[Endoscopic closure of a recurrent tracheoesophageal fistula].

We report a successful endoscopic obliteration of a large recurrent tracheo-esophageal fistula (diameter 0.6 cm, length 2 cm) in a 12-year old girl, using a combination of N-butyl-2-cyanoacrylate followed by polidocanol injected through a polyethylene catheter. The severe pulmonary infection, that rendered surgery potentially life-threatening, resolved after endoscopic closure. The girl remained asymptomatic for 14 months after the first obliteration. Chronic coughing was the symptom of relapse. A follow-up endoscopy revealed that most of the N-butyl-2-cyanoacrylate had disappeared from the fistula, whose endoscopic appearance was unchanged. A second obliteration with the same products was performed. The girl has again been asymptomatic for 10 months. The endoscopic obliteration is a worth-while technique, to be considered as an alternative to surgery in patients presenting with a complicated recurrent tracheo-oesophageal fistula.

Spinal meningeal cyst.

A case of meningeal cyst of the spine in the thoraco-lumbar region is described. Although water-soluble myelography may disclose a filling defect and possibly reveal communication between the cyst and the subarachnoid space, computerized tomographic myelography more readily reveals communication between the cyst and the subarachnoid space. This finding allows accurate diagnosis of a spinal meningeal cyst and rules out other mass lesions. A classification comprising three categories is made: spinal extradural meningeal cysts without spinal nerve root fibers (Type I), spinal extradural meningeal cysts with spinal nerve root fibers (Type II) and spinal intradural meningeal cysts (Type III). Final characterization is based on operative inspection and histological examination.

Effect of an apple brandy from Normandy and of ethanol on epithelial cell proliferation in the esophagus of rats.

The effects on cell population kinetics in the distal esophagus of an apple brandy from Normandy (Calvados), of ethanol diluted at a similar concentration, and of saline were investigated in rats. Animals were killed at 0, 6, 12, 18 and 24 h after a single intraesophageal instillation, and at 3 weeks after chronic administration of these solutions. In vivo labeling with [3H]-thymidine and autoradiography were carried out to determine the proliferative parameters. Vincristine sulfate was used to increase metaphase figures during the chronic experiment. Instillation of saline did not modify the parameters under study. Single instillations of both apple brandy and 56 degrees ethanol were followed by a significant increase in labeling (p less than 0.01) and mitotic indices (p less than 0.01). Chronic administration of both apple brandy and 56 degrees ethanol increased the labeling index (p less than 0.01) as well as the metaphase index (p less than 0.01). The proliferative responses after the apple brandy and after 56 degrees ethanol were similar in amplitude. These data suggest that ethanol alone may explain the mitogenic effect of Calvados in the esophagus.

Hydrochloric acid. A trigger of cell proliferation in the esophagus of dogs.

The effect of a single instillation of acid and pepsin on the cell proliferation in the distal esophageal mucosa was investigated in four dogs. The doses of acid and pepsin used were lower than those provoking acute esophagitis and erosions. Using in vitro labeling with [3H]thymidine and autoradiography, the epithelial mitotic and DNA synthesis indices were determined at 0, 4, 8, 12, 16, 20, and 24 hr after instillation of saline, of, acid alone, or of acid with pepsin. Instillation of acid alone was followed, 16 hr later, by an increase (P less than 0.01) in DNA synthetic activity in the proliferative area. A mitotic peak (P less than 0.01) started from the 20th hour. After instillation of acid with pepsin in the same animals, a similar sequence of kinetic phenomena was observed, suggesting that the concentration of pepsin used did not potentiate the stimulating effect of acid on the cell proliferation in this epithelium. Our data indicate that hydrochloric acid stimulates the proliferative activity in the normal esophageal epithelium.

Congenital right-sided diaphragmatic hernia in an elderly patient. A case report.

Congenital diaphragmatic hernia is a rare condition in adulthood. It is even more exceptional when located on the right side. We describe a case of right-sided congenital diaphragmatic hernia in a 74-year old woman. The diagnosis was only made when the patient developed an acute intestinal obstruction after a laparotomy for a gynaecological benign tumour. The treatment of this condition is discussed. In our opinion, in elderly, the advantages of an elective operation in asymptomatic congenital diaphragmatic hernia have to outweigh the risks of the operation. However, if the patient shows symptoms of pulmonary dysfunction or motility disorders of the gut, or even when he has to undergo a laparotomy for a different reason, we advise to repair the hernia in order to prevent complications.

Asymptomatic aneurysm of the hepatic artery. Management options.

Hepatic aneurysms are rare. The majority of patients present acutely with aneurysm rupture, with an attended high mortality. Diagnosis is difficult and often delayed, owing to the non-specific symptoms and lack of clinical findings. We present three cases of hepatic artery aneurysm diagnosed in a pauci- or asymptomatic stage, illustrating the different therapeutic options described when these aneurysms are diagnosed in this stage: expectant management, embolization, or surgery.

Postoperative duodenal fistulas. A report of 23 cases with emphasis on the surgical treatment by the Roux-en-Y operation.

A series of 23 patients-15 with a spontaneous postoperative duodenal fistula and 8 with a catheter duodenostomy fistula-admitted in both departments between 1974 and 1983, is reviewed. Overall mortality rate was 9%. Using conservative management and parenteral nutrition, spontaneous closure of the fistula was observed in 78% of the cases after an average duration of 33 days. Three patients with an intractable fistula were successfully managed by surgical internal drainage using a Roux-en-Y jejunal loop. Healing time of catheter duodenostomy fistulas was similar to that of spontaneous fistulas. Therefore, deliberate creation of such duodenal fistulas should be carefully considered. The Roux-en-Y operation is recommended for those fistulas that do not heal with conservative management. The latter technique could perhaps be used primarily for the closure of difficult duodenal stumps.

Meckle's diverticulum in association with Crohn's disease.

Although it is not uncommon to encounter a Mecklel's diverticulum in a patient with Crohn's disease, the involvement of this diverticulum by Crohn's disease is less common. A well performed radiological small intestinal study strongly suggests the diagnosis, which is definite after histologic examination of the reported.
