Mandibular angle implants.

A new type of implant designed to increase the angle of the mandible in three dimensions is described. Made of silicone and shaped like a saddle, this implant makes the technique of implantation relatively simple, and its versatility makes it a desirable addition to the tools for skeletal framework reconstruction of the face. The satisfaction rate has been high and the complications easy to manage. The surgical technique consists of an intraoral incision and the development of a subperiosteal pocket at the mandibular angle area, often performed under local anesthesia.

Augmentation cheiloplasty.

The description of a technique of augmentation cheiloplasty is given. It involves the advancement of two triangular distally based flaps in a V-to-Y technique. The plication of the orbicularis muscle adds to the thickening effect to the lip. The scars are inconspicuous, and the results are satisfactory with minimal complications.

Male chest correction. Pectoral implants and gynecomastia.

Implants have been developed in different sizes and shapes to mimic the pectoralis major muscle and its nipple-areolar complex below it. These soft, solid silicone implants have the approximate dimensions in height, length, and thickness as the muscle the implant is to mimic. Some improvements in the treatment of breast hypertrophy have also allowed a better result in the surgery of gynecomastia. The challenge of obtaining a smooth and flat result has been helped by the technique of liposuction with circum-areolar excision.

Calf contour correction with implants.

Implants have been developed in a shape allowing mimicking of the muscle while they are placed on top of it, thus giving a natural appearance to the leg. The author presents a satisfactory technique of calf implantation and its potential, although uncommon, problems.

Surgical treatment of gynecomastia in the body builder.

The surgical correction of gynecomastia presents particular problems in the body builder who takes medical treatment of anabolic steroids. In 38 corrections, no fat suction was necessary and only an extensive surgical extirpation of the gland was performed. Superior cosmetic results were obtained with this treatment, with the only problem being increased bleeding in these patients who have very large chest-wall muscles and an enlarged circulation.

Calf implantation.

The process of calf implantation has been standardized by the author, who uses routine incisions and develops a subfascial pocket before inserting the oblong implants of different sizes. The evolution of the technique has led to a series of satisfactory results and a small percentage of complications. One-hundred and twenty-one patients with implantation had satisfactory increases in calf circumference. Complications included infections and capsule formation, which were dealt with in a satisfactory manner.
