Evaluation of the 1-year clinical performance of dentin-bonded ceramic crowns and four case reports.

OBJECTIVE: Dentin-bonded crowns are a comparatively recent addition to the dentist's armamentarium. Although 1-year data cannot predict the long-term performance of restorations, it may be useful for new treatment modalities, given that it will describe early, catastrophic failures. In this study, the 1-year performance of 109 dentin-bonded crowns was assessed. METHOD AND MATERIALS: The crowns were placed in two locations (Glasgow, Scotland, and Belfast, Northern Ireland) for 27 patients. The principal reasons for crown placement were tooth substance loss, esthetic reasons, and tooth fracture. Minimal preparations were used. Ninety-eight crowns were available for review at 1 year. RESULTS: Ninety crowns were found to be intact, six had a crack visible on transillumination and two had fractured. No crowns had debonded. No secondary caries was noted. No unsatisfactory scores were recorded for marginal adaptation, adjacent gingival health, or color match. CONCLUSION: The dentin-bonded crowns placed during this study provided good overall performance at 1 year.

The four generations of dentin bonding.

The achievement of an adhesive bond between enamel and dentin and restorative materials has been an objective for generations of dental research workers. Dentin bonding agents have been classified into generations, with earliest generations showing unreliable bond strengths. Some currently available systems show in vitro shear bond strengths to dentin which are similar to the bond strength to enamel, this being considered an ideal property. Alongside other previously recognized ideal properties, appropriate criteria for state-of-the-art dentin bonding systems at the present time may be considered to include the ability of systems to operate in moist environments and be technique insensitive.

Resin luting materials: the current status.

Compared with composite resin restorative materials, which have evolved considerably since their introduction over two decades ago, the development of resin-based luting materials has been slow. Only with the introduction of restorative techniques such as resin-bonded bridgework, porcelain veneers, and composite resin or ceramic inlays, have resin-based luting materials gained importance.

Sandblasting and tin-plating-surface treatments to improve bonding with resin cements.

The superior cementation strengths of the adhesive resin cements can now be used in the dental surgery for posts, crowns and bridges and for intra-oral repairs to fractured porcelain fused to metal crowns or bridges, thanks to the availability of miniature sandblasters and portable tin-platers. The author describes the techniques involved.

An unusual infra-orbital foreign body.

A case report of a patient with a lower eyelid wound with a retained foreign body is given. The localisation, treatment and complications of orbital foreign bodies are discussed.

Contemporary concepts in the pathogenesis of the Class V non-carious lesion.

The aetiology and pathogenesis of non-carious loss of tooth tissue are not fully understood, especially with respect to the cervical area of teeth. In this article the authors describe the current state of knowledge and emphasize the practitioner's duty to become aware of the pathogenesis of these lesions.
