[Sterilization of the mentally handicapped as a sacred process].

On the basis of the concept of sacredness, this article sheds light on the motivations of parents who request sterilization for their mentally handicapped child. Sterilization is characterized as a ritualized practice whose meaning and function must be clearly defined. With this surgical procedure, the special status of the mentally handicapped is established. They become "sacred" beings, who, by definition, are forbidden to reproduce. This concept of sacredness through sterilization emphasizes the representation of handicapped individuals in angelic forms.

Long-term survival and functional status in the elderly after cardiac surgery.

BACKGROUND: Hospital mortality and morbidity of cardiac surgery have been well described in the elderly, but there is a paucity of data regarding long-term functional status. OBJECTIVE: To evaluate long-term survival and functional status of patients aged 70 years or older after cardiac surgery. DESIGN: Retrospective observational study. SETTING: University-affiliated tertiary care hospital. PATIENTS: Three hundred and twenty-nine consecutive patients aged 70 or older who had undergone cardiac surgery from January 1990 to December 1993. INTERVENTIONS: Besides postoperative mortality and morbidity, data regarding long-term survival and functional status and preoperative variables affecting these outcomes were analyzed. MAIN RESULTS: Mean age at surgery was 73.9 +/- 2.9 years, and 70.2% of the procedures were performed on an urgent basis. Postoperative mortality was 9.4%. Five-year Kaplan-Meier survival in postoperative survivors was 85.9% and is comparable with the survival of the general sex- and age-matched Quebec population. Preoperative variables affecting long-term survival were atrial fibrillation (RR 3.8; 95% CI 1.4 to 10.0), smoking status on admission (RR 3.6; CI 1.6 to 8.1), peripheral vascular disease (RR 2.9; CI 1.4 to 6.2) and low creatinine clearance (RR 1.4; CI 1.1 to 1.7). Functional status evaluated by the Karnofsky score showed a successful functional outcome in 84.6% of patients at follow-up (mean 2.6 +/- 1.1 years). Preoperative variables unfavourably influencing a successful functional outcome were hypertension (OR 0.34; CI 0.15 to 0.76), cerebrovascular disease (OR 0.35; CI 0.15 to 0.87) and low creatinine clearance (OR 0.73; CI 0.55 to 0.96). CONCLUSIONS: Despite severe cardiovascular and often urgent conditions, sustained functional improvement and good long term survival can be objectively documented in most elderly patients after cardiac surgery.

[The institutional policy concerning sexuality: the necessary transformation of the sexology paradigm].

An increasing number of hospitals and rehabilitation centres include sexuality in their mandate. Some of these establishments have developed a policy in this matter or would like to. Based on a study of recorded policies, the author discusses various aspects of these policies, including legitimacy, necessity, content and implementation. Most policies under study are built around the notion that sexuality is a problem. The policy is therefore designed as an instrument for normalization and social control. The author proposes another paradigm: that which envisions sexuality positively by emphasizing the need to improve one's quality of sex life. As a result, policies would be more attuned to the meaning of sexuality rather than to sexual norms and rules. The author argues that such a concept widens the scope of institutional policy, from sexuality-as-a-problem to sexuality-as-a-health-issue.

Internalized homophobia and psychosexual adjustment among gay men.

The present study examined links between internalized homophobia and psychosexual tendencies among 261 homosexual men. Analysis indicated that men who accept their homosexuality less rate themselves higher on sexual anxiety, sexual depression, fear of sexuality, and concern about sexual image and lower on internal sexual control, sexual esteem, and sexual satisfaction. Although no significant differences were found between HIV-seropositive and HIV-seronegative subjects, the associations between internalized homophobia and certain psychosexual tendencies seemed more pronounced in the seropositive respondents. We believe it would be beneficial for homosexual men who are very homophobic to seek psychoeducative or therapeutic consultation to develop more positive attitudes toward their homosexuality and thereby better the quality of their lives.

[Predictors of sexual behavior in a group of heterosexual university students at risk for AIDS in Montreal].

In order to evaluate the predictors of risky sexual behavior and AIDS, 573 male and female college students enrolled in French speaking universities in Montreal answered a questionnaire dealing with their sexual behaviors, from which an index of risks was computed (CASH). The analyses show that the best predictors of the CASH for men are associated to their sexual partner taking the pill, peer attitudes toward sexuality and individual sensation-seeking index. For women the best predictors of risky sexual behaviors are the perception of chances to catch AIDS, use of drugs or alcohol during coital behavior, peer group attitudes, personal sexual attitudes and using the pill as a contraceptive. These results suggest that risky sexual behaviors depend on complex processes associated to lifestyle and personality structures.

[The sexual needs of homosexual HIV seropositive men].

A two-step study was conducted to identify the sexual needs of a group of seropositive homosexual men. At first, there was an interview with 30 men which aimed to define their objectives of sexual health. This was followed by a questionnaire distributed among 88 seropositive men and 173 seronegative men, who evaluated the objectives stated in the first step. The statistical analysis of collected data allowed for the identification and narrowing down of four major needs: being in good health, being in love, being sexual and, to a lesser degree, being secure. Results are discussed and point to the role played by the biological, psychological and sociological uneasiness in the context of the difficulties that seropositive homosexual men have in satisfying their sexual needs. This evaluation can prove useful in the planning and the implementation of sexological services for HIV-positive persons.

Homophobia and attitudes about AIDS.

A random sample of 407 French Canadian adults responded to a questionnaire about perception of AIDS. Negative attitudes about AIDS are better predicted by homophobia than other measures as are attitudes about extramarital relations and attitudes abut adolescent heterosexuality.

Path analysis of parents' conservatism toward sex education of their mentally retarded children.

The authors investigated three probable reasons why parents of mentally retarded children would be opposed to sex education being offered as a school subject: general sexual attitudes, specific sexual attitudes, and prejudices against the retarded child's own sexuality. A questionnnaire was given to 60 parents of retarded children and 62 parents of nonretarded children. A correlation analysis revealed evidence of all three kinds of reasons and led to a causal model of conservatism on sex education. A path analysis revealed that (a) sexual prejudices are a major obstacle to sex education, and (b) each group of parents represents a specific dynamic of conservatism on sex education. Means of pedagogical intervention to modify parents' attitudes toward sex education of handicapped children were discussed.
