[A study of mydocalm efficiency in the treatment of chronic headache of tension].

Mydocalm was used for the treatment of 31 patients, mean age 45 +/- 4 years, with chronic headache of tension in a dosage of 150 mg 3 times per day during 4 weeks. Twenty-six patients have completed the therapy course. Before- and after treatment, neurological status, dynamics of psychoautonomic disorders, an extent of anxiety and depression, quality of sleep, quality of life, pain syndrome measured by a number of tests, were assessed, along with electromyography and examination of nociceptive reflex. Subjective evaluation of the drug efficacy was taken into account as well. A marked positive effect of mydocalm was observed in 70% of patients, moderate effect--in 30%. A predictor of mydocalm efficacy was severity of muscle tonic syndrome.

[A psychoneurophysiological evaluation of non-specific cerebral systems in patients with permanent autonomic disorders of psychogenic etiology during therapy].

Fifteen patients with permanent vegetative disorders combined with a pronounced anxiety were examined before and after treatment by the Nott drug. The clinical-and-neurological technique, psychometric evaluation of cognitive functions and the neurophysiological method of acoustic cognitive induced potentials P300 were made use of. Before treatment, pronounced vegetative anxiety-related and cognitive disorders were observed in patients. A reliably decreased amplitude of the P300 peak was noted in patients versus healthy subjects while registering the cognitive induced potentials. After treatment, there was a reliably decreased severity of vegetative, anxious and cognitive disorders in patients; the P300 amplitude went up reliably. Supposedly, the changed P300 amplitude, as observed in patients with permanent vegetative disorders; denotes the malfunction in non-specific limbic-reticular brain structures. A growing P300-peak amplitude combined with the arrest of psychovegetative and anxious disorders and with an improvement of cognitive functions in patients due to therapy are indicative of a lower degree of the functional disintegration in the non-specific limbic-reticular brain structures. Finally, the method of endogenous induced P300 potentials can be used as a diagnostic tool in the objective evaluation of a condition of non-specific brain systems in patients with the psychovegetative syndrome; it can also be an objective criterion in the efficiency assessment of a conducted therapy.

[Assessment of phenomenology of attacks of atrial fibrillation before and after treatment with clonasepam].

AIM: To assess phenomenology of attacks of atrial fibrillation using psychovegetative index (PVI) and to compare PVI values obtained before and after treatment with clonasepam in patients with paroxysmal atrial fibrillation. MATERIAL: Patients with nonrheumatic atrial fibrillation (n=105). METHODS: PVI was calculated as ratio of difference between sums (numbers) of cardiac and noncardiac symptoms to sum (number) of cardiac symptoms. RESULTS: Psychovegetative disturbances predominated during attacks of atrial fibrillation and because of that PVI was less than zero. Greatest values of PVI were noted during attacks of idiopathic atrial fibrillation, in women, in patients with high initial level of personal anxiety, in patients with frequent and nocturnal attacks. CONCLUSION: Patients with negative or zero PVI require drugs for correction of psychovegetative disturbances. Therefore it is expedient to use PVI in patients with attacks of atrial fibrillation to select candidates for psychotropic therapy and to evaluate its efficacy.

[Comparative analysis of phenomenology of paroxysms of atrial fibrillation and panic attacks].

AIM: To study phenomenology of attacks of atrial fibrillation (AF) and to compare it with phenomenology of panic attacks for elucidation of pathogenesis of atrial fibrillation and for elaboration of rational therapeutic intervention including those aimed at correction of psychovegetative abnormalities. MATERIAL: Patients with nonrheumatic paroxysmal AF (n=105) and 100 patients with panic attacks (n=100). METHODS: Clinical, cardiological and neurological examination, analysis of patients complaints during attacks of AF, and comparison them with diagnostic criteria for panic attack. RESULTS: It was found that clinical picture of attacks of AF comprised vegetative, emotional and functional neurological phenomena similar to those characteristic for panic attacks. This similarity as well as positive therapeutic effect of clonazepam allowed to propose a novel pathogenic mechanism of AF attacks. Severity of psychovegetative disorders during paroxysm of AF could be evaluated by calculation of psychovegetative iudex: CONCLUSION: Psychovegetative index should be used for detection of panic attack-like component in clinical picture of AF paroxysm and thus for determination of indications for inclusion of vegetotropic drugs, e. g. clonazepam, in complex preventive therapy.

[Computer tests for research of the oculomotor reactions in patients with complains of dizziness].

New methodological principles for a study of vestibular function, intersensory interactions, and eye tracking function are proposed by parameters of spontaneous oculomotor responses and those induced by visual and vestibular stimuli using the series of computer stimulating programs, which provided polymodal, separate and combined stimulation of the visual and vestibular inputs for the OCULOSTIM unit. The method suggested allows to objectivity the subjective complaints of dizziness and equilibrium disturbances, to differentiate diagnosis of cerebral organic process and psychogenic disease of central nervous system (CNS), and to specify physiological systems involved in their pathogenesis as well as to diagnose subclinical cerebral vestibulopathy. Qualitative and quantitative characteristics of eye tracking movements with and without a visual background (retinal optokinetic stimulation) are the most informative in the differential diagnosis of organic and functional psychogenic CNS disturbances.

[Betaserc treatment of psychogenic vertigo].

An effect of betaserc on the character and intensity of vertigo (V), psycho-autonomic state of patients, vestibular functions and oculomotor system, using computer stimulating programs, has been studied in 39 patients with V. After 4 weeks of the treatment, betaserc exerted a positive effect on patients; however, the optimal effect was shown in patients with psychogenic V (73%). In this group, betaserc reduced both V and other psycho-autonomic disturbances. After betaserc treatment, patients with psychogenic V demonstrated a normalization of the vestibular responsiveness parameters in 70% of the cases and of the parameters of spontaneous oculomotor activity and pursuit eye function in 40 to 90%.

[Spectral analysis of heart rate variability in diagnosis of vegetative autonomic dysfunction in patients with attacks of atrial fibrillation.].

AIM: Elucidation of possible role of suprasegmental and segmental autonomic vegetative disturbances in pathogenesis of paroxysmal atrial fibrillation. MATERIAL: Untreated patients (n=22) with nonrheumatic paroxysmal atrial fibrillation and 10 healthy subjects. METHODS: Clinical cardiological and neurological examination, assessment of the state of vegetative nervous system, emotional sphere, patterns of sleep, and spectral analysis of heart rate variability. RESULTS: According to data of spectral analysis of heart rate variability patients with paroxysmal atrial fibrillation were characterized by marked vegetative dysfunction which was more pronounced in women, patients older than 52 years, with higher level of anxiety, with duration of symptoms longer than 6 years, and with predominantly nocturnal attacks. CONCLUSION: The data obtained should be considered in selection of therapy aimed at prevention of attacks of atrial fibrillation.

[Use of clonazepam for treatment of patients with paroxysmal atrial fibrillation with regard to their psychoautonomic status].

AIM: To study the efficacy of clonazepam in patients with paroxysmal atrial fibrillation (PAF) with reference to their psychovegetative status. MATERIAL AND METHODS: 50 patients with PAF of non-rheumatic origin entered the study group, 20 healthy examinees served control. All of them were examined using a clinicocardiological, cliniconeurological tests; their autonomic nervous system, emotional sphere, sleep disorders were studied before and after clonazepam treatment in a dose 0.5 mg 2 times a day for 30 days. RESULTS: A positive effect of clonazepam was noted in 20 (80%) patients with PAF, less frequent paroxysms occurred in 12 (48%) patients, in 5 patients they stopped, 12 (48%) patients tolerated the paraxysms better. The paraxysms became shorter in 10 (40%) patients, 10 (40%) patients felt better in the postparoxysmal period. A marked relief of psychovegetative syndrome, improvement of activity, mood and quality of sleep were noted in all the patients. CONCLUSION: Clonazepam is recommended for patients with PAF to improve antiarrhythmia therapy.

[Clinical and psychological analysis of migraine population].

The study presents a clinical analysis of a large population of migraine patients (n = 320). Computed database used in the study included over 100 clinical parameters and was based on analysis of primary medical records. As a result of statistic processing of the migraine archive, a general review of clinical picture and migraine course in patients from Moscow and Moscow region community and analysis of the peculiarities of migraine heredity, concomitant symptoms, provoking factors, co-morbid diseases, state in attack-free period, psychological characteristics and quality of life are provided. Factors determining course severity and quality of life in migraine are discussed.

[Psychoautonomic correlations in patients with paroxysmal atrial fibrillation].

AIM: To study psychovegetative correlations in patients with paroxysmal atrial fibrillation (PAF). MATERIAL AND METHODS: The trial included 50 patients with non-rheumatic PAF and 20 healthy subjects. Clinical cardiological and cliniconeurological examinations, assessment of autonomic nervous system, emotional sphere, sleep disorders were performed. RESULTS: The diagnosis of marked psychovegetative syndrome was made in all PAF patients. It manifested stronger in older women, young men, survivors of myocardial infarction or acute disorders of cerebral circulation, in frequent paroxysms of cardiac fibrillation, its long-term history, in night PAF attacks, in sleep apnea. CONCLUSION: The policy of antiarrhythmic antirecurrence therapy should consider modalities correcting psychovegetative disorders.

[Therapy of autonomic disorders by xanax (alprazolam)].

The paper presents an open noncomparative investigation of 36 patients with different manifestations of the syndrome of autonomic dystonia. 20 patients (group 1) had permanent autonomic disorder in context of generalyzed anxious disorders, 16 patients (group 2) had panic attacks. The examination was performed before and 4 weeks after monotherapy with xanax (1.5-2.5 mg/day). Clinical-neurologic study estimated both presence and a degree of manifestations of the syndrome of autonomic dysfunction, hyperventilatory syndrome and sleep disorders. Psychologic investigation included estimation of anxiety according to Spilberg's test, depression according to Beck's scale; SCL Scale was also used. Algesic syndrome was estimated by complex algesic questionnaire. Neurophysiologic study determined a contingent negative deviation and nociceptive flexory reflex. A positive therapeutic activity of xanax was established. The highest therapeutic effect was achieved in group 1 (83%) using lower doses (1.5 mg/day). In group 2 higher doses were needed (2.5 mg/day). In this case the effect was achieved in 83% of the cases, but full absence of panic attacks was observed only in 25% of the patients. Predictors of the drug's efficiency appeared to be short duration of the disease, slight manifestation of depression and absence of the algesic syndrome.

[The influence of phototherapy on psycho-autonomic syndromes].

Therapy by bright white light was applied to 51 patients. 4 blocks of psychoautonomic syndromes of neurotic nature were analysed: 1) pronounced (14 cases) and mild (12) hypothalamic dysfunction; 2) moderate and pronounced depression (21) and the state without depression (9); 3) headache of different types--37 patients, including 21 ones with chronic headache of effort, combined with migraine--5 cases, or combined with vascular headache--11 patients; 4) paroxysmal disorders (43 individuals: 25--with typical panic attacks, 18--with atypical panic attacks). More pronounced positive effect was found in the group with atypic PA and in the group with slight hypothalamic dysfunction, accompanied by solitary neuro-endocrine symptoms with moderate or severe depression, with chronic headache of strain including a combination with migraine. Neuroendocrine, motivative, psychovegetative, algesic and psychopathologic symptomatology was decreased significantly in all the groups with positive effect together with the improvement of the objective psychophysiologic indices. Intrahemispheric interactions were also improved, exactly: the power of frequency EEG spectrum increased, in general, because of both the increase of the slow rhythms from both sides and the approach of the coefficient of asymmetry to the control. The least effect was observed in the patients with senesto-hypochondric syndrome, with prevalence of anxiety over depression, with rude hypothalamic dysfunction, with vascular headache, with typic PA.

[Emotional-personality condition of patients with some gastroenterological diseases].

Characteristics of personality and emotional condition were studied in 160 patients from Moscow (Russia) and Chardzhou (Turkmenistan) with various gastroenterological diseases: duodenal ulcer, chronic gastritis, chronic cholecystitis, irritable colon syndrome. Actual psychogenia occurred frequently in all the patients, children's psychogenia was more frequent in Moscow patients with duodenal ulcer. In patients with duodenal ulcer and chronic gastritis both from Moscow and Chardzhou the incidence of stress in everyday life was rather high. All the examinees suffered from mild or moderate depression found more frequently in Moscow patients with chronic gastritis.

[The effect of phototherapy on psychoautonomic disorders of a neurotic nature].

Bright white light therapy was applied to 51 patients with syndrome of autonomic dysfunction of neurotic origin (1 hour of light exposure every day in the morning, during 2 weeks, 60 sm from the lamp, 3300 lux). Improvement occurred in 52% of the patients (responders--group 1, nonresponders--2). Changes occurred in nearly all symptoms: neuroendocrine, motivation, psychoautonomic, pain, psychopathologic. After the treatment in group 1 there was an increase of power of EEG spectrum, intensification of manifestations of the slow activity and decrease of the fast one from the two sides, an approach of the coefficient of asymmetry to the control levels as well as elevation of the urine excretion of metabolites of both catecholamines and serotonin. Initially higher power of EEG spectrum in group 2, became still more increased due to intensification of manifestations of theta and beta-2 rhythms from the two sides. Meanwhile coefficient of asymmetry was sharply decreased as well as general secretory activity inhibited. There were such symptoms and indices which had changed either negatively or positively under the influence of phototherapy.

[The treatment of tension headache with sirdalud].

For the first time sirdalud was used in the treatment of 32 patients with chronic form of tension-type headache. The preparation was applied during 14 days (4 or 6 mg daily). Positive effect was observed in 75% of patients (a decrease of both headache and tension of pericranial muscles). However, sirdalud was so effective neither in patients with pronounced depression nor in ones without pericranial muscular tension. The drug had a number of psychotropic effects: it decreased anxious-depressive manifestations, normalized night sleep, significantly increased the thresholds of both ache and nociceptive flexor reflex.
