[Guidelines of the Spanish Society of Cardiology for car driving, airplane flying,and underwater activities in subjects with heart disease].

Car driving, airplane piloting and underwater activities by subjects with heart disease may cause sudden incapacitation leading to the loss of the safety margins necessary to avoid accidents. In the case of car driving and airplane piloting the risk affects, not only the driver or pilot, but also passengers and/or bystanders within an accident zone. In the case of diving the risk resides basically in the loss of control of the vital support mechanisms necessary in a very hostile medium. This document reviews the possible causes of unexpected incapacitation, with or without loss of consciousness, in the light of the pathophysiologic consequences of fatigue, hypoxia, stress or barotrauma posed by each activity. Detailed recommendations are made for limiting driving, piloting and diving, based on official Spanish and European regulations and the addresses of specialized centers are provided for consultation. Moreover, recommendations for airplane travel for patients with heart disease are indicated.

[Double post-acute myocardial infarction complication: rupture of the interventricular septum and acute mitral insufficiency].

We present a case of double post acute myocardial infarction complication: ventricular septal defect and acute and severe mitral insufficiency. As a consequence of the delay in the diagnosis, the patient developed pulmonary hypertension with values at the systemic level. The patient underwent surgery in order to close the ventricular septal defect and aneurysmectomy, resulting in posterior regression of mitral insufficiency and pulmonary circuit values became normal. The ethology, diagnosis, evolution and treatment of this exceptional association of acute post myocardial infarction complications are discussed.

[Incessant fetal tachycardia coinciding with supraventricular tachycardia of the mother].

The case of a gestant diagnosed fetal arrhythmia in the 35th week of gestation is presented. A fetal echocardiography indicates an aneurysm of the interauricular septum and a fetal supraventricular tachycardia. Also is obtained an ambulatory 24-hour Holter electrocardiograph of the gestant and fetus rhythm. The register makes evident a gestant supraventricular tachycardia and as well a fetal supraventricular tachycardia, occurring in both without clinical company. Following the birth, the tachycardia in the newborn is identified to be automatic and incessant, requiring digital for its control. In postpartum registers of the mother no significant arrhythmia of any type is observed. We conclude that the ambulatory monitoring with a Holter electrocardiograph could be a good method of diagnosis and follow-up of fetal and gestant arrhythmias.

[Fistula from the coronary artery to the peripheral branches of the left pulmonary artery].

A case is presented of a coronary artery fistula originating in the circumflex artery which drains in a branchified pattern in the peripheral branches of the left pulmonary artery. The diagnosis came as a result of a diagnostic coronarography in a woman with ischemic cardiopathy. The etiology, evolution and treatment of this exceptional coronary anomaly is being discussed.
