Suction fixation system for stereotactic radiosurgery of intraocular malignancies.

We designed a suction fixation system for the radiosurgical treatment of intraocular malignancies with the Leksell gamma unit (gamma knife). OUr device consists of a circular suction chamber and an adjustable unit to be fixed to the Leksell stereotactic head frame. All components are made of plastic materials in order to avoid artifacts in CT or MRT imaging. A permanent suction of 600 to 800 millibars is provided by a standard vacuum pump, powered by a portable battery. Suction times up to 40 minutes were well tolerated in all cases. In the gamma knife of the Neurosurgical Department at the University of Vienna, we successfully used this device. Up to January 1994 we have performed 19 radiosurgical treatments in 9 patients with large or extra-large uveal melanomas and in one patient suffering from a choroidal metastasis.

In the Gamma Knife Unit of the Neurosurgical Department at the University of Vienna, we successfully used this device. Up to April 1994 we successfully performed 25 radiosurgical treatments in 12 patients with large or extra-large uveal melanomas and in 1 patient suffering from a choroidal metastasis.

[Ondansetron (GR 38032F), a competitive 5-HT3 receptor antagonist as an antiemetic in cytostatic drug-induced nausea and vomiting. An open, substance-oriented phase II/III study].

In an open, drug-oriented phase-II/III-study 24 patients were treated with the 5-HT3-antagonist Ondansetron as an antiemetic drug for chemotherapy-induced nausea and emesis. Patients with treatment regimen containing cisplatin were excluded. All patients had suffered from severe nausea and vomiting under conventional antiemetic drugs during a previous identical chemotherapy cycle and were treated with 8 mg Ondansetron t.d. on the day of the chemotherapy and on the four following days. The drug was given with 90 cytostatic cycles ranging from 1 to 14 cycles per patient. Only 2 patients (8%) did not experience an improvement of their symptoms in any of the treatment cycles as measured by a self-conducted grading of nausea and by the frequency of vomiting in comparison to a previous treatment cycle under conventional antiemetic therapy. Eleven out of 18 patients, who were treated with Ondansetron more than once (61%) noted a diminished frequency of vomiting in each treatment cycle with Ondansetron. Sixty of the 90 therapy cycles with Ondansetron resulted in complete (no vomiting) or major (one to two vomits within 24 h following chemotherapy) protection from emesis (37 and 29 per cent, respectively). The most frequent side effect noted was obstipation (7 patients), followed by slight diffuse abdominal pain (4 patients, probably also due to chemotherapy) and slight to severe headache (3 patients, 1 patient was therefore withdrawn from the study). No other side effects were seen. In conclusion, our study indicates that Ondansetron is an effective and safe drug for the treatment of cytostatic drug-induced nausea and vomiting.

Estimation of energy expenditure by a portable accelerometer.

A small portable accelerometer was developed to estimate the energy expenditure of daily activities. The accelerometer is reported to be an improvement over movement counters currently on the market. The oxygen requirement of 14 different activities was measured in 21 subjects while each wore the accelerometer on the waist. A movement counter (mercury switch), which is available commercially, was also worn on the waist and another was worn on the left wrist. The reproducibility of the accelerometer readings was high (4 subjects, 14 activities; r = 0.94) and was superior to either the waist movement counter (r = 0.63) or the wrist movement counter (r = 0.74). In estimating oxygen requirement (VO2) the standard error of estimate, based on 21 subjects and 14 activities, was 6.6 ml X min-1 X kg-1 for the accelerometer. This was also better (smaller) than for the waist movement counter (9.2 ml X min-1 X kg-1) or for the wrist movement counter (7.9 ml X min-1 X kg-1).
