OBJECTIVE: To describe food habits of school-aged population having school meals and to evaluate perceived quality of the service. STUDY DESIGN: Descriptive cross-sectional study. SETTING: Community School lunchrooms in different Spanish regions. PARTICIPANTS: Random population sample of children and young people (3-16 year) having school meals in Spain. MEASUREMENTS: The study protocol included socio-economical data, food consumption and dietary habits at the school and out of the school. Information was collected by means of two questionnaires: one completed by children at school and a second one completed by the family at home. RESULTS: Valid response was collected from 322 children and 212 families, a response rate of 96.1% children and 63.3% of the families. 88% of the children reported daily consumption of breakfast, 45% used to have a midmorning snack, which consisted of a sandwich (29%); sweet bakery products (27%) or biscuits (15%). 45% of the 12-16-year-old old group reported eating only half of the portion served at lunch in the school, mainly due to disgusting taste or preparation (50%). In the school menus vegetables and fish were offered less frequently than other food groups. 81% of the children referred having an after-school snack in the afternoon, consisting of a sandwich (65%); bakery products (20%); bread and chocolate (20%); yoghurt or fruit (20%). CONCLUSIONS: School meals provide a wonderful opportunity to widen the variety of foods in the daily diet of children. Recommendations about school meals should address the quality and variety of the foods served for a healthy diet and stress the educational dimension.
OBJECTIVE: Our objective was to obtain spirometric reference values for children. PATIENTS AND METHODS: We performed a transversal study in schools in Bilbao of children of both sexes that were between 7 and 14 years of age, non-smokers and healthy (according to a questionnaire and physical examination). A sample of 657 subjects was calculated based on the standard deviations of height for each sex and age group. The sample units were the schools and they were selected randomly. The following data were collected: age, sex, weight, height, FVC, FEV1, FEV1/FEC, PEF, FEF25, FEF50, FEF75 and FEF25-75. Forced spirometry was carried out according to the ATS guidelines by using a MasterScreen model 4.10.b (Jaeger, Germany). A descriptive statistical study was performed, as well as a normality test and linear regression. RESULTS: We studied 765 subjects (415 boys and 350 girls). The natural logarithm of the spirometric variables was taken to obtain a Gaussian distribution. These variables were related to age, weight and height. The relationship improved when associated with sex, with the best association being between height and sex. The equations obtained were only valid for FVC and FEV1, and were as follows: in boys FVC(L) = -1.968 + 0.020 height and FEV1(L) = -1.831 + 0.018 height and for girls FVC(L) = -1.879 + 0.019 height and FEV1(L) = -1.809 + 0.018 height [R2 = 0.893, 0.891, 0.868 and 0.871, respectively]. CONCLUSIONS: The pediatric reference values for forced spirometry were established for our population, complying with the current applicable guidelines.
OBJECTIVE: The objective of this study was to know the prevalence and severity of bronchial asthma in 13-14 year old children living in Bilbao, a seacoast industrial town in the North of Spain. PATIENTS AND METHODS: A sample of 13-14 year old children living in Bilbao was studied. Random sampling amongst school centers was done. Fifty-eight centers were included and a total of 3,578 children between 13-14 years old were studied. The International Study of Asthma and allergy in Children (ISAAC) written and video questionnaires were used. Statistical analysis included bar diagrams, qualitative variable frequencies and the Chi 2 test to study the contrast and the association between variables. The statistical significance level was 95%. The relative risk according to sex was obtained. RESULTS: The questionnaires that were valid (3,213) represented 89% of the selected sample. The accumulated asthma prevalence was 18.9%. The actual asthma prevalence (asthma during the last year) was 11.9%. Severe asthma was present in 11.9% of the children. Asthma had been previously diagnosed in 16.3% of the children. The relative risk of being diagnosed was 1.39 for males. CONCLUSIONS: These figures are similar to those obtained in other populations with the same geographic and climate characteristics.

The prevalence of hepatitis B virus (HBV) infection was studied in 346 subjects (148 employees and 198 mentally retarded) in four institutions for the mentally subnormal in Vizcaya-Spain. The prevalence of markers in the overall group was 32.6%; 13.5% in the employees and 46.9% in the mentally retarded. Early age and institutionalization time increases the risk of infection in the mentally retarded group, but not in the employees group. 220 subjects (118 employees and 102 mentally retarded) had negative HBV markers and were vaccinated with recombinant-DNA hepatitis B vaccine. The immunogenicity was 80% and the weight-height index the only factor implicit in the group of subjects no seroconverters. The reactogenicity of vaccine was 6.7%.
Prophylaxis of febrile convulsions has been very controversial. Often this prophylaxis is given based upon subjective criteria and even in the same Department of Paediatrics, criteria are not uniformly carried out. We consider of great interest to value some circumstances of the febrile convulsions, which could be worth for valorating recidives. We have quantified some parameters, that can be obtained easily. Those have systematically been used in our Department, leading us to establish and score to determine the risk factors that lead us to use or not a prophylactic treatment in children with febrile convulsions.
Five cases of perinatal HBV infections of vertical transmission (mother-child) are reported. The authors comments the most important clinical and epidemiological characteristics of this way of transmission in paediatrics patients and emphasized the interest that has the screening in pregnant women. Finally, the guide-lines of neonatal prophylaxis recommended is described.
