[The effect of chenodesoxycholic acid on cholesterol gallstones (author's transl)].

30 patients with radiolucent gallstones and 2 patients with radioopaque stones recieved chenodesoxycholic acid (CDC). In 12 patients stones were dissolved completely by CDC therapy, in 6 of the patients the size of the stones decreased by 40-70%. Only 1 patient developed severe diarrhea, so therapy had to be stopped. In 5 patients there was a slight and transient increase in SGOT and SGPT, in 2 other patient gammaGT increased; value normalized while treatment was continued. In 23 cases liver biopsies were done before and during treatment: no histological changes could be found in the specimens. 5 of the patients, whose gallstones had been dissolved, received 500 mg CDC/day thereafter, another 5 patients received 500 mg/d every other day. No recurrence of lithiasis was observed in either group. One patient on 500 mg CDC twice a week developed lithiasis after 6 months, another one stopped treatment completely and gallbladder stones reappeared after 8 months. The patients with radioopaque stones did not show any changes during treatment.

Therapeutic evaluation of chronic aggressive nepatitis and its relation to the Australian antigen.

Twenty-seven patients with chronic aggressive hepatitis were treated with azathioprine and methylprednisolone for a long term ranging from 7 to 92 months. HbsAg was positive in 59% of cases. Of the 27 patients, 12 revealed healing between 7 and 82 months after therapy was started; seven patients revealed histological improvement with an interval in treatment between 12 and 58 months; two of them did not show histologic changes from 37 to 84 months and 6 developed cirrhosis at the 24th and 92nd months after treatment. It was noted that all the negative HbsAg either improved or healed, while the positive HbsAg achieved a favourable evolution in 50% of cases; the rest remained either in an histologically unchanged condition or developed cirrhosis. On considering the healed patients we may confidently state with 95% of certainty that treatment leads to normalization between 9% and 50% for the positive HbsAg patients and between 37% and 92% for the negative HbsAg patients.

Adolescent

[Early detection of colonic and rectal cancer; extensive study of occult blood].

Due to the high frequency and usually late diagnosis of cancer of colon and rectum the "haemoccult test" has been recommended as an easy, quick, and unexpensive method for stool determination of occult blood that will allow us to suspect the existence of these tumors in their early asymptomatic stages. The test was performed in 1437 patients attending the gastroenterology clinic which had no clinical evidence of bleeding, of these, 1150 returned the samples. In 52 (4,6%) the test was positive for occult blood. The studies performed revelated that 4 were upper gastrointestinal ilions, one ileal diverticulum and 40 (3,47%) patients had colonic disease of the latter 19 were diverticular disease, 1 crohnis disease of rectum, 2 adenomatous and 2 villous polips, 1 Hodkgin disease and 7 carcinomas. There were 6 false positive results. The high frequency of positive "haemoccult test" in colonic disease, specially benign and malignant tumors, make this test and appropiate method of this time in the detection for colo-rectal carcinoma.

[Ultrastructural study of chronic gastritis].

A study of gastric biopsies with optical and electronical microscope from patients with gastritis in different stages of evolution was realized. The ultraestructural results consist in phenomenal alterations at the epitelium level, apparition of bodies of mielinic aspects in the inside of the Oxintcs cells, reduction of the number of this and the peptic cells and even the disappearance of the last. There is an increase of the mucous cells with mitotic changes, notable hyperplasia of the arget cells and intestinal metaplasia in the advanced cases. In the stroma is demonstrative the infiltration of plasmatic cells having a endoplasmatic reticule in different stages of activity, linphocytes and macrophagous full of primary and secondary lysosomas. A very important fact is the increase of the argentofilous cells and its probably relation with the appearing of the gastritis. The presence of the plasma cells and lynphocyts mark the role that takes the immunitary reaction in this process.
