PET studies of [18F]methyl-MK-801, a potential NMDA receptor complex radioligand.

Using positron emission tomography (PET), the potential of 18F-labelled fluoro-methyl-MK-801([18F]FMM) as a radioligand for in vivo studies of the NMDA receptor complex was investigated in baboons. In baseline conditions, there was a slight differential retention of [18F]FMM in cerebral cortex and striatum relative to cerebellum, compatible with specific binding. However, neither pretreatment with pharmacological doses of MK-801 or phencyclidine, nor severe, transient brain hypoxia, were able to clearly alter [18]FMM brain regional kinetics, indicating limited usefulness of this radioligand for in vivo PET investigations of the NMDA receptor.

[Calcifications after intra-disk injection of triamcinolone hexacetonide in lumbar disk hernia. Evaluation of therapeutical results in 3 years].

The development of disk or epidural calcifications is a frequent possibility following intra-disk injection of triamcinolone hexacetonide. It was found 10 times in 26 follow-up CT scans obtained 2 to 3 years after the injection. These calcifications are often clinically silent, but they sometimes accompany a recurrence of the initial painful symptomatology. Furthermore, evaluation at 3 years of therapeutic results in a previously published series of patients who had received an intra-disk injection of triamcinolone hexacetonide showed a marked decrease in favourable results (30% vs 67% at 6 months). These two arguments: disappointing long term results and possibility of disk calcifications, are felt by the authors to justify abandoning the technique of triamcinolone hexacetonide by intra-disk injection in the treatment of lumbar disk prolapse.

[Prognostic factors in delivery with breech presentation. Apropos of 357 single-fetus pregnancies at term].

For a better definition of the factors which could be at the origin of neonatal distress, 357 cases of mono-fetal term pregnancies with breech presentation were retrospectively studied between January 1st, 1984 and December 31st, 1989. The population is identical to that of all deliveries except for a higher percentage of first pregnancies. The risk of emergency caesarean section mostly concerns large primiparous women, underweight, with normal or large pelvis. Caesarean section is absolutely necessary if the biparietal diameter exceeds 100 mm, if the head is positioned in hyper-deflection and/or if the pelvis is defective. Without being mandatory, the procedure is preferable in case of chronic fetal distress or scarred uterus. On the contrary, neither the age of the mother nor the age of the pregnancy, nor the type of the breech, nor the number of pregnancies have any effect on the prognosis. The current rate of caesarean section is 47.9 per cent with almost complete disappearance of the large breech extraction (0.6 per cent) and the Mauriceau's maneuver (0.3 per cent). The corrected mortality rate is 0.28 per cent. In addition to the parameters considered in our coefficient of risk of breech delivery, it was noted that a pre-pregnancy weight exceeding 70 kg or a weight gain of more than 15 kg represent unfavorable factors. It is desirable to calculate prospectively the severity of the risk in small size cases, when the biparietal diameter ranges between 96 and 100 mm, or when one of the diameters of the bony pelvis is decreased.

Adolescent

[Acute hydrocephalus drained in emergency. Consequence of cerebellar infarction in Haemophilus meningitis].

A 2 year-old child admitted for Haemophilus meningitis was immediately treated by adequate antibiotic treatment. Three days later multiple hypertonic strokes and periodic respiration occurred; a resuscitation was necessary. CAT scan showed an acute hydrocephalus with non visible 4th ventricle and low-density areas in both cerebellar hemispheres allowing the diagnosis of cerebellar infarction. External drainage of CSF was rapidly performed and maintained for 11 days with success. The child was secondarily discharged with temporary cortical blindness and persistent moderate static cerebellar signs. The etiology of the cerebellar infarction was likely to be an arterial thrombosis in the vertebro-basilar area, probably secondary to cerebral arteritis related to Haemophilus.

Comparison of two new tests for rapid diagnosis of respiratory syncytial virus infections by enzyme-linked immunosorbent assay and immunofluorescence techniques.

The sensitivity and the specificity of two new commercial reagent tests, an indirect fluorescent antibody test (FAT) with a mouse monoclonal antibody (MAb) against respiratory syncytial virus (RSV) and an enzyme-linked immunosorbent assay (ELISA) RSV antigen detection kit, were determined by a comparison of results from these tests with those of tissue culture isolation and an indirect FAT with bovine polyclonal antibody (BPA). Of 251 nasal aspirates from infants with suspected RSV infection, positive results were found for 99 (39%) by the FAT-MAb, 93 (37%) by the FAT-BPA, and 87 (35%) by the ELISA; 69 of 240 (29%) were positive by cultures. The FAT-MAb was a more sensitive technique than cultures, with 87% sensitivity for the FAT-MAb and 84% for the ELISA. It was also more sensitive than the FAT-BPA, with 97% sensitivity for the FAT-MAb and 85% for the ELISA. This could be caused only by the distinctive volume of suspended specimens used in these tests. Of 171 negative culture specimens, positive (but not false-positive) results were found for 18% by the FAT-MAb and for 12% by the ELISA. Inversely, 13% of 69 culture positive specimens were FAT-MAb negative and 16% were ELISA negative, emphasizing the importance of tissue cultures for the maximum recovery of RSV, as well as for detection of other respiratory viruses. The FAT-MAb and ELISA were easy to perform and interpret, thus facilitating wider use.

[Rhinovirus and acute respiratory infections in infants].

Human leucocytary interferon (IFN) in a concentration of 2,000 Ul/ml stops the development of 9 Rhinovirus (RV) strains. The use of serum IFN antibodies (SAIF) reverses this effect. Of 256 nasal aspirates culture negative for other respiratory viruses, 40 RV were isolated with SAIF as opposed to 19 without serum anti IFN. The frequency and some clinical aspects are emphasized in infants: 43% had bronchiolitis similar to respiratory syncytial virus (RSV) infections and atopy was frequent.
