[Evaluation of the functional capacity of patients in the acute period of mild craniocerebral injuries].

An analysis of the results of a combined psychophysiological examination has demonstrated that the absence of marked clinical manifestations cannot be considered as a valid criterion justifying the denial or discontinuation of inpatient treatment. Hence, the results of psychophysiological examination may serve only as an additional criterion in the evaluation of the patients' condition.

[The clinical classification and conceptual approaches to the treatment of the sequelae of craniocerebral trauma].

A clinical and morphological classification of the sequelae of craniocerebral injury is proposed, which identifies tissue, spinal fluid dynamic, and vascular forms of posttraumatic pathology. The paper shows the necessity of leading development of present-day conceptual approaches to treating the sequelae of craniocerebral injury by using research knowledge of their pathogenesis and new medical technologies. A hundred eighty one cases of chronic subdural hematomas were used as an example to show a role of conceptual approaches to expanding the field of applying minimum invasive surgery and to substantially improving the results of management.

Craniocerebral Trauma↗

[The differentiated treatment of traumatic intracranial hematomas].

The article deals with the analysis of the clinical and computed tomography data, treatment, and outcomes in 94 patients with traumatic intracerebral hematomas (TICH). The indications for nonoperative treatment of TICH are a level of consciousness of the patient of no lower than 10 marks of the Glasgow coma scale, hematoma diameter of less than 4 cm, and the absence of clinical and computed tomography signs of brain stem compression. Osteoplastic trephination and encephalotomy is the principal surgical method. Puncture evacuation of the TICH may be resorted to if more than three fourths of its volume can be aspirated. The stereotaxic method is recommended for removal of TICH situated in the region of the basal ganglia.

[Basilar skull fractures: clinical and prognostic aspects].

The incidence of basilar skull fractures and their clinical and prognostic aspects were studied in victims with varying severity of brain injury. Of 947 cases, 449 (46%) patients were diagnosed as having basilar skull fracture and 181 (18.6%) had basal spinal fluid leakage. The frequency of fractures and basal spinal fluid leakage increased in proportion to the severity of brain injury. The above 14-day history of spinal fluid leakage was an absolute indication for surgical fistula closure.

[Effect of nitroammofoska on the aldolase activity of the normal hemolymph in the mollusk Planorbarius banaticus and in trematode invasion].

The infection of Planorbarius banaticus with sporocysts of Cotylurus cornutus is accompanied by an increase in aldolase activity of molluscs' haemolymph of 1.2 fold. In solutions of nitroammofoska (0.1, 1 and 10 mg/1) the activity of this ferment in infected individuals increases much higher than in non-infected ones. This results in fast carbohydrate expenditures by molluscs, intermediate hosts of trematodes, and their death from exhaustion.

[Volume of surgical procedures in severe craniocerebral injuries].

The case records of 3,150 patients with severe forms of craniocerebral injury (intracranial hematomas, contusion and crushing of the brain) are analysed. The causes of fatal outcomes in surgical management of this contingent of patients are considered. Not only the time factor (operation performed within 3-6 hours after admission) but also the volume of the surgical intervention are determinant in the treatment of severe craniocerebral injury. The operation must be radical with removal of the hematoma and foci of brain contusion and must include measures for relieving the CSF system and freeing the brain stem from compression; a system of irrigation and active aspiration from the region of the cerebral wounds must be applied in the postoperative period.

[The differential treatment of focal lesions of the frontal lobes].

The differential policy of surgical and drug therapies was evidenced by comprehensive clinical, computed and magnetic resonance tomographic studies and follow-up of 113 patients with focal frontal lobar lesions. Five-year follow-up findings show that mild and moderate posttraumatic focal changes and mild posttraumatic diffuse changes detected by CT and MRT data favour a better clinical recovery and satisfactory social and working rehabilitation in most victims with crushing foci and intracerebral hematomas of the frontal lobes.

[The diagnosis and surgical treatment of posttraumatic hydrocephalus].

Posttraumatic hydrocephalus is an active progressive process of excessive accumulation of cerebrospinal fluid due to liquorodynamic disturbances following cerebrocranial injury. In addition to clinical assessments, it is necessary to use special diagnostic techniques to define indications for bypass surgeries which are an effective therapy for progressive posttraumatic hydrocephalus.

[The effect of shunting operations on the psychopathological symptoms of posttraumatic hydrocephalus].

A total number of 35 patients underwent CSF diversion procedures for posttraumatic hydrocephalus (HCP). To facilitate the analysis of correlation between the onset and clinical course of HCP and psychopathology, all patients were divided into 2 groups. Group 1 consisted of patients who developed HCP within the first 6 months after head injury interfering with the psychiatric recovery. Group 2 was formed by patients who developed HCP after being stable for more than 6 months after injury and having some sort of psychiatric condition (e.g., vegetable status, dementia) before HCP was evident. We considered surgery to be effective if patients had signs of postoperative psychiatric improvement. This was demonstrated in 68.8% of the cases. Mental changes were more prominent in Group 1 vs Group 2. Worsening of the condition was noted only in six patients, all from the second group. It was transient in 4 cases and progressive in 2 cases. All complications occurred due to intracranial infections.
