[Endovesical antireflux plastic surgery. Original technique].

The authors present a technique of endovesical transposition of the urethra with orthotopic reimplantation of latter. This very effective technique is simple. In addition, it allows secondary catheterization and reduces post-operative complications.

Diagnosis of bovine cryptosporidiosis by an enzyme-linked immunosorbent assay.

This paper describes an enzyme-linked immunosorbent assay (ELISA) for the diagnosis of cryptosporidiosis. A monoclonal antibody with a high affinity against an oocyst antigen was used to set up the test. The efficiency of this assay was compared with that of the flotation test; 275 calf faecal samples were examined by the two methods. There was 96% agreement between the two tests. For the 11 conflicting samples, the two tests were repeated and a modified Ziehl-Neelsen staining was performed on faecal smears. All these 11 samples contained few oocysts, but only five and six of them were shown to be positive by the ELISA and flotation tests, respectively. The degree of sensitivity of the ELISA and flotation tests is comparable; samples heavily or moderately contaminated with oocytes are detected by both methods. This ELISA is reliable and never gives rise to false positive results. Nevertheless, as with the flotation test, the occasional case containing very few oocysts will not always be detected by this test. If necessary, very accurate diagnosis can be made by a staining technique or by a direct immunofluorescent assay. In veterinary medicine, the ELISA seems to be a method of choice; it appears to be a fast and reliable technique which could be used as a routine test for the detection of Cryptosporidium oocysts. Nevertheless the degree of sensitivity must always be borne in mind. There is no need for a microscopic examination, which is an additional advantage.

Sequence comparison between the fusion protein of human and bovine respiratory syncytial viruses.

The nucleotide sequence was determined for the fusion (F) protein-coding mRNA of the bovine respiratory syncytial virus (strain RB 94) and the amino acid sequence of the F protein was deduced for comparison with the sequence of human respiratory syncytial virus subtypes A and B (RSS-2 and 18537 strains). The human and bovine RS virus F proteins (excluding the cleaved signal peptide) share 83 to 84% homology. The greatest divergence occurred within the F2 subunit in the region preceding the cleavage activation site.

Toward a prospective payment system for ambulatory surgery.

In this article, ambulatory surgery among the aged Medicare population in 1985 is examined. Total hospital facility charges for ambulatory surgery in that year were estimated at $1.8 billion, with about one-half of that amount involving cataract surgery. The possibility of using diagnosis-related groups for a prospective payment system for ambulatory surgery was examined and was rejected for two reasons: (1) about 20 percent of the dollar volume of hospital-based ambulatory surgery fell into medical diagnosis-related groups and (2) the ratio of inpatient diagnosis-related group weight to outpatient billed charges for the ambulatory procedures falling into a given diagnosis-related group varied more than tenfold, making diagnosis-related group weights impossible to use in a consistent manner. A newly developed version of ambulatory visit groups and the even newer ambulatory patient groups were then considered as an alternative for a prospective payment system. These are briefly described.

Measuring quality in medical case management programs.

Private insurers and health care providers alike currently offer medical case management programs as a means of containing costs and enhancing the quality of care for patients with high-cost illnesses or injuries. Since 1986, Brandeis University's Bigel Institute for Health Policy has been involved in a project to evaluate medical case management programs. The study found that such programs have become popular because of the perception that high-cost patients are typically handled inefficiently. The study also found that greater attention needs to be given to developing systematic ways to measure and monitor the quality of the case management process and its effect on outcomes.

Isolation and purification of bovine immunoglobulins: use of Sephacryl S-300 filtration avoids protein precipitation steps.

A simplified purification method for bovine IgG1, IgG2, IgM, IgA and S-IgA is presented. It takes advantage of Sephacryl S-300 gel filtration. When applied to this column, colostral whey can be separated into IgM, S-IgA and IgG rich fractions. Filtration of blood serum leads to IgM, IgA and IgG rich fractions. IgM, IgG1, IgG2, S-IgA and IgA are then further purified by anion exchange chromatography.

Training in racial harassment issues for community staff.

Community nurses, in their daily routine of work, come across different forms of racial harassment. However, not all community nurses have access to help and information on positive approaches to combat harassment experienced by themselves or their clients. This paper describes a training initiative for community nurses supported by the local trust.
