[Multiple sclerosis and pregnancy (two additional cases)].

The authors made a short analysis of the epidemiology, etiology, pathogenesis, clinical presentation and treatment of multiple sclerosis. There were presented facts about the influence of pregnancy over the progress of this neurological disease, and the effect of multiple sclerosis on the pregnancy and delivery development. There also were described two clinical cases of pregnant women with multiple sclerosis, who delivered in the last 10 years in Clinic of Obstetrics in MBAL-Pleven.

[An evolution in the conduct for ectopic pregnancy in gynecological clinic, "UMBAL - D-r G. Stranski", Pleven for 7 years' period].

AIM: The aim of the present study is to show the evolution of the conduct for ectopic pregnancy for 7 years' period after introduction in gynecological practice of contemporary diagnostic and therapeutic methods for ectopic pregnancy. For the fulfillment of this aim was made a prospective study for 7 years' period of the patients with diagnosis "Ectopic pregnancy", treated in Gynecological clinic of "UMBAL - D-r G. Stranski" EAD, Pleven. MATERIALS AND METHODS: The objects of observation were 198 women with diagnosis: "Ectopic pregnancy". There were used the following methods: clinical, technical devices, statistical methods. RESULTS: The authors analyze the results of the use of laparoscopy, conventional surgery and application of Methotrexate describing the indications and the risk for the patient. CONCLUSION: The authors emphasized the advantages of the gynecological laparoscopy for precise diagnosis and contemporary treatment of the intact ectopic pregnancy.

[Gynecological laparscopy, endometriosis and sterility].

The authors presented the importance of the gynecological laparoscopy for diagnosis of the external genital endometriosis in its different forms. In the last 20 years, it was seen that laparoscopy is not only the basic method for diagnostics but also for the surgical treatment of the endometriosis. Thirty to fifty percent of the females with endometriosis in reproductive age have available sterility. The authors presented the cases, which have been laparoscopically proven for endometriosis and sterility for 7 years' period in Gynecological clinic in MBAL-Pleven and the results from the treatment with analog of the Gonadoliberins (Zoladex).

[Results of diagnostic hysteroscopy in a 7-year period in the gynecological clinic of "UMBAL-Pleven"].

The aim of the authors is to show the data for the reception diagnosis, age, histological results and the conduct after the performed diagnostic hysteroscopies in Gynecological clinic of UMBAL-Pleven. For the fulfillment of this aim was made a prospective study for 7 years' period: from 01/01/1997 to 31/01/2003. The objects of observation were 74 women of age from 16 to 65 years, with performed hysteroscopies for gynecologic complaints. There were performed 74 diagnostic hysteroscopies for the studied period. The hysteroscopic findings were 20 cases with endometrial polyposis, 14--submucosal myoamatic nodes, deforming the uterine cavity, 4--cervical polyp, 19--increased endometrium, 9--Asherman syndrome, 1--bicomous uterus, 1--a suspected section for endometrial carcinoma and 6 cases without pathologic findings. There were performed 59 trial abrasions and the removed materials were sent for histological examination The performed comparative analysis between the hysteroscopic presentation and histological findings showed a coincidence of the diagnosis. It was made the conclusion, that the hysteroscopy is an easy, accessible and inexpensive diagnostic method, which must take its place as one of the basic contemporary diagnostic methods in gynecology.

[Gynecological laparoscopy and treatment of ectopic pregnancy].

The aim of the present study is to show the advantages of the gynecologic laparoscopy for the diagnosis and treatment of the intact tubal pregnancy. For the fulfillment of this aim was made a prospective study for 5 years' period of the patients with diagnosis "Ectopic pregnancy", treated in Gynecological clinic of UMBAL-Pleven. The objects of observation were 33 women with diagnosis: "Intact ectopic pregnancy". Methotrexate was used by plan for 6 patients, in 7 patients was made laparotomy, and in 20 patients--gynecological laparoscopy. From the performed 20 gynecological laparoscopy, 16 women were recovered laparoscopically, in 14 of which was made partial salpingectomy, and in 2--milking. In one of the last two patients was injected Methotrexate in the bed of the gestational bag. In 4 women was necessary laparotomy, because of impossibility of performing of laparoscopic surgery. The authors emphasized the advantages of the gynecological laparoscopy for precise diagnosis and contemporary treatment of the intact ectopic pregnancy.

[Progress in methods of therapeutical abortions during the second trimester in gynecological clinic "Umbal - Pleven"].

The aim of the authors is to discover the progress in the methodology for the interruption of pregnancy by medical indications during the second trimester in Gynecological clinic in UMBAL - Pleven. A retrospective study has been performed for the used methods in the last 10 years. For the same period were performed abortions in 49 women by medical indications. The interruption of pregnancy until the 16th gestational week (g.w.) of gestation was performed in one-stage. From the 16th g.w. to the 20th g.w., the most common method till the year 2001 was intraamniotic instillation of 25% solution of NaCl by Aburel (1934). After the 20th g.w. the pregnancy was interrupted either by metrierasis (using balloon catheter), or by sectio parva. After the 2001 was described radical change in conduct by therapeutic abortion during the second trimester, after the beginning of the prostaglandins use in obstetrical practice. The authors analyze the results of the use of prostaglandins describing the type, dosage of used medicine, period of abortion, contraindications and the risk for the mother. They reach to the conclusion that the prostaglandins are presently suitable means for therapeutical abortion during the second trimester, which fully replace the classical, traumatically and risky methods for the pregnant woman.

[Treatment of tubal pregnancy with methotrexate in gynecological clinic of MBAL-Pleven during five year period].

The authors presented a short survey for application of Methotrexate for treatment of intact ectopic pregnancy and its usage in Gynecological clinic of MBAL-Pleven during the last five years. There was made a prospective study, and for the studied five years' period in the clinic there were treated 149 women with ectopic pregnancy, 33 of which were hospitalized with intact tubal pregnancy. Methotrexate was used by plan for 6 patients as for 3 of them the treatment was successful. The authors also described two of these cases according to the diagnosis, way and usage period of the cytostatic and the result from the disorder. In conclusion there was emphasized that the treatment of intact tubal pregnancy with Methotrexate has a place in gynecological practice in view of the fact that it decreased physical pain, psycho-emotional stress and bed staying of the patient, but the cases must be strongly selected.

[Gynecological laparoscopy and Mayer-Rokitansky-Kuster-Hauser syndrome (report of three cases)].

The syndrome is described in 1838 and it is named after the four authors. It includes combined agenesis of uterus, cervix and the upper 2/3 of the vagina, but the fallopiantubes and ovaries are normally presented. Expect abnormalities of the female reproductive system there are abnormality of the kidneys, vertebral bodies and loss of hearing. The authors of the present publication describe in details etiology, pathogenesis, clinical presentation, differential diagnosis and therapeutic methods for syndrome's treatment. There are presented three clinical cases, which are by laparoscopy proved. In conclusion it is indicated the importance of the gynecological laparoscopy like a modern method for diagnosis of the congenital anomalies of the female genital tract leading to primary amenorrhea.

[Paraurethral cysts--diagnosis, differential diagnosis and treatment (a case report)].

The aim of the authors is to reflect clinically upon a rare pathology of the female genital system vaginal cysts. The presentation includes description, diagnostical methods, differential diagnosis and treatment of the paraurethral cysts and urethral diverticulum. It was fully described on the basis of clinical case from the gynecological practice on a patient with paraurethral cyst, which was removed surgically with good outcome and result.

[Aids and pregnancy--contemporary aspects of the problem].

The authors made a short analysis of the epidemiology, pathogenesis, diagnosis of the HIV-infection and AIDS. There was presented contradictory attitudes about the influence of the pregnancy over the progress of the HIV-infection and AIDS, and the effect of HIV-infection and AIDS on the pregnancy development. There was also described the injuries of the fetus and newborn. There was exposed conceptions about the obstetric behavior and the way of delivery by pregnant women, who have been infected with HIV, AIDS sick persons and zidovudine treated patients.

[A rare case of ovarian cancer in a patient treated for secondary sterility].

The authors described a rare case of ovarian cancer in a young woman who looked for gynecological help because of secondary sterility. There were found cysts of the both ovaries, which were conservative treated by insistence of the patient for three months. After unsuccessful gestagenic therapy there was performed a gynecological laparoscopy, which showed that on one of the ovaries there was an exophyte growths suspected for carcinoma tissue. It was offered an emergency operation, which was made only after 1,5 months because of personal causes of the patient. It was taken a sample that was with unconvincing data for ovarian cancer. The clinical experience and professionalism of the surgical team were put on the test. It was performed a radical operation with removal of the uterus, adnexa uteri, partial peritoneoectomy, total lymph dissection, appendectomy, total omentectomy. The patient was discharged from hospital in good condition and she is liable to postoperative chemotherapy.

[Gynecologic laparoscopy and Swyer syndrome (two additional cases)].

The syndrome belongs to the group of the congenital abnormalities and is also known as syndrome of the pure gonadal dysgenesis with the presence of medullar elements. The girls with the Swyer syndrome are characterized by the presence of the male karyotype--46,XY and they are sex-chromatin negative. The authors of this publication describe in details etiology, pathogenesis, clinical presentation, differential diagnosis and therapeutic methods for syndrome's treatment. There are presented and two clinical cases, proved by chromosome analysis and verified by laparoscopy. It is about the significance of gynecologic laparoscopy for precise diagnostics in congenital anomalies of the genital system.
