Long-term results of varicocelectomy.

Between 1983 and 1985, 257 infertile men were examined for any present varicocele by means of the Doppler sonography, telethermography and palpation. Based on these findings, the spermiogram and the infertility history, high ligation of the spermatic vein was indicated in 89 patients with varicocele and a median duration of infertility of 36 months (6-88 months). Postoperative sperm examinations have shown a significant improvement in sperm count and morphology, but not in motility. The most significant drop was observed in germ cell concentration from 1.68 to 1.06 mio/ml (p less than 0.025 Wilcoxon signed rank sum test). The follow-up examination 6 years after the beginning of the study has shown that only 56 out of the 89 patients underwent surgery, whereas 33 patients refrained from it. Pregnancy rates were 42% (23 out of 55, 1 patient lost to follow-up) in the operated group and 45% (14 out of 31, 2 patients lost to follow-up) in the nonoperated group. The comparison of the two graphs showing pregnancy incidence clearly demonstrates that pregnancies in the nonoperated group occur earlier than in the operated group; it has to be noted, however, that several patients only refrained from being operated on because pregnancy had occurred before surgery was planned. On the one hand, our study confirms other authors' results, i.e. that pregnancy rates in the nonoperated group are relatively high despite present varicocele. On the other hand, the operated group achieved practically the same high pregnancy rate when monitored over a longer period of time.(ABSTRACT TRUNCATED AT 250 WORDS)

Reduced bladder capacity in patients receiving intravesical chemoprophylaxis with mitomycin C.

After complete resection of superficial bladder carcinoma (Tis-2, G1-3), 75 patients were treated prophylactically with either 20 or 30 mg mitomycin C. Six patients developed a substantial reduction in bladder capacity (less than 200 ml). In 2 patients radical cystectomy of an otherwise tumour-free fibrotic bladder was necessary. The results of long-term treatment (2 years) suggest that reduced bladder capacity correlates with the cumulative dosage of mitomycin C.

[Continent ileum reservoir with anastomosis to the urethra].

We report our experience with the directly to the urethra anastomosed continent ileal reservoir. Our results suggest that the S-Pouch is superior to the proceeding suggested by Kock both from clinical and urodynamic view. Continence is to be expected at an anastomosis of this pouch with the bladder neck, whereas at an anastomosis with the posterior urethra occasional cases of nightly incontinence were registered. This always happened when the neurovascular bundle had not been preserved intraoperatively. If an ileal reservoir is installed metabolic acidosis has to be carefully avoided.

[Bacteriology of the ejaculate--a useful study?].

In a retrospective study we analyzed the results of 318 bacteriologic tests of ejaculates from 175 patients with suspected infections of the prostate, epididymis and urethra. Only 55% of all bacteriologic tests were positive, and the bacteria most frequently found were those normally present in the anterior male urethra, enterococci and coagulase-negative staphylococci. These results indicate that bacteriologic testing of the ejaculate does not yield such reliable results as the segmented urine culture technique and the examination of expressed prostatic secretions.

Unilateral pyonephrosis with septic thrombosis of the renal vein and vena cava.

A case of unilateral abscess-forming pyonephrosis with thrombosis of the renal vein and of the adjacent wall of the vena cava will be presented. This septic thrombosis as a consequence of an antecedent septicemia appears to be a late complication of pyonephrosis. In the differential diagnosis renal carcinoma could not be excluded whether pre- nor intraoperatively.

[Therapy of anaerobic infections].

The treatment of anaerobic infections still poses various problems for the physician. The authors deal with the surgical, but also, and in particular, the antibiotic treatment of anaerobic infections. The in-vitro activities, indications, side effects and dosage recommendations of the first choice (clindamycin, metronidazole and ornidazole, cefoxitin, penicillin G) and second choice antibiotics (chloramphenicol, ticarcillin) are discussed.

[Use of antibiotics in a surgical clinic. A prospective study of the Waid Community Hospital, Zurich].

A study was performed to analyse antibiotic usage in a surgical department, with the object of providing a basis for improving antibiotic policies. Patients admitted to the Department of Surgery (134 beds) of the Waid Community Hospital in Zürich (400 beds) after 1st Jan. 1979, and who received antibiotics during the period between 22 Jan. and 31 March were included in the study (n = 154). Daily chartreview provided all necessary information on clinical, diagnostic and therapeutic data of the patients included. 154 patients had 178 treatment courses which were reviewed according to guidelines proposed by the "audits of Antimicrobial Usage" (1977) and categorized as follows: I. agree with indication, appropriate choice and use of antibiotics; II. same, inappropriate choice and/or use of antibiotics; III. administration not justified. 83 of 178 treatment courses were prophylactic and 95 therapeutic. 58% of the prophylactic treatment courses were not justified (cat. III), and the remaining 42% were inappropriate in choice and/or use of antibiotics (cat. II). 41% of therapeutic treatment courses were adequate (cat. I), 43% were indicated, but these patients would have benefited from a more appropriate choice and/or use of antibiotics (cat. II), and in 16% the use of an antibiotic was not justified (cat. III). These results are compared with similar studies. On the basis of our findings, appropriate propositions for improvement of antibiotic policies are discussed.
