Human herpesvirus 8-associated hemophagocytic lymphohistiocytosis in human immunodeficiency virus-infected patients.

We retrospectively reviewed 5 cases of hemophagocytic lymphohistiocytosis (HL) associated with human herpesvirus 8 (HHV-8) reactivation in human immunodeficiency virus (HIV)-infected patients. All patients had clinical and biological features characteristic of HL. Pulmonary symptoms were present in all patients and were frequently life threatening. The mean number of HL episodes was 6. Four patients had HL-associated Kaposi sarcoma, and 3 had multicentric Castleman disease. The mean CD4 cell count was 200 cells/mm(3). HIV loads were stable in all patients. All patients had high levels of HHV-8 in peripheral blood mononuclear cells during attacks, and a significant increase in this parameter before the attacks was seen in 3 patients. Although 2 patients died of HL, 3 are still alive and receiving etoposide therapy (mean follow-up, 3 years). HHV-8-related HL is associated with life-threatening symptoms and biological HHV-8 reactivation, and it may be controlled in the long term by etoposide therapy combined with highly active antiretroviral therapy.

Fate of Escherichia coli O157:H7 during the manufacture of Mozzarella cheese.

AIMS: The fate of Escherichia coli O157:H7 was investigated during the manufacture of Mozzarella cheese. METHODS AND RESULTS: The Mozzarella cheese was made from unpasteurized milk which was inoculated to contain ca 10(5) cfu ml(-1)E. coli O157:H7. Two different heating temperatures (70 and 80 degrees C), commonly used during curd stretching, were investigated to determine their effects on the viability of E. coli O157:H7 in Mozzarella cheese. Stretching at 80 degrees C for 5 min resulted in the loss of culturability of E. coli O157:H7 strains, whereas stretching at 70 degrees C reduced the number of culturable E. coli O157:H7 by a factor of 10. CONCLUSIONS: The results show that stretching curd at 80 degrees C for 5 min is effective in controlling E. coli O157:H7 during the production of Mozzarella cheese. Brining and storage at 4 degrees C for 12 h was less effective than the stretching. SIGNIFICANCE AND IMPACT OF THE STUDY: Mozzarella cheese should be free of E. coli O157:H7 only if temperatures higher than or equal to 80 degrees C are used during milk processing.

[Abstract quality assessment of articles from the Annales de Dermatologie].

INTRODUCTION: Article's abstracts are an important part of the publication, widely available in electronic databases. We assessed the quality of abstracts in the Annales de Dermatologie. The main objective was to compare abstract quality in 3 periods in the past decade. In this Journal, structured abstracts are required since 1993. The secondary objective was to compare structured and non structured abstract quality. MATERIAL AND METHODS: Three periods were assessed: 1991-92, 1996 and 2000. We selected the abstracts of original studies, excluding non focused descriptive case series. We used a criteria scale elaborated by Narine et al. Thirty criteria were assessed for each abstract, divided into 8 categories: purpose, research design, setting, subjects, intervention, outcome measurement, results and conclusion. If applicable to the study, a criterion was rated 1 (present) or 0 (absent). The final score for each abstract was defined as the ratio of positive answers among rated criteria. Mean scores for each period were compared using analysis of variance (mean +/- standard deviation). The temporal trend was calculated by simple linear regression. The mean scores of structured and unstructured abstracts were also evaluated and then compared. RESULTS: For each period, chronologically, 8, 17 and 18 (total 43) abstracts were evaluated. Mean scores for each period were 0.72 +/- 0.20 (1991-92); 0.69 +/- 0.12 (1996) and 0.83 +/- 0.08 (2000). These 3 scores were significantly different (P=0.006), with a trend toward increasing (Regression coefficient R(2)=0.136; P=0.015). Research setting obtained the lowest score (0.4). Structured (0.72 +/- 0.20; n=35) and non structured abstracts (0.76 +/- 0.12; n=8) were not significantly different. DISCUSSION: Abstracts of articles published in the Annales de Dermatologie contained on average 3/4 of the required informative items. The trend toward better scores may be explained by an increased attention of the editorial board as well as authors and reviewers. The quality could be improved by systematically mentioning the research setting.

[Descriptive epidemiology and knowledge of erysipelas risk factors].

Few epidemiological data related to erysipelas or cellulitis is available in the literature. Descriptive data, such as incidence, has mainly been assessed in hospital settings, and exceptionally in the general population. In the only case-control study available, main risk factors for erysipelas of the leg were lymphoedema and the site of entry. Leg edema, venous insufficiency, and overweight were associated to erysipelas to a lesser extent. Given its high attributable risk, secondary prevention strategies targeted at toe-web intertrigo should be evaluated. Risk factors for severity are difficult to assess if they are transient - such as NSAIDs intake. A study on prognostic factors is needed before intervention strategies are tested in appropriate groups of patients.

[Herpes zoster: incidence study among "sentinel" general practitioners].

INTRODUCTION: Herpes zoster is a frequent disease but its incidence in France is unknown. METHODS: We conducted a postal survey among the general practitioners of the "Sentinel" network. The incidence of acute herpes zoster was extrapolated from the number of cases diagnosed during the year 1998, by the general practitioners who answered the questionnaire. The general practitioners were also surveyed on their prescriptions and attitude. RESULTS: Among the 1,368 "Sentinel" general practitioners, 744 (54.4 p. 100) participated in the survey. The incidence in 1998 was 3.2 cases for 1,000 inhabitants (95 p. 100 confidence interval: 3.0 - 3.4). For acute herpes zoster, 73 p. 100 of the patients have been given an oral antiviral drug, and 63 p. 100 an antalgic. Among the 605 reported herpes zoster cases, 111 (18.4 p. 100) subsequently had chronic pain. DISCUSSION: The estimated incidence is comparable to the incidence from others developed countries. To be interpreted, this estimation has to be discussed according to the sample of population that was studied and the representativity of the "Sentinelles" general practitioners who participated the survey.

Risk factors for erysipelas of the leg (cellulitis): case-control study.

OBJECTIVE: To assess risk factors for erysipelas of the leg (cellulitis). DESIGN: Case-control study. SETTING: 7 hospital centres in France. SUBJECTS: 167 patients admitted to hospital for erysipelas of the leg and 294 controls. RESULTS: In multivariate analysis, a disruption of the cutaneous barrier (leg ulcer, wound, fissurated toe-web intertrigo, pressure ulcer, or leg dermatosis) (odds ratio 23.8, 95% confidence interval 10.7 to 52.5), lymphoedema (71.2, 5.6 to 908), venous insufficiency (2.9, 1.0 to 8.7), leg oedema (2.5, 1.2 to 5.1) and being overweight (2.0, 1.1 to 3.7) were independently associated with erysipelas of the leg. No association was observed with diabetes, alcohol, or smoking. Population attributable risk for toe-web intertrigo was 61%. CONCLUSION: This first case-control study highlights the major role of local risk factors (mainly lymphoedema and site of entry) in erysipelas of the leg. From a public health perspective, detecting and treating toe-web intertrigo should be evaluated in the secondary prevention of erysipelas of the leg.

Oral Crohn disease: clinical characteristics and long-term follow-up of 9 cases.

BACKGROUND: Oral localization of Crohn disease is uncommon and must be differentiated from nonspecific lesions. Its natural course and its long-term prognosis are unknown. OBSERVATIONS: We studied 9 patients (8 male, 1 female; age range, 7-52 years; median age, 16 years) with Crohn disease and specific oral lesions, including deep linear ulcers, pseudopolyps, and/or labial or buccal swelling and induration. The prevalence of such lesions was 0.5%. The median follow-up was 11 years. Oral localization developed before (n = 2), at the same time as (n = 2), or after (n = 5) the onset of the digestive disease. Noticeable associated localizations were observed in the anoperineum (n = 8) and the esophagus (n = 3). The median duration of the oral lesions was 4 years (range, 1-13 years), without necessary parallelism with the digestive localization. Five patients had complete healing after a median delay of 2 years. CONCLUSIONS: Oral localization of Crohn disease is characterized by a marked male predominance, a young age at onset of Crohn disease, and a very protracted course. The high prevalence of associated anal and esophageal involvement suggests that Crohn lesions have a particular trophicity for squamous cell epithelium.

[Symptomatology and markers of the severity of erythematous drug eruptions].

OBJECTIVES: Skin reactions to drugs affect about 2.2 percent of inpatients. More often, they are described as morbilliform or maculopapular. The objectives of this study were to characterize these skin reactions and to look for severity markers. METHODS: We retrospectively analyzed 133 cases of drug reactions collected in a Dermatology unit from 1973 to 1995 for whom, photographs and blood cell count were available. Well recognized disorders were excluded (Stevens-Johnson syndrome, Lyell syndrome, TEN, AGEP, vasculitis, phototoxicity, fixed drug eruption). Severity was defined on 3 criteria: hospitalization, skin reaction prolonged more than 21 days and visceral involvement. For each criteria, we studied the association with clinical and laboratory findings, first with univariate analysis, then with multivariate analysis for significant associations. RESULTS: Characteristics of the 133 included cases were: women: 58 p. 100, mean age: 52 years, inpatients: 80 p. 100, mean interval from beginning of drug therapy to reaction: 12 days, mean duration: 26 days, responsible drugs: antibiotics: 41 p. 100, nonsteroidal antiinflammatory drugs: 11 p. 100, anticonvulsants: 10 p. 100. A few reactions showed a monomorphous pattern fitting a single denomination in a classic dermatological nomenclature: maculopapular: 10 p. 100, morbilliform: 6 p. 100, scarlatiniform: 5 p. 100, small papules: 5 p. 100. Polymorphous reactions were observed in 73 p. 100 of cases with 3 different patterns on the average. Nearly half of the cases exhibited maculopapular and scarlatiniform lesions simultaneously. We observed fever (61 p. 100), mucosal lesions (26 p. 100), lymphadenopathy (30 p. 100), eosinophilia (count > 500/mm3: 44 p. 100, > 1500/mm3: 17 p. 100), and visceral involvement (22 p. 100), including hepatitis (18 p. 100). Four severity markers were identified: fever, lymphadenopathy, erythema involving more than 60 p. 100 of the body surface area, eosinophilia. CONCLUSION: This study underlines the polymorphous clinical presentation of skin reactions to drugs and the minority of maculopapular or morbilliform patterns leading us to propose the term of "erythematous drug reactions". Despite several biases (hospital recruitment of more severe reactions, retrospective analysis) this study is the first to show some severity markers. Close to the criteria used to define the "hypersensitivity syndrome" these simple clinical markers may have important practical implications.
