[The tooth transplant in orthodontic treatment planning].

The analysis of all examined transplant cases combined treated with orthodontics shows a success rate of 76%. It must be underlined that the germ development at the time of the transplantation is of utmost importance. The best possible time proved to be at a root development of 2/3 to 3/4. Exact indication position, a careful atraumatic operation method and a short time of operation are also important factors of a successful transplantation. If all these preconditions are met, this method can be recommended.

[Periodontal development after tooth germ transplantation within the framework of orthodontic treatment].

The normal development of the periodontal structures was confirmed histologically on young beagle dogs following tooth germ transplants. In humans, clinical and radiographic examinations revealed no pathology of the periodontium following transplantation of tooth germs. The grafted teeth could be moved orthodontically in the same way as any other teeth. No precautions need to be taken. This paper describes the most common indications for transplanting the tooth germs of canines, bicuspids and molars.

Adolescent

[Sandwich osteotomy versus interpore implants for maxillary versus mandibular ridge augmentation].

Horse-shoe sandwich osteotomy with interposed cancellous bone for augmentation of highly atrophic maxillas is described. 2-year results were encouraging in terms of denture fit, but long-term results are needed for a definitive assessment. For severely atrophic mandibles the use of porous hydroxyapatite (Interpore 200) is shown to be beneficial and prevents further bone resorption. After complete healing dislocation and loss of ridge height were absent, except in one special case. Subsequent vestibuloplasty for relative augmentation proved to be unnecessary. For a definitive assessment of this clinically successful method long-term results will have to be waited for.

Alveolar Ridge Augmentation

[Fixed appliances current state of the art].

Recent advances in fixed appliances are reviewed and discussed. These include cosmetically more appealing brackets (ceramic, lingual); new wires (nickel-titanium and titanium-molybdenum alloys); new biomechanical approaches (accurate computation of forces and torques according to Burstone resulting in the use of new mechanical concepts); experiences in the treatment of transplanted teeth made in the past 15 years; new approaches to the orthodontic finish (implementation of Andrews' "six keys to normal occlusion" with straight wires, accurate positioning of brackets, and gnathological positioners). Cases will be demonstrated.

[Reconstruction of upper and lower jaw defects by microsurgical bone grafting].

Indications for microvascular bone grafts in maxillo-facial surgery are defined. Surgical procedures at the main donor sites for upper and lower jaw reconstructions, vascularized iliac bone grafts and scapular flaps, are described. Applications are demonstrated in various cases. Vascularized growth-plate transplantation for reconstructing hemifacial microsomia introduces new possibilities. Microsurgery is a safe method with a high success-rate (94%) and a low morbidity at the donor site.

Adult

[Autotransplantation: technic and long-term results].

Dental transplantation can be a useful and efficious technic. It was first described by Ambroise Pare and the authors have had over 12 years success and failures enough to present clinical cases and illustrations of the surgery and the orthodontic treatment. Success depends essentially on the right indication, the time and the exact methodical technic, reproducible in a treatment plan.
