Laugier's disease.

Pigmented macules of the malpighian mucous membranes have been described under numerous names since the first description by P. Laugier in 1970 of the 'pigmentation mélanique lenticulaire essentielle de la muqueuse jugale et des lèvres'. In this paper, we show that (1) the lesions may occur in all malpighian mucous membranes, (2) are easily distinguished from melanoma both clinically and if necessary by histology, and we suggest (3) that, whichever the localization is, the condition should be called Laugier's disease.

Necrobiotic xanthogranuloma: a case without paraproteinemia but with transepithelial elimination.

We report a female patient, now aged 60, followed for 20 years for lesions originally diagnosed as necrobiosis lipoidica diabeticorum. In fact, the anatomical and clinical features of her disorder correspond to the new entity described as necrobiotic xanthogranuloma. Two elements distinguish this from earlier cases: 1) two examinations failed to reveal paraproteinemia; 2) there was transepithelial elimination of cholesterol crystals and degenerated xanthomatous cell debris via hair follicles. This demonstrates the characteristic histology of the disorder and indicates that the diagnosis of necrobiotic xanthogranuloma should be considered even in the absence of paraproteinemia.

[Basal lamina with a garland-like pattern in a case of sclero-atrophic lichen. Ultrastructural study].

The basal lamina stands at the heart of dermal-epidermal interactions. Its formation and pathology are more or less directly related to the basal keratinocytes, so that any lesion of these cells, such as vacuolar alterations of the interface, results in pathological changes in the basal lamina. Images of rupture, discontinuities, multiplications, festooning and budding of the basal lamina have been reported in psoriasis, lichen planus, lupus erythematosus and lichen sclerosus and atrophicus. We present a case of lichen sclerosus and atrophicus of the glans penis. Histopathological examination was performed, using the routine technique, the semi-thin large area sections technique and electron microscopy. Histological changes in the basal lamina were particularly pronounced, with garlands penetrating deeply into the dermis. Electron microscopy showed that the basal lamina contained immature collagen fibres, but no anchoring fibres. This garland-like pattern undoubtedly represents an extreme degree of the festooning and budding classically described in the diseases listed above. A pathogenic theory is offered to explain the formation of that pattern.

[Collagenous papules of the external ear. Micropapular hyalinosis of the external ear with transepithelial elimination. Relation to primary localized cutaneous amyloidosis].

Sanchez has recently described a new entity characterized by the presence of papules bilaterally located on the aural pinnae. The four presented cases concerned women. Clinical picture of the disease is, without any doubt, unusual but "hyalin" masses are non-specific since they are similar to amyloidosis on account of thioflavine T positivity and microfilamentous aspect in electron microscopy. We reported at the Meeting of the French Dermatological Society (Toulouse, November 10, 1981) a very similar case under the name of "Hyalinose micro-papuleuse de la conque des oreilles avec élimination trans-épidermique". In the present paper we report this personal case, and we point out some original findings: pigmentary changes, trans-epidermal elimination of the hyalinized findings material and presence of an inflammatory process.

[Constitutional painful callosities. Analgesic efficacy of etretinate].

We have recently evaluated eight patients with an unusual genetic dermatosis that was characterized by the development of painful callosities at pressure points of soles, with tender yellowish-brown hyperkeratosis. These lesions are very painful: walking produced pain that was severe. The patients performing manual labor, in addition, had multiple callosities on the palms. Hyperhidrosis of the soles and palms was frequently present. Skin lesions were first noted in adolescence. There was no similar family history. There was no personal nor family history of hair, nail or dental disease. A biopsy was performed in one case and showed a simple hyperkeratosis similar to these observed in callosities. Classification of our cases is difficult. They are not similar to the entity named "hereditary painful callosities" by Roth et al. Review of the literature did not show similar cases. The patients received Etretinate (1 mg/day/kg). In all 8 cases, a dramatic improvement was obtained within seven days; the pain disappeared entirely and the patients were able to walk again without any pain. Then, the per-day dose was diminished without reappearance of the pain.

[Mammary lymphatic scintiscans by intratumoral injection in the assessment of breast cancer. 105 examination in 100 patients (author's transl)].

One hundred and five scintiscans of the mammary lymphatic system were performed in 100 patients with breast cancer by intratumoral injection of 99 m Tc-labelled colloidal rhenium. The progression of the radioactive colloidal agent was followed on a series of films taken 1, 2 and 4 hours after the injection, and the images obtained were compared with post-operative findings of lymph node involvement. Patients with more than 3 carcinomatous lymph nodes had less than 2 foci of activity, while those with less than 3 carcinomatous lymph nodes had more than 2 foci of activity, owing to more rapid progression of the compound. The difference was highly significant (p less than 0,001).

[Atopic dermatitis and pox-virus superinfections (author's transl)].

Subjects afflicted with atopic dermatitis are particularly predisposed to such viral superinfections as verruca, herpes (eczema herpeticum) and vaccinia (eczema vaccinatum). Knowledge of superinfection by the molluscum contagiosum virus is more recent and the authors describe one case. The orf virus (responsible for ovine ecthyma contagiosum), however, has never been described as responsible for the profuse superinfection of an atopic eczema. The authors observed one case of this in a child, atypical as result of his age (16 months), the cervical localization and the aspect, which was a telangiectasic granuloma type of the initial element, and finally of the satellization observed one week later. This child suffered from a generalized atopic eczema. Propagation of the orf lesions could have been favored by scratching, local corticoid treatment and by the deficit in cellular immunity observed in patient with atopic dermatitis. Indeed, viral superinfections, which are unusual by their profusion or their chronic nature, are often observed in immunodepressed subjects.

[Macroglobulinemia (Waldenström) with specific cutaneous nodules (author's transl)].

The authors present a case of Waldenström's disease in a 68-year-old woman. The only symptoms were several nodes on the leg. The pathological, and immunological finding showed the presence of lymphoplasmocytic cells in the middle dermis; there was a positive reaction with the anti-IgM serum. Ultrastructural studies showed large cells with dilated reticulum and abnormal IgM globulin was found in the serum in large amounts. The rate of the others immunoglobulins was normal. There was a lymphoplasmocytic proliferation in the bone medullar. The specific cutaneous manifestation of the Waldenström's disease are 1) large "plaques" or/and 2) nodes. The pathological studie shows numerous lymphoplasmocytic or immunoblastic cells and sometimes the presence of an amorphous material which is the abnormal globulin.
