[Bacterial DNA alteration by plasma generated atomic nitrogen. Real-time PCR detection contribution].

AIMS: Bacterial sterilization by the technology of plasma in post-discharge shows a growing interest. The main appeal of this new process resides in its action at dry and low temperature (60 degrees C). This technology would be therefore useful for the complex medical equipment, sensitive to the oxidization, humidity and/or requiring a temperature lower than 60 degrees C. The objective of this survey is to demonstrate the activity of an atomic flux emanating a plasma of pure molecular nitrogen on the bacterial DNA: does the plasma of nitrogen damage the genetic material? MATERIALS AND METHODS: The bacteria tested (Bacillus stearothermophilus, Staphylococcus aureus MRSA and Helicobacter pylori) are cultivated on suitable agar, and the bacterial DNA is extracted from every CFU by the technique of the columns (High Pure PCR Template, Roche). Every quantity of DNA extracted is diluted in 1 ml pure water. Then, 50 microl of each of these solutions of DNA are laid down in sterile Nunclon's plates holes, which undergo an advanced emptiness cycle during 60 minutes. The DNA residues will be then introduced during 40 minutes in a plasma sterilization surrounding wall (Plasmalyse), Satelec) where the debit of nitrogen, the pressure and the temperature are adjusted respectively to 1 L x min(-1), 5 Torrs and 60 degrees C. The DNA so ''plasmalysé'', is recovered then by aspiration in 500 mul pure water and processed to undergo an amplification/detection by Real-Time PCR (LightCycler2.0, Roche). The DNA ''plasmalysé'' will be compared to the intact DNA control(1), to the DNA control control(2) having undergone the cycle of emptiness solely, as well as to the DNA control(3) solely heated to 60 degrees C during 40 min. RESULTS: The amplification curves demonstrated that the only advanced emptiness and the only heat don't have any activity on the bacterial DNA. On the other hand, the DNA ''plasmalysé'' shows a deterioration of the amplified sequences. CONCLUSION: The genomic bacterial DNA, once extracted, is damaged by the gaseous flux of nitrogen plasma. A new sterilization process of the medical material will presumably impose itself in medium-term.

Absence of Helicobacter pylori in the oral cavity of 10 non-dyspeptic subjects demonstrated by real-time polymerase chain reaction.

Helicobacter pylori plays a significant role in gastric disease. However, the presence of this bacterium in the oral cavity remains controversial. The aim of the present study was to detect and quantify H. pylori in 29 different sites of the oral cavity in non-dyspeptic subjects by means of real-time polymerase chain reactions (PCR). Ten subjects without gastric symptoms were studied. Samples from unstimulated saliva, three sites of the tongue, oral mucosa, and 12 sites of both supragingival and subgingival plaque were collected from each subject. DNA was extracted from the oral samples and analysed for the presence of H. pylori by real-time PCR (LightCycler) using JW23/22 primers which targeted the 16S rRNA gene. DNA from H. pylori DSM 4867 was used as a positive control. Amplification efficiency for the LightCycler 2.0 runs ranged from 1.8 to 2.4. Melting curve analysis identified all the positive control capillaries, which contained H. pylori reference DNA, as a single and narrow peak at a melting temperature between 84.5 and 84.9 degrees C. All the negative control capillaries with no template control and the 29 oral samples from each subject showed either no melting peaks or broad melting peaks below 80 degrees C, which were considered as primer dimers. Therefore, H. pylori was not detected from any of the 290 oral samples. As a conclusion, H. pylori seems not to be permanently present in the oral cavity of a non-dyspeptic population.

Environmental lead exposure and its relationship to traffic density among Senegalese children: a cross-sectional study.

Leaded-gasoline is probably the primary source of lead (Pb) exposure in Dakar (Senegal). The present cross-sectional study was undertaken to investigate the levels of Pb in Senegalese children and to present helpful data on the relationship between Pb levels and changes in biological markers of heme biosynthesis and oxidative stress. A total of 330 children, living since birth either in rural or urban areas (ie, Khombole (n = 162) and Dakar (n = 168), respectively) were included. During this cross-sectional study, the mean blood (B)-Pb level in all children was 7.32 +/- 5.33 microg/dL, and was influenced by the area of residence and gender. In rural children, 27 subjects (16.7%), 18 boys (19.6%) and nine girls (12.9%), had a B-Pb level > 10 microg Pb/dL, whereas 99 urban children (58.9%), respectively, 66 boys (71.8%) and 33 girls (43.4%), had alarmingly high B-Pb levels. Accordingly, urine delta-aminolevulinic acid levels were higher in children living in the urban area than in the rural areas (P < 0.001), and closely correlated with the B-Pb levels (P < 0.01). Moreover, glutathione peroxidase (GPx) activity, selenium (Se) level, glutathione reductase (GR) activity, and glutathione status were significantly influenced by area of residence and/or by gender. GPx activity and Se level were not only negatively correlated with B-Pb levels, but also positively correlated together (P < 0.01). Taken together, the present results allow us to conclude that urban children have higher B-Pb levels than rural children, and that of these children, boys have higher B-Pb levels than girls, leading thereby to alterations of heme biosynthesis and pro-oxidant/antioxidant balance. We also suggest that exposure to Pb and the Pb-induced adverse effects merits attention and that the development of preventive actions are of increasing importance in Senegal.

[Sonographic features of gestational choriocarcinoma].

PURPOSE: To describe the sonographic morphological features of gestational choriocarcinoma. MATERIALS AND METHODS: Retrospective evaluation of 13 cases of gestational choriocarcinoma diagnosed by clinical and laboratory (BHCG) criteria in all cases and confirmed by histological data in 8 cases. RESULTS: The tumor presented multiple features (nodular, submucosal, macrocystic, multicystic, compact and microcystic) often with involvement of surrounding tissues. All cases showed myometrial involvement. The size of the ovaries was normal with small corpus luteum cysts noted in only 5 cases. In 2 cases, ultrasound modified the clinical staging. CONCLUSION: Sonographic features of gestational choriocarcinoma are variable, and may mimic other diseases of the endometrium or myometrium. The involvement of multiple layers of the uterus suggests a malignant disease.

[Perceptions and behavior toward dentistry in students and non-students in Dakar (Senegal)].

The authors try to appreciate the perception by student and non student population of dental care throughout fear of dentist, pain and dentist-patient relationship, also the patient preventive attitude. Results show more fear of the dentist by students and expressed pain. Dental cares are expensive according both students and non students but seem to be comfortable. Dentist-patient relationship is more appreciated at the Odontologic Institute which form future dentists and 80 per cent of patients consult for prophylactics care against 29 at the medico-social centre of the university. Dental care accessibility in developing country is a real problem and rate of dental diseases cannot be taken down without support of information and communication waves.

[Quality of butter sold in Dakar (Sénégal)].

Among foods that are introduced in Senegal, butter takes a significant place particularly at home where it is frequently eaten. Nevertheless, Senegalese authorities don't carry out any quality control under those different types (mark) of butter. In consequence, to find butter without quality or characteristics of authentic butter may be high. The aim of this work is to analyse quality of main varieties of butter in Dakar market. Chemical methods were used to determine the characteristic of butter samples and gas chromatography was made to authentify their fatty acid profile. Our results have shown that most of analysed sample respected the standards of quality. In fact the composition of fatty acid and characteristic factors of authentic butter were certified by this study.

Environmental lead exposure and its relationship to traffic density among Senegalese children: a pilot study.

In Senegal, as in many developing countries, traffic density is increasing in urban areas; in Dakar more than 50% of vehicles use gasoline. Yet the extent and real magnitude of the problem has neither been recognized nor assessed in these countries. Systemic data assessment of lead pollution and people's exposure are not well known in Senegal. This study was also designed to determine the impregnation levels of the lead released by the exhaust of cars and the changes of some early biological markers in Senegalese children. Blood lead (BPb) levels showed that all the children enrolled were exposed. However, lead exposure levels (from 34.7 to 145.8 microg/L) were less important for children living in rural areas (60.9+/-18.3 microg/L) than for those living in urban areas (106.7+/-16.9 microg/L). These changes could be correlated to the difference in the automobile traffic between both these regions (P < 0.001). BPb mean levels found in boys were higher than those in girls (P < 0.05). Despite elevated BPb levels, all values for blood zinc protoporphyrin and urine delta-aminolevulinic acid were within physiological ranges. In addition, variations in some biological markers of oxidative stress and renal disorders were seen; however, they must be confirmed by a future epidemiological study.

In vitro production of immunoglobulins of various classes and subclasses by cord blood B cells in African neonates: modeling and assessment of determination.

Cord blood B cells obtained from neonates of healthy Senegalese mothers were assayed in vitro for their capacity to fully differentiate and secrete immunoglobulins (Ig) of various classes and subclasses. Stimulation of mononuclear cells with SAC particles or anti-micro antibodies in the presence of IL-4, or with IL-2 and IL-10 induced a strong production of IgG, provided that an additional CD40/CD40L signal was present, in contrast to adult cell cultures. Cord blood mononuclear cells differentially stimulated with various cytokines in order to lead to Ig heavy chain switching and production of the various classes/subclasses consistently produced IgG1, IgG3, IgG4, IgE and IgA. This system has been applied to immune cells from African neonates that have not been extensively studied previously. Estimation of Ig production as OD ratios could be applied to cultures where cord blood B cells are stimulated with defined antigens of human pathogens to which the fetus immune system was primed in utero.

[Peripartum idiopathic myocardiopathy: prospective echocardiographic study].

Still frequent in Africa, the idiopathic peripartum cardiomyopathy (PPC) is the subject of only few prospective studies. The aim of this prospective work was to assess the echocardiographic abnormalities of the PPC and to determine on 26 patients the evolution and the prognostic factors of this disease. Six women had mild to moderate pericardial effusion. The abnormalities of the wall motion, constantly found, were diffuse in 20 cases (77.1%) and localized or prevalent on the interventricular septum or the left ventricular posterior wall in the other cases. The cardiac chambers were dilated in 24 cases (92.3%). Left ventricular hypertrophy was noted among 16 patients (61.5%); it was eccentric in 15 cases. The left ventricular systolic dysfunction was constant. The other abnormalities were: the abnormal left ventricular relaxation (one case), low mitral and aortic flow (12 cases), the mitral (21 cases) and tricuspid regurgitation (five cases). One noted a septal hypertrophy and an isolated dilatation of the right ventricle. The mean follow-up was 7.3 +/- 1 month (1-18 months). Two patients died at the 4th and 8th month. Among the 24 survivors, 11 had normalized were: the gestity (p = 0.01), the parity (p = 0.01), the cardiothoracic ratio (p = 0.04), the left ventricular volumes (p = 0.02), and the parameters of left ventricular function. The echocardiography of patients with PPC usually shows a pattern of dilated and hypokinetic cardiomyopathy, but many variations are possible. Most of the patients keep echocardiographic abnormalities after mean term follow-up.

Experimental IgG antibody production in vitro by peripheral blood and tonsil surface gamma+ B lymphocytes from Plasmodium falciparum-immune West Africans.

Antigen reactive B cells in tonsil specimens from teenagers from a region moderately exposed to P. falciparum were capable of being differentiated in vitro and producing specific immunoglobulin (Ig)G in up to 33% of individual experiments. Mononuclear cells or purified (s)gamma+ CD19+ B cells from peripheral blood or tonsil specimens from P falciparum-immune Senegalese subjects produced antigen-specific IgG upon appropriate stimulation in vitro. One fraction of this IgG was produced de novo by differentiated B cells and another fraction was likely bound on the surface of circulating or resident CD19+ sgamma+ B cells which were found in significantly greater numbers in individuals from rural Senegal as compared to nonimmune European controls. This study further documents the baseline levels of in vitro driven anti-P. falciparum IgG antibody production by mononuclear cells from blood and tonsils in immune populations exposed to P. falciparum differentially. Furthermore, this study demonstrates the relevance and potential utility of tonsils as a source of B lymphocytes to characterize further specific antibody responses to P. falciparum antigens in immune populations.

[Differential changes in IgG1 and IgG3 against a major antigen of blood stage Plasmodium falciparum (MSP1(19)) as a function of the parasite transmission period: study among immune Senegalese adults].

This study examined the evolution of P. falciparum specific IgG1 and IgG3 antibodies "before" and "after" the highest transmission period in clinically immune Senegalese adults settle in Dielmo, a holoendemic area for P. falciparum transmission. Plasma was tested for antibodies to an antigen (Q-KNG- MSP1(19)) known to react with IgG1 and/or IgG3 in the majority of these individuals. There was a decrease in titers in individuals with low, but not high titer IgG1 whereas specific IgG3 remained unchanged following the highest transmission period. These results raise the question of the differential role of anti-MSP1(19)g-IgG1 antibody fractions in the maintenance of immunity to P. falciparum.

Polyphasic characterization of rhizobia that nodulate Phaseolus vulgaris in West Africa (Senegal and Gambia).

Fifty-eight new isolates were obtained from root nodules of common bean (Phaseolus vulgaris) cultivated in soils originating from different agroecological areas in Senegal and Gambia (West Africa). A polyphasic approach including both phenotypic and genotypic techniques was used to study the diversity of the 58 Rhizobium isolates and to determine their taxonomic relationships with reference strains. All the techniques performed, analysis of multilocus enzyme electrophoretic patterns, SDS-PAGE profiles of total cell proteins, PCR-RFLP analysis of the genes encoding 16S rRNA and of the 16S-23S RNA intergenic spacer region (ITS-PCR-RFLP), auxanographic tests using API galleries and nodulation tests lead to the consensus conclusion that the new rhizobial isolates formed two main distinct groups, I and II, belonging to Rhizobium tropici type B and Rhizobium etli, respectively. By MLEE R. etli and group II strains showed several related electrophoretic types, evidencing some extent of internal heterogeneity among them. This heterogeneity was confirmed by other techniques (ITS-PCR-RFLP, SDS-PAGE and host-plant-specificity) with the same nine distinct strains of group II showing some differences from the core of group II (54 strains).

[Contamination by aflatoxins of local peanut oils prepared in Senegal].

Aflatoxins are mycotoxins produced by certain strains of fungus (Aspergillus) which develop in peanut seeds. Peanut oil and peanut meal are widely used in Senegal. The risk of intoxication is quite real. We determined aflatoxin contamination levels in peanut oil and food prepared by small scale production plants in the Kaolack and Diourbel regions of Senegal. High performance liquid chromatography (HPLC) of the different samples showed that 80% of them were contaminated. Aflatoxin B(1), B(2), G(1) and G(2) were detected. The pattern of contamination was similar for the two regions. Aflatoxin B(1) was prevalent and found in over 85% of the samples. The mean content of this mycotoxin (the most dangerous toxin) was about 40 ppb, a value which largely exceeds specifications for allowable levels.

[Analgesic effect of Pilostigma reticulatum (nguiguis)].

Mankind has always given himself means to fight pain by using at first, means offered to him by his environment particularly the plants. African pharmacopoeia is rich of thousand of plants. It changes in term of its ecosystem and its vegetation. Decocted leaves of pilostigma reticulatum (nguiguis in ouolof) are used in western Africa, because of its analgesic properties in case of "borom bop" literally meaning headache associated to odontalgias and mumps. In our study we used lyophilisate obtained from dry leaves of the plant which has been used for experimentation while the decocted dry leaves have been used for clinical application. So, it appeared that leaves of pilostigma reticulatum are almost atoxic when administrated by oral tract (DL50 = 17 g/kg) according to GLEASON classification which recognizes as atoxic every substance having a DL50 higher to 15 g of lyophilisate by kilogram of corporal weight. Elsewhere the study of the peripheric analgesic activity (according to the acetic acid test) has shown a very significant peripheric analgesia since the dose of 750 mg/kg which climbs with it. The clinical survey carried out at the dental community center of Pikine Icotaf based on the usual method of the utilization of the decocted (as mouth rinse) has shown that, this plant procures pain sedation in 97% of the patients having undergone dental avulsion and in 78% of the cases of desmondontal syndromes. This inaugural report must be continued in order to certify the pharmacologic or toxic effects of that plant and define clinical doses from experimental doses we have cleared.

[Antiphospholipid antibodies and recurrent spontaneous abortions at the Aristide Le Dantec University Hospital Center].

Antiphospholipids including lupus anticoagulants (LA) and anticardiolipins (ACL) are known to be able to lead to many obstetricals complications like gravidic chorea, eclampsia and repeated spontaneous abortion. Our objective was to determine place of antiphospholipids among women presenting repeated spontaneous abortion in Senegal. We tested prospectively 96 women in a breeding age with a past history of at least 2 spontaneous abortion. 14.6% and 21.1% of tese women present present respectively LA and ACL. The prevalence of antiphospholipids in among women presenting repeated abortion is higher in Senegal and similar to those previously reported confirming its place in this pathology important. A strategy of diagnosis should be defined between obstetricians and biologists in order to improve the obstetrical prognosis.

[Toxocological study of Guiera senegalensis Lam (Combretaceae)].

The study was based on an aqueous extract derived from a 60 degrees ethanolic tincture containing 0.032 g of dry matter per ml. The leaves of Guiera senegalensis Lam (Combretaceae) were collected in December 1991 at Nguekhokh a village within 20 km from Mbour (Senegal). The extract was administered for six months through daily forced-feeling at 2 g/kg to Wistar male and female rats whose weight at the beginning of the experiment tanged between 140 g and 180 g. The urine was analysed during the study and the animals were weighed every four weeks. At the end of the experiment, the animals were slaughtered and various analyses carried out. Haematological features in relation with erythropoiesis, haemoglobinogenesis and leucopoiesis, were studied in relation with renal and hepatic functions; biochemical features too. Some organs (heart, lungs, kidneys, brain, cerebellum, spleen and liver) were removed and examined in order to detect possible lesions following the experiment. Judging by the results, Guiera senegalensis Lam (Combretaceae), as used in the experiment, did not show any significant toxicity.

[Mango leaves (Manguifera indica, Anacardiacea) used as biomarker of pollution by pp'Dichlorodiphenyltrichloroethane (pp'DDT)].

Dichlorodiphenyltrichloroethane (DDT), organochlorine pesticide, byanother way cumulative pesticide is banned in mostdeveloped countries. Whatever, it is still used in many countries in the Third World. This work consist to check whether this chemical compound is still used in Senegal. The biomarker of pollution by this pesticide for this research was mango leaves (mango tree is widespread in Senegal) open to accumulate halogenated hydrocarbons. Leaves sample were taken in many sites in the centerline of Dakar-Thiès (Senegal), then analysed by gas chromatography. The results allowed to note the presence of DDT and its metabolites [(dichlorodiphenyltrichloroethylen (DDE), (dichlorodiphenyltrichloroethane (DDD)] in most of samples. The comparison of DDT content in relation to those of the principal metabolite allowed to make the difference according to sample sites, an old contamination of an utilisation more or less recent of this pesticide. From those results, we can conclude that DDT, typical pesticide by its big persistence is still used in some spherical zones in this country, in particular in farming gardening zones.

[Aflatoxin contamination level in artisanal and industrial peanut butter food in Dakar (Senegal)].

Aflatoxins are mycotoxins produced by some strains of fungus (Aspergillus) which develop in peanut seeds. Peanut oil and past are very used up in Senegal, then the aflatoxin poisoning risk is very actual. The aim of this study was to determinate the aflatoxin level in artisanal and industrial peanut pastry food from Dakar (Sénégal). High Performance Liquid Chromatography (HPLC) analysis of the different samples showed that the most contaminated by aflatoxins are artisanal pastry sold in different market of Dakar (Sénégal). Indeed, 40% of these samples contained mean values of aflatoxin B1 (the most dangerous) widely over allowable EEC specifications (5ppb). Furthermore, most of industrial and domestic peanut pastry were cleaned and could be consumed without risk.
