[Automated diagnosis--new possibilities through user-specific software].

After four years of development and testing, we have devised a software system especially adapted to the recording, storage and monitoring of radiological findings. After full implementation of this system, sufficient practical experience has been collected for effecting a comprehensive assessment. The qualitative and quantitative aspects of a discipline-specific software are described in detail and can be used for comparison with other EDP systems. The two essential criteria for the practicability and acceptance of a system for automatized collection and recording of findings are a short interaction time at the input terminal and a high degree of logical and reference flexibility enabling individual formulation and patient-related specificity of the textual formulation of findings comparable with conventional sound dictation. By means of interlinking of information with other processing programmes it becomes possible to continue data processing for each case in both administrative and research aspects after a single input operation of patient and examination data.

[Evaluation of spontaneous renal extravasation of contrast media in the excretory urogram].

Spontaneous renal extravasation is the term used to refer to the radiological demonstration of contrast medium outside the collecting system without previous trauma, ureter catheterization, operation on the kidney or its vicinity, and without external compression during urography. In a review of 1300 intravenous urograms, 13 cases of extravasation of contrast medium were found to satisfy the above criteria. Differences in the radiological appearance may cause problems with regard to evaluation and classification. From a therapeutic point of view it is important to distinguish between two forms: transient sinus extravasation due to minute tears in the calyceal fornix after an increase in intrapelvic pressure and persisting rupture of the previously impaired renal pelvis, which may require surgical intervention.

[Extragonadal germ cell tumors--diagnostic and therapeutic aspects].

The extratesticular manifestation of a tumor of the germ cells without apparent involvement of the testicle is a rare disease. During the last few years, seven patients with extragonadal tumors of the germ cells have been treated at our hospital. Tumor site was the retroperitoneum in four patients and the mediastinum, the lung, and the pineal body in one patient each. Based on the clinical courses of the seven cases, the authors discuss the diagnostic and therapeutic experiences with extragonadal tumors of the germ cells of various histologies and localizations. In order to make possible a curative therapy, the treatment should be performed according to the therapeutic conceptions existing for comparable histologic grades of primary testicle tumors.

[Typical skeletal changes in metastatic neuroblastoma].

Compared with other solid tumours in childhood, neuroblastomas show a marked tendency to metastasise to the skeleton. The differentiation of these lesions from inflammatory and other malignant bone lesions in this age group is often difficult. The radiological findings in ten patients with metastasising and histologically confirmed neuroblastomas have been reviewed and the typical appearances in the skeleton are described. The most important features in the differential diagnosis are discussed and the significance of bone changes in the diagnosis of neuroblastoma have been evaluated.

[Percutaneous intramedullary bore-wire fixation of typical localized radial fractures in danger of dislocation].

During the years 1973 through 1981 127 of altogether 1156 patients with fractures of the distal end of the radius (representing 11%) were treated with a percutaneous intramedullary pin fixation at the surgical clinic, Grevenbroich. The technique was used in the treatment of transverse, oblique or marginal fractures with dorsal displacement (Colles' type) or palmar displacement (Smith's type) of the distal fragment. A follow-up examination of 120 patients revealed excellent results in 36.7%, good results in 53.3%, fair results in 9.2% and poor results in 0.8%. Of seven epiphyseal fractures of the distal and of the radius five had an excellent and two a good result. The total results were analysed regarding to the age and sex of the patients and the fracture types I-VIII of Frykman's classification and were compared with indication, technique, complications and results of different methods of operative fixation. The advantages of percutaneous intramedullary pin fixation are simple practicability, minimal injuries of soft tissues, low expenditure of personnel and equipment and avoidance of secondary repositions.
