the second observation, a complex translocation t (5;6;X) never described before in literature, was observed. Chromosomal findings in ALL are not only a help to diagnosis but, by cytogenetic data, are also a help to accurate prognosis and adequate treatment.

[Infant encephalopathies with rapid EEG rhythms. Apropos of 2 cases].

The authors report two cases of infant encephalopathies with psychomotor retardation. The cerebrospinal fluid, the electroretinogram, the electromyogram and the motor conduction velocity are normal. The neuromuscular cutaneous conjunctival biopsies are normal. No biochemical abnormalities are found. The electroencephalogram presents continuous high voltage fast rhythms (250-400 microV) with reduced or absent evoked responses. Early infantile neuroaxonal dystrophy and lissencephaly are suggested.

Hydromediastinum and hydrothorax in the neonate with central vein catheterization.

A near fatal complication of central venous catheterization in a newborn infant is reported. Five days after insertion, the catheter tip eroded through the wall of the vein, causing hydromediastinum and hydrothorax with acute circulatory and respiratory distress. The diagnosis was confirmed by noting that the tip of the catheter had become curved downwards, by aspirating slightly bloodstained fluid from the pleural space and by injecting radio-opaque dye through the catheter. Recovery followed removal of the catheter, intercostal drainage, intubation and positive pressure respiration. The erosion of the vein may have been brought about by the location of the catheter tip at the junction of the left subclavian and innominate veins--a less than optimal position. The factors causing this complication are discussed. Recommendations are made about the type of catheter, its appropriate placement and maintenance. The diagnosis and management are surveyed.

[McKusick-Kaufman syndrome in a new born girl (author's transl)].

In a newborn, the presence of an abdominal tumor due to hydrocolpos resulting from vaginal atresia, with polydactyly and a congenital heart disease suggest the diagnosis of McKusick-Kaufman syndrome. This syndrome is different from the Ellis Van Creveld syndrome, which also includes polydactyly and congenital heart disease, associated with "chondrodysplasia' and "ectodermodysplasia'.

[Postaxial polydactyly in a female neonate associated with hydrocolpos due to vaginal atresia and with a congenital cardiopathy: the McKusick-Kaufman syndrome].

The existence in a new-born child of post-axial polydactyly, associated with an abdominal tumor due to hydrocolpos, because of a low vaginal atresia, and with congenital heart-disease, recalls the diagnosis of the McKusick-Kaufman syndrome. This syndrome must be differentiated from the Ellis-Van Creveld syndrome, which also includes polydactyly and congenital heart disease, associated with a "chondrodysplasis" and an "ectodermodysplasia".
