Educating the diabetes care professionals in eastern Europe: a Bulgarian experience.

The first educational course for 30 medical doctors and 16 nurses, engaged in outpatient diabetes care in Bulgaria, was carried out with the aim of assessing the existing level of knowledge of diabetes, to implement a training programme matching the assessed needs, and to evaluate the efficiency of the education delivered. The initial level of knowledge was assessed by an anonymous pre-test questionnaire on general issues of diabetes care. After 5 days of training, a post-test questionnaire was administered which included more specific questions on matters that had been taught during the course. The results of the pre-test showed that, on average, 60.3 +/- 20.6 (SD) % of the doctors and 50.5 +/- 25.0% of the nurses gave correct answers. The results of the post-test were significantly improved (73.5 +/- 20.6% for the doctor and 70.8 +/- 23.5% for the nurses), although the post-test questionnaire had been expertly evaluated as more complex and specific. The analysis of the results allowed us to draw general conclusions regarding the educational needs, the optimal structure, and implementation of such training courses in the specific conditions of Bulgaria. Some of the results and conclusions might be useful for other countries from Central and Eastern Europe in their efforts to promote successful diabetes education programmes.

[The place of vacuum extraction in modern obstetrics].

A clinical trial was carried out on the use of vacuum extraction for delivery of 45 women. The aim of the study is to revive again this method for it was rarely used during the last years. The authors remind of the indications and necessary condition for the use of vacuum extraction and report about complications and the course of the placental period. Parallel to this, the 45 babies born with vacuum, are followed up according birth weight, gestational age, clinical status and laboratory data by birth, resuscitation after delivery and birth trauma. By summing up the results the authors divide women (and babies) in 3 groups: deliveries with vacuum extraction (VE), VE+ forceps, Vstimmulation and forceps, Conclusions are made that VE is a useful method nowadays if classical conditions and indications are followed. The babies born by VE are in good status without serious traumas. Vstimmulation + forceps is rejected as more harmful method.

Ionized calcium in patients with chronic renal failure.

Parameters of various renal disorders were studied in 31 patients (17 men and 14 women) who all were in the stage of chronic renal failure (CRF). These parameters included total serum creatinine, ionized calcium, serum creatinine, creatinine clearance and acid-base balance. The results were compared with a control group of 30 healthy subjects. In the patients with first stage CRF, the total serum calcium did not significantly decrease (mean = 2.19 +/- 0.12 mmol/l) and ionized calcium was within reference value limits (mean = 1.28 +/- 0.02 mmol/l). The percentage of ionized calcium in total serum calcium was higher for these patients (mean = 58.43%) than for the control group (mean = 52.25%). For patients with first and second stage CRF, the total serum was significantly lower (mean = 1.92 +/- 0.03 mmol/l) than for the healthy controls (p < 0.001) as well as for patients with first stage CRF (p < 0.05). The ionized calcium in these patients was not significantly lower (mean = 1.14 +/- 0.01 mmol/l); however, its percentage was higher than that of serum calcium (mean = 59.38%). There was a slight negative correlation between the levels of ionized calcium and serum creatinine in patients with first stage CRF (r = -0.30) and a significant correlation in patients with second and third stage CRF.

[A clinical trial of the preparation palin in women with refractory colpitis].

The authors carried out clinical trial of the preparation Palin (pipemidinic acid) on 43 women with persistent colpitis after preliminary testing. There was negative microbiological finding in 67% of women. The results of the authors differed from those of other authors, since Palin was used mainly in women with chronic colpitis resistant to routine treatment. Minimal side effects were found. Usage of Palin is recommended in women with colpitis, caused by: E. coli, Enterococcus, Klebsiella and mixed aerobic flora.

[The pregnancy outcome and changes in the blood volume in relation to the time of the appearance of late toxicosis symptoms].

The author studied plasma volume and mass plasma proteins on pregnant women with late toxicosis, occurring before and after 37 weeks' gestation, as well as respective groups of healthy pregnant women--113 in all. In comparison with control groups of healthy pregnant women, the women with late toxicosis, occurred before 37 weeks' gestation, had a considerable reduction in plasma volume and in the mass plasma proteins and an increase in hematocrit, but women with symptoms of late toxicosis, occurred after 37 weeks' gestation, had an increase in plasma volume and mass plasma proteins and a reduction in the hematocrit. Delivery occurred on the average during 38.6 weeks' gestation and the newborns had weight lower with 10.8% than the respective children in the country at the same period in the group of women with toxicosis, occurred early. In women with symptoms of late toxicosis the delivery occurred during 40.4 weeks' gestation larger children than normal were born and the pregnant women gained weight considerably more. The results from the study give foundation to assume that there is no late toxicosis when the symptoms of late toxicosis occur after 37 weeks' gestation, because of which the time of occurrence of the disease should be taken into consideration in establishing the diagnosis of late toxicosis of pregnancy.

[Experimental Salmonella dublin infection in calves].

Calves, aged 20 days, were experimentally infected with Salmonella dublin. The animals of one-group were treated at the rate of 6.10(8), and those of another--at 2.10(10) up to 4.10(10) live bacterial cells. The calves of the first group showed no clear clinical symptoms of a disease; neither did they manifest regular shedding of bacterial cells. The calves of the second group presented a typical clinical picture, with more regular shedding of bacteria through nasal discharges and the feces. After killing the animals Salmonella dublin organisms were found in all parenchymal organs and in the lymph nodes. Morphologically, the prevalence was established of inflammatory necrobiotic processes in the liver, with activation and proliferation of the sinus endothelium of the lymph nodes, and catarrhal inflammation of varying manifestation in the small intestine.

[Histogenesis of leukemic changes in cows].

A histopathologic study was carried out on lymph nodes and viscera of 98 cows and 28 heifers that responded positively in serologic examinations for leukosis. The animals did not exhibit clinical and paraclinical symptoms, neither were there found any morphologic changes characteristic of leukosis. There existed activation and proliferation of the reticuloendothelial tissue in the sinuses of the lymph nodes. The same response was shown by the endothelium of the capillaries in visceral organs. There was also metaplasia that concerned mainly the connective tissue cells of the lymph node trabeculas as well as the adventitial cells of the vascular walls. It was considered that the infection could be manifested morphologically as a reticuloendotheliosis. By its clinical course and morphologic manifestation bovine leukosis resembles some other virus diseases that have been systematized as "delayed virus infections'.

[Histochemical changes in leukemia in cattle].

Histochemical investigations were carried out to demonstrate the activity of succinic dehydrogenase, diphosphopyridine-nucleotid-diaphorase, alkaline phosphatase, and acid phosphatase in the liver, kidneys, lymph nodes, and heart muscle of a total of 20 cows with positive serologic response for leukosis as well as in organs of 2 normal controls. It was found that the lymphoid cells of the leukotic proliferations and the activated endothelial and adventitial cells of the blood vessels had high alkaline phosphatase activity and negligibly expressed acid phosphatase activity. Dehydrogenase activity was low in the lymphoid-cell proliferations and in the activated cell of the vessels. The cell metabolism of the leukotic proliferations was like-wise disturbed. Histochemical methods of investigations could be used test-like in the diagnosis of bovine leukosis.
