Experimental trichinellosis in horses: biological and parasitological evaluation.

Three groups of three horses each were, respectively, infected with 5000, 20,000 and 50,000 larvae of Trichinella spiralis. The strain used was isolated from a human biopsy during horsemeat-related outbreaks of trichinellosis in France. Transient muscular disorders were only observed in two of the horses infected with 50,000 larvae but none of the horses had fever. A significant increase in blood eosinophils was noticed in 5 horses. Serum LDH, aldolase and CPK peaked at the fifth week post-infection. Specific IgG assayed by indirect immunofluorescence and ELISA, appeared 2-5 weeks post-infection and disappeared between 16 and 40 weeks. The distribution of T. spiralis larvae was maximal in the tongue, masseters and diaphragm, but a large decrease in the number of larvae recovered from the muscles was noticed among the horses slaughtered at the beginning and end of the experiment. In muscular histological sections, larvae were observed in an intramyofibrillar position and were surrounded by a mild to severe inflammatory reaction.

Is fresh or frozen semen to be used in in vitro fertilization with donor sperm?

Sixty-six partners of either severely oligoasthenospermic or azoospermic men were treated by in vitro fertilization with donor sperm (IVF-D), usually (86%) after failure of artificial insemination with donor sperm. One hundred twenty-nine IVF trials were performed with either fresh (59%) or frozen-thawed (41%) donor sperm. Characteristics of sperm preparations were significantly lower in the frozen-thawed group (P less than 0.001). In the latter group, fertilization rates had slightly decreased (but without statistical significance); embryonic vitality scores and cryopreservable spare embryos had significantly decreased (P less than 0.05). However, cumulative ongoing pregnancy rates were strictly equivalent in both groups (40% after three trials). Frozen-thawed sperm thus can be substituted for fresh donor sperm without entailing impairment of the outcome of IVF, and this method must be preferred for its greater safety regarding transmission of human immunodeficiency virus.

Fertility prognosis in IVF treatment of patients with cancelled cycles.

Among 429 patients admitted in our in-vitro fertilization (IVF) programme during the last 3 years, 106 experienced cancellation of one or more treatment cycles. A low oestrogen (E2) response represented by far the main reason for cancellation (144/184). Significantly lower clinical pregnancy rates were found in the group of patients with low E2-cancelled cycles (E2CC) as compared to the rest of the IVF population (13 versus 37%). Further comparison between these groups showed only little or no difference in the mean age of patients, basal FSH levels, number of egg collections, rate of oocytes obtained and fertilized per trial and mean embryonic vitality score. A large majority of E2CC patients were found to belong to the tubal infertility group (75%) as opposed to a lower proportion of tubal cases among other IVF patients (56%). E2CC tubal patients showed a lower pregnancy rate than other cases with abandoned cycles and also than other tubal patients (8.9 versus 25 and 35%, respectively). Mean numbers of previous laparotomies, of attempts at oocyte retrieval and of oocytes recovered per trial were similar in tubal patients with or without E2CC. However, in the E2CC tubal group mean age and basal FSH levels were somewhat higher, whereas E2 peak values and fertilization rates were lower, leading to a greater proportion of trials without embryo transfer. Embryonic scores and the E2/P ratio at day 3 of the luteal phase did not differ significantly. It may be concluded that cancelling of a cycle because of a low E2 response in a patient with tubal infertility is indicative of a poor prognosis in further IVF trials.

[Should one use fresh or frozen sperm in in-vitro fertilization with donors?].

Between 1984 and 1987, 66 women whose partner had severe oligo-asthenoteratospermia or azoospermia underwent in vitro fertilization (IVF) with donor sperm. The mean duration of sterility was 7.5 +/- 3.2 years. Investigations performed in the women showed no abnormality in 30%, pure endocrinopathies in 21% or tubal diseases isolated or associated with other pathologies in 40%. Most of these women (86%) had had previous unsuccessful inseminations with donor sperm. Our study involves 129 attempts of IVF with donor sperm (1 to 3 attempts per patient); the sperm was fresh in 59% of the cases and cryopreserved in 41%. Initial and final motility and sperm density are significantly lower for cryopreserved sperm (p less than 0.001). The fertilization rate is lower with cryo-preserved sperm but the difference is not statistically significant. Moreover, the score of embryonal vitality and the number of embryos that can be cryopreserved are lower with cryopreserved sperm (p +/- 0.05). However the number of pregnancies is identical with one or another type of sperm (40% of the women after 3 IVF). To conclude, cryopreserved sperm may be used for IVF without lowering the percentage of success. In fact, the fertilization rate, the embryonal score and the number of embryos to deep freeze are slightly less with cryopreserved sperm and this difference could reveal to be significant with a larger cohort of patients. But the advantage of fresh sperm must be balanced by the risk of AIDS contamination, avoided with cryopreserved sperm.

[RAST and acari in stored goods].

During a multicentre trial, responses of serum from 60 asthmatic children with allergy to the mite Dermatophagoides pteronyssinus (D1) were compared to D. farinae (D2) and 4 storage mites. The known cross reaction between D1 and D2 was confirmed but no such reaction was found between D1 and/or D2 and the storage mites. Because the important cross reactivity between storage mites, the choice of the allergen will depend on the historical and epidemiological data of the disease.

Growth hormone dependency of the effect of human serum on thymidine uptake by lectin-activated human lymphocytes.

This study of the effect of serum dialysis on thymidine incorporation into lectin-activated blood lymphocytes cultured in vitro showed that (1) after serum dialysis 3H-thymidine incorporation is very low or absent; (2) the addition of L-glutamine to dialyzed sera leads to recovery of 18% of 3H-thymidine uptake, and (3) addition of L-glutamine reduces the difference in the stimulating effects of acromegalic and hypopituitary sera observed before dialysis in a medium free of L-glutamine. Since the plasma level of glutamine is increased in acromegalics and reduced in hypopituitary patients, it is suggested that this amino acid acts as a growth hormone dependent factor or cofactor in the regulation of cell growth.

[Assistance in human oocyte fertilization using micromanipulation].

In recent years different types of micromanipulation have become available to improve human gametes interaction, particularly in cases of severe sperm defect. Direct injection of one spermatozoon into the cytoplasm of the egg has not been applied in man, not only because of technical difficulties and low efficacy, but also in view of the theoretical risk of chromosomal anomalies. However, subzonal sperm injection (SUZI) and partial zona dissection (PZD) are now used routinely by a number of IVF centers. In our hands this latter method has increased significantly the fertilization rate (21.6 vs 2.9 p.cent among intact oocytes) and has allowed to obtain 3 pregnancies (of which 2 ongoing) after 28 attempts. Our results and those of the literature are discussed with respect to the risks and advantages linked to these techniques.

[Low serum thyroxin and thyroxin-binding globulin in neonatal respiratory distress (author's transl)].

Measurements of thyroxin (T4), thyroxin-binding globulin (TBG) and TSH were carried out in 34 full-term newborns, 21 prematures and 11 neonates with respiratory distress (6 with hyaline membrane disease) at 5 days of age. In cases with neonatal respiratory distress and to a lesser extent in prematures, low T4 due to a decrease of TBG was found, TSH being identical in all groups. The positive correlation between TBG and transferrin suggests a disturbance in hepatic synthesis. The authors conclude that, in cases with neonatal respiratory distress, low T4 does not mean hypothyroidism and does not require a treatment, provided TSH remains within normal limits.

Unpredictable cases of complicated ovarian hyperstimulation in IVF.

OBJECTIVE: To characterize a subpopulation of complicated cases of ovarian hyperstimulation syndrome (OHSS). METHOD: Descriptive retrospective study. RESULTS: 0.75% of our IVF-ET population suffered from OHSS. Among this group, 33% did not exhibit any recognized risk criteria of OHSS in terms of infertility characteristics and ovarian response to exogenous gonadotrophins. Only severe (ascites) OHSS cases were considered (n = 5) in this study. Previous IVF-ET attempts had been uneventful and during the complicated trial, estradiol peak levels and numbers of oocytes retrieved remained below 2,500 pg/mL (conversion factor to SI unit, 3.671) and 10, respectively. In all cases, the luteal phase was supplemented by hCG and all patients became pregnant. A threshold level of exogenous and/or endogenous hCG seems to be responsible for the occurrence of OHSS. CONCLUSION: One-third of the patients developing an ovarian hyperstimulation syndrome after IVF-ET had not previously shown risk criteria. A causal role of exogenous and/or endogenous hCG is suggested.
