[Foreign bodies and injuries of the rectum].

Personal experience of 33 cases of foreign rectal bodies introduced transanally with or without associated lesions is reported. After examining the principles of diagnosis, stress is laid on therapy and personal experience and it is suggested that more energy should be applied to extracting the foreign body transanally even using more or less deep anaesthesia, so avoiding the need for abdominal surgery.

Adolescent

[Our experience in laparoscopic diagnosis in general and emergency surgery (author's transl)].

After a brief review of the history of laparoscopy starting from the first endoscope, the authors remark on the great usefulness, ease, and essential harmlessness of the procedure. They list all the possible uses of laparoscopy in general and emergency surgery, exemplifying their claims with the illustration of personal cases and recommending widespread use of the method in surgery.

Acute Disease

[Aimed liver biopsy in the course of laparoscopy (author's transl)].

Following a discussion of the various methods that have been devised for liver biopsy, and an explanation of the advantages accruing from aimed liver biopsy in the course of laparoscopy, the authors express their preference for this method on account of its unquestionable diagnostic value, ease of execution, and freedom from complications. Accordingly, and in good agreements with other experts such as Kalk, Beck, and Dagnini, they advocate more widespread use of the procedure in liver pathology.

Biopsy, Needle

[Acute granulomatous appendicitis. (Contribution to preventive appendectomy in acute ileitis)].

Starting with one case of acute granulomatous appendicitis of their own observation, the authors review the scant literature on the subject (only 23 published cases to date) and advocate systematic appendicectomy in all isolated appendicular localizations of Crohn's disease. On the grounds that no post-appendicectomy fistulization has ever been reported in such patients, the authors recommend prophylactic appendicectomy also in the far more frequent cases of acute ileitis, to forestall the possibility of evolution into chronic Crohn's disease and the risk of missing a nongranulomatous acute appendicitis appearing with the same clinical symptoms.

Adult

[Pulmonary blastoma. (Personal case)].

The authors give a detailed description of one case of pulmonary blastoma recently come to their observation. Then, after a review of pertinent literature, they discuss the more interesting aspects of this pulmonary pathology, namely its rarity, uncertain histogenesis, difficult diagnosis, and above all baffling prognosis. They conclude with an appeal for further contributions on this interesting subject, in the hope of understanding it better through the study of an adequate number of cases.

Carcinoma, Squamous Cell

[A case of giant hamartochondroma of the lung].

The authors present a case of pulmonary hamartochondroma of exceptional size, of which they give a clinical descrition and explain the surgical treatment. Next, aloso in the light of recent literature on the subject, they review the possible causes of this disorder in terms of several histopathogenetic hypotheses, and they discuss problems of differential diagnosis as well as the histopathological aspects of this rare, benign tumor of the lung. They conclude that even though malignant transformation of hamartochondroma is exceptional, these tumors should always be removed surgically as soon as they are diagnosed or reasonably suspected.

Hamartoma

[Our experience in esophago-colonic plastic surgery for esophageal stenosis caused by caustics].

The Authors report their experience (16 cases) in esophago-colon plastic surgery for esophageal stenosis from caustics and emphasize the advantages presented by the colon as a substitute for the esophagus, as compared with the stomach and jejunum. Although the results were equally satisfactory irrespective of the selected segment of colon, the Authors indicate their present preference for the left colon because of some technical aspects making the operation easier and more reliable.

Adolescent

[Anatomo-surgical aspects of peroperative cholangiography in biliary lithiasis].

Biliary anomalies and volumetric changes in the ductal system often make it difficult to identify individual structures during the course of operations for biliary lithiasis. The Authors reviewed 406 peroperative cholangiographies during the course of 1250 operations, evidencing some statistical data regarding the anomalies of greatest interest for surgical practice and stressing the significance and limits of dilation of the V.B.P. On the basis of their personal experience and the relevant literature the Authors conclude that peroperative cholangiography constitutes a useful method not only for diagnostic purposes, but also for correct definition of certain aspects of the extrahepatic biliary ducts.

Cholangiography

[Pseudocysts of the pancreas. (Personal cases)].

After reviewing all the various procedures for surgical treatment of pseudocysts of the pancreas, the Authors, analysing the advantages and disadvantages of each method, present a set of nineteen personal cases treated by simple cystogastrostomy, simple cystojejunostomy and cystojejunostomy combined with entero-entero-anastomosis according to Braun, or cystojejunostomy on defunctionalised loop according to Roux. After stading the overall criteria for selection of the technique to be applied individually, the Authors conclude by stressing the advantages that cystojejunostomy on defunctionalised loop according to Roux offers as compared with all the other procedures.
