Persistent immunologic abnormalities in long term survivors of kidney transplantation: role of different immunosuppressive treatments.

Although immunosuppressive treatment with corticosteroids and azathioprine has become a widely employed and highly effective mode of therapy in a variety of conditions, its influence on cellular and humoral immunity is still somewhat intriguing and controversial. In order to further investigate the status of the immune system during various maintenance immunosuppressive treatments, we have studied a group of 32 long term survivors of kidney transplantation who were treated with different immunosuppressive regimens. The present investigation demonstrates the presence of a persistent defect of T and B peripheral lymphocytes in long term kidney transplant recipients. Moreover, it shows that the immune deficits are influenced by the time-dose schedule of the maintenance immunosuppressive therapy but not by the duration of the treatment. In particular methylprednisolone in a daily dose schedule (8 mg per day as a single oral dose in the morning) with or without azathioprine (1.5-3 mg/kg per day as a single oral dose in the morning) produces a global defect of T and B cell populations, while the same dosage of methylprednisolone administered every other day (16 mg every other day as a single oral dose in the morning) in association with daily doses of azathioprine exerts a sparing effect on T cell populations. The humoral parameters do not show definite abnormalities. The factors that may have contributed to the genesis of the immune imbalances we have detected are discussed.

Different survival of long-term kidney transplant recipients treated with daily or alternate day corticosteroids.

To define the effect of different immunosuppressive treatments on survival of long-term kidney transplant recipients, we have followed, for a period of four years, a group of 18 long-term survivors (more than three years) of kidney transplantation who were subjected to two different corticosteroid regimens (daily or alternate day corticosteroids) plus azathioprine. THis study not only confirms that an alternate day corticosteroid treatment reduces the incidence of infectious episodes and of side effects, but also suggests that it may steadily improve the survival of these patients. One of the six patients who performed an alternate day corticosteroid regimen died and only one lost renal graft function. On the contrary, three of the twelve patients who received daily corticosteroids died of causes related to immunosuppressive treatment, while another five patients lost renal graft function. A very low level of peripheral blood lymphocytes, of early E rosette and E rosette-forming cells was detected in patients who died of causes related to immunosuppressive treatment.

[The use of methylprednisolone in the therapy of renal transplants from cadavers].

The role of steroids, and specifically of methylprednisolone, in the pharmacological treatment of cadaver kidney transplants is discussed. The action mechanisms of the substance, and the modalities for its use in basic treatment of the transplanted organ and in rejection crisis are described. Finally, the results obtained in 147 transplanted patients, the complications involved and the measures taken to eliminate, or at least reduce, the problems of prolonged steroid therapy are reviewed.

[Whole-body osseous scintigraphy and Ca++ kinetics in patients with renal transplants].

Bone scintigraphy with 99mTc-diophosphonate and a whole body scanner was performed on 18 subjects with good renal function who had received a kidney transplant more than six months previously. This demonstrated scintigraphyc abnormalities in 17 of the cases. The alterations were characterized in 44,5% of the cases by focal uptake of the tracer. Diffuse alterations were observed in 4 of the 18 cases studied (22%). In these patients increases of serum alkaline phosphatase and in PTH levels were present. Ca++ exchangeable pool values and accretion rate were normal in the subjects with focal lesions. In the cases where scintigraphy showed diffuse lesions both the above parameters were much increased.

[Ocular complications in patients undergoing long-term hemodialysis and renal transplants from cadavers].

In 36 subjects in chronic hemodialysis treatment and in 36 patients with a renal transplant, well functioning for more than one year, the ocular complications related to the specific form of treatment were evaluated and compared. In the group of the dialyzed subjects ocular pathology was composed by: lens opacities in 9 cases, corneal calcifications in 7 cases, hypertensive fundus in 8 cases. In no patient these lesions produced measurable visus reductions. In the transplant group ocular pathology was composed by: lens opacities in 28 patients, hypertensive fundus in 7 and intraocular hypertension in one. One case of herpes cheratytis and one case of endophtalmytis have also been observed. In 9 patients these lesions produced a remarkable visus reduction. In conclusion renal transplantation shows a greater incidence of ocular complications if compared to hemodialysis. This situation has not important consequences on the rehabilitation of the transplanted subject for the possibility of surgical correction of the cataract.

Catheter lock solutions: it's time for a change.

In recent years, the number of patients on hemodialysis (HD) with central vascular catheters (CVCs) has grown. However, CVC use is often associated with an important risk for catheter related bloodstream infections (CR-BI) and inadequate dialysis due to flow problems. In this study, we reviewed alternative solutions to heparin for locking HD CVCs. Several experiences have demonstrated that trisodium citrate (TSC) (30-47%), citrate (4%) and taurolidine (1.35%) solutions are effective and safe for the prevention of CRBI, while heparin stimulates biofilm formation. High citrate (47%) concentrations can also provide significant advantages in reducing catheter clotting, but controlled studies with larger populations are necessary to confirm and to extend the use of such solutions in clinical practice. Side effects with high sodium citrate concentrations have been reported only immediately after locking, the symptoms are probably caused by a temporary drop in ionized calcium and magnesium, but it is evident that these solutions should only be used by skilled and authorized personnel, with a rigorous protocol.

Dimensional and hemodynamic differences between native and transplanted kidneys, evaluated by color Doppler ultrasonography.

Aim of the study was to asses the differences in size and hemodynamics in the normal kidney and well-functioning renal graft by color Doppler ultrasonography (CDU). Sixty healthy subjects, 75 well-functioning cadaver renal transplant recipients, 15 couples of living donors and related graft recipients were compared by CDU. Renal diameters, volume, renal blood flow (RBF) and renal resistance index (RI) were the variables studied. Cadaveric transplants, living donors and related recipients had a longer kidney (p<0.00001) and greater volume (p<0.001) than normal native kidneys. This was not associated with any significant increase in RBF. RI was lower in healthy subjects and in kidney donors than in transplant recipients (p<0.00001). Transplanted kidneys had a higher arterial RI but apparently normal function.
