Hypertensive disorders, diabetes mellitus, and anemia: three common medical complications of pregnancy.

Hypertensive disorders, diabetes mellitus, and anemia are three common medical complications of pregnancy. In many cases, primary care physicians manage them. The understanding of these conditions and recommendations for their treatment are in constant evolution. Care for patients with these problems requires a working knowledge of current information. This article reviews the current knowledge about and treatment of these three disorders.

Differential expression of alpha2-7, alpha9 and beta2-4 nicotinic acetylcholine receptor subunit mRNA in the vestibular end-organs and Scarpa's ganglia of the rat.

To further characterize the pattern of expression of the nicotinic acetylcholine receptor (nAChR) subunits in the peripheral vestibular system, we conducted RT-PCR of all known mammalian nAChR alpha and beta subunits in mRNA extracted from adult rat vestibular primary afferent neurons (Scarpa's ganglia) and vestibular end-organs. Transcripts encoding the alpha2-7 and beta2-4 nAChR subunits were found in the vestibular ganglia, while alpha3, alpha5-7, alpha9 and beta2-4 nAChR subunits were expressed in the vestibular end-organs. These results support previous electrophysiological, immunocytochemical and molecular biological data, and also provide a more complete understanding of the role of nAChRs in the neurochemical transmission subserving the efferent-afferent interaction in the vestibular periphery.

Energy cost of walking and of wheelchair propulsion by children with myelodysplasia: comparison with normal children.

The oxygen consumption of 15 myelodysplastic children during walking and propelling a wheelchair was studied. In comparison to normal children, they walked more slower and consumed more oxygen per meter, but had a similar rate of oxygen consumption. A swing-through gait pattern was 33 per cent more energy-efficient than a four-point gait pattern for these children. Wheelchair propulsion produced velocities and energy efficiencies similar to normal children walking. Oxygen consumption and velocity measurements were found to be significantly related to the clinical factors of level of lesion and strength of hip and knee extension. Regression lines and equations are presented to permit prediction of energy cost and efficiency from heart-rate data of myelodysplastic children.

Toxicity of an in situ oil shale process water to rainbow trout and fathead minnows.

An in situ oil shale process water, designated Omega-9 water, was used in flow-through bioassays with fathead minnows, rainbow trout and rainbow trout eggs. Of the two fish species, rainbow trout were more sensitive to acute exposure to Omega-9 water with 96-hour LC50 dilutions of 0.51% and 0.41% in two independent determinations. In embryo-larval studies, the length of fry from eggs hatched and maintained in 0.16% process water was significantly less than that of eggs hatched in control water. A solution of the 13 major inorganic constituents of Omega-9 water, with a 96-hour LC50 of 0.56% for rainbow trout, showed that inorganics accounted for most of the acute toxicity of Omega-9 water.

Further characterization of the inhibition of aldehyde dehydrogenase activity by pargyline.

The in vivo inhibition of low Km mitochondrial aldehyde dehydrogenase (AlDH) activity by pargyline was not maximal until more than 30 minutes after i.p. injection. Enzyme activity returned to control levels within 36 hours of drug injection but the return of activity was slowed by cycloheximide pretreatment. Female rats and higher basal total and low Km mitochondrial AlDH activities than did males. Injection of pargyline inhibited low Km mitochondrial AlDH activity more in males than in females. Incubation of rat liver microsomes with an NADPH-generating system and pargyline produced an in vitro inhibitor of low Km mitochondrial AlDH activity. Pretreatment of rats with phenobarbital increased the AlDH inhibitor produced by incubation of their microsomes with pargyline. Injection with benzylpropargylamine, N-demethylated pargyline, also preferentially inhibited the low Km form of mitochondrial AlDH activity. Neither pargyline nor benzylpropargylamine injections affected microsomal AlDH activity. Total AlDH activity, measured with 5mM propionaldehyde, in rat liver 100,000g supernatant was not changed by administration of either drug. Supernatant activity assayed with 50 microM propionaldehyde was inhibited by both pargyline and benzylpropargylamine treatment.

The effect of an in situ-produced oil shale processing water on metabolism.

The effects of in situ-produced oil shale retort water on the metabolism of various substrates was studied both in vivo and in vitro. The induction observed in rats was classified as Type I due to an increase in metabolism of hexobarbital and ethylmorphine without subsequent increases in zoxazolamine metabolism. The maximal absorption of the cytochrome-P450-CO complex was observed to be 450 millimicron, also consistent with Type I inducers. Cytochrome P-450 levels were also significantly increased over controls.

Imprisonment of patients in the course of rehabilitation.

The patient carries the major responsibility and plays the most significant role in the rehabilitation process. It is appropriate to continuously examine rehabilitation management techniques to determine the degree to which they facilitate or inhibit patient participation. Of 213 consecutive inpatients on the rehabilitation service of an urban municipal hospital serving a black community, 53 requested discharge before the team considered it advisable. Thus, one patient in four differed with the staff on the decisive issues of functional ability within the environment and readiness for discharge. These "imprisoned patients" were confined against their wills. Improved communication between patients and staff, patient education and mutual goal-setting would yield greater patient participation on the decision-making process.
