[HIV infection and immunity disorders in patients with hemophilia].

The clinical, serological and immunological status of 23 haemophilia patients (21 with haemophilia 9 and 2 with haemophilia B) was investigated. In 12 of 23 haemophiliacs antibodies to HIV were detected. All seropositive patients were treated by imported clotting factor concentrates, and cryoprecipitates obtained by local donors. Six HIV positive haemophiliacs were in asymptomatic stage of HIV infection, and six others were in persistent generalized lymphadenopathy stage. HIV positive haemophiliacs showed a decreased number of CD4 and CD2 lymphocytes, inversed CD4/CD8 lymphocytes ratio, anergy to skin tests, decreased NK (natural killer), cell cytoxic activity, increased IgG and increased immune complexes. HIV negative haemophiliacs showed no different immunologic abnormality than healthy controls. Our results also showed differences in the degree and type of immunological abnormalities depending on different stages of HIV infection. Haemophiliacs in asymptomatic stage of HIV infection showed qualitative abnormalities of T cell immunity, manifested with decreased NK cells cytotoxic activity and anergy to skin tests, while haemophiliacs in persistent generalized lymphadenopathy stage, besides qualitative, showed quantitative abnormalities of T cell immunity, manifested with decreased lymphocyte subpopulations (CD2, CD3, CD4). Immunological disorders observed in our haemophilia patients were in direct correlation with the presence of anti HIV antibodies in peripheral blood, and we suppose that they may be attributed to HIV infection rather than to chronic antigenic stimulation with foreign proteins in blood products.

[The importance of using various methods in the determination of serum immune complex levels].

Concentration of immune complexes (IC) in the serum of patients with various diseases (glomerulonephritis systemic lupus erythematosus--SLE, rheumatoid arthritis--RA and with positive antibodies to human immunodeficiency virus--HIV+) has been determined using two methods: PEG precipitation and laser nephelometry (NL) of binding to Latex = C1 12q complement components in aqueous media. The presence of antiimmunoglobulin antibodies, that is, rheumatoid factor (RF) and its eventual effect on the precision of measurement of IC using LN C1q method has been determined by LN-Latex RF method. The results have shown that the increased IC values obtained by one method can be within physiological levels if the other method was applied and vice versa. Depending on the group of examinees such negative correlation ranged from 33% to 64%. This is the result of already known limitations of the studied methods and confirms the necessity of simultaneous application of different methods for determination of IC concentration. Regardless of anticomplementary activity of serum rheumatoid factor its presence in the serum has not shown to have any effect on the precision of determination of IC concentration using IC by LN- C1q method.

[Repair of a severed nerve with denatured muscle graft--preliminary results of a new surgical technic].

In Neurosurgical Clinic of Military Medical Academy prospective study about the use of denatured muscle graft in nerve repair is in progress, based on numerous experimental and sporadic clinical literature data. The first, experimental part of the investigation is directed to operative technique details and also to find out the manner and duration of muscle denaturation which provide ideal conditions for regenerating axons sprouting. It is concluded that greater pectoral muscle have to be muscle graft donor for nerve defects shorter than 6 cm, and sartorius muscle for those longer than 6 cm. Denaturation for 30 seconds in liquid nitrogen and after that in distilled water for 3 minutes results in micromorphological status in muscle graft which is optimal for axonal regeneration. In the second, clinical part of the work reparations of missile neurotmesis of radial nerve with denatured muscle grafts are performed. Preliminary results 9 months after operation are quite comparable with results after sural nerve graft reparation.
