The vertical dimension of occlusion refers to the length of the face as determined by the amount of separation of the jaws. Its determination is important for fabrication of all restorations. Many techniques have been used for measurement of the vertical dimension of occlusion in dentulous and edentulous patients. These range from using preextraction records to the use of swallowing, functionally acquired jaw positions associated with phonetics, and cephalometric radiographs and evaluation of radiopaque paste in the vestibular fornix. There is no universally accepted or completely accurate method of determining the vertical dimension of occlusion in edentulous patients. There seem to be no significant advantages of one technique other than those of cost, time, and equipment requirements. It is the end result that matters. It should be satisfactory to the dentist and the patient from an esthetic point of view and not induce degenerative changes from a functional standpoint. Regardless of the technique, the vertical dimension of occlusion must be determined carefully by the dentist for a successful prosthesis.

Impressions were made from the anterior segment of the mouth in 25 subjects with their teeth in centric occlusion. Measurements were made on each impression to determine the relation of the maxillary and mandibular vestibular folds and of the anterior teeth to the maxillary and mandibular mucolabial reflections. The mean distance between the depth of the mucolabial reflections in the canine region was 36.70 mm for the right side and 36.94 mm for the left side. The mean distances for the right and left central incisor regions were 34.20 mm and 34.06 mm. The variations in the measurements of different teeth confirmed the anatomic individuality of each patient. Nevertheless, the findings of this study will be helpful as guides in the initial placement of the maxillary and mandibular anterior teeth and in the initial determination of the vertical dimension of occlusion.
The connective tissue reactions to 3% tetracycline ointment were studied in 14 Sprague-Dawley white male rats. Using polyethylene tubes, the ointment was implanted subcutaneously in the pouches surgically created on the backs of the experimental group. Empty tube implants, tubes with the vehicle (vaseline/lanolin), and a sham operation (surgical pouches with no implant) served as controls. The animals were killed on day 14 and tissue blocks were taken containing the tubes and a generous amount of the peripheral connective tissue. The connective tissue surrounding the tube opening furthest from the surgical incision was histologically examined for the severity of tissue reaction (STR), the number of inflammatory cell infiltrates (II), and the spread of the reaction area (RSI). The data were statistically analyzed. The sham operation group showed minimal inflammatory response. All three parameters (STR, II, RSI) were significantly greater in the tetracycline group when compared with the empty tube group at 0.05 level of significance. There were significant differences between the mean values of the STR and RSI scores in the tetracycline and vehicle groups (groups 1 and 2). The vehicle group (group 2) had significantly higher STR and II values than the empty tube group (group 3); however, the RSI scores were not statistically different. Necrosis was observed in the reaction site in group 1 and vacuole-containing macrophages were noted in groups 1 and 2. This study, although an animal investigation, questions the use of topical 3% tetracycline ointment on sutured surgical flaps.
Data from three different sources of periodontal patients were collected and examined to find if there was a significant difference between these groups relative to the prevalence of medical problems. Data were evaluated to show if age and sex were significantly related to the existence of medical problems and to determine which problems were the most prevalent. A total of 581 periodontal patients' records were obtained. Results of the evaluation showed that the private office group had 27.6% medical problems, the academic dental center had 46.3%, and the hospital dental clinic had 74.1%. Cardiovascular disease was the most prevalent medical problem in all groups and orthopedic disease or injury was second. Within each group, sex was not significantly related to the presence of a medical problem. Age was a highly significant factor, with the prevalence of medical problems increasing with advancing age.

Fifteen biopsy specimens from hyperplastic maxillary anterior freni were examined histologically by three investigators, and tissue components were subjectively rated on a predesigned scoring sheet. The presence of calcification and ossification associated with high levels of inflammatory infiltrates and reactive connective tissue confirm the fact that surgical excision of the hyperplastic frenum is warranted.
A total of 250 wild sheep (Ovis orientalis) from different national parks and protected regions of Iran were examined for gastrointestinal nematodes at necropsy. Twenty five species of nematodes were found. Marshallagia marshalli, Ostertagia spp; Nematodirus spp; and Skrjabinema ovis were the most prevalent. Although all the species found are recorded from wild sheep for the first time in Iran, 88% were reported previously from domestic sheep. New host and distribution records for Nematodirus davtiani, N. gazellae and Nematodirella longissimespiculata were established during the present study.

Four different species of helminths: Anisakis spp. Diplozoon paradoxum, Aspidogaster limacoides and Asymphylodera kubanicum were found in 1.4%, 28%, 15% and 5% of 140 Rutilus frisii katum from the south Caspian Sea respectively. Generally, the number of worms collected was low and cannot be considered pathogenic to examined fish.
Two species of fish, tuna fish (Euthynnus spp.) from the Persian Gulf and pikeperch (Lucioperca lucioperca) from the Caspian Sea were examined for nematode larvae. The larvae of Anisakis which is considered to be one of the zoonotic helminths, was found in 75 percent of the tuna fish and 20 percent of the pikeperch. The possible role of fish as a source of infection in man in Iran is discussed.
Forty-six anergic patients (37 males and 9 females, age range 55-79 yr) were selected from ninety-one patients suffering from COPD due to frequent exacerbations and impaired delayed cutaneous reactivity (43.9%). The phenotype of circulating lymphocytes, their proliferative response to a panel of polyclonal T-cell activators and the candidacidal activity (CA) of circulating PMNs (polymorphonuclear cells) were measured. In 13 patients presenting a defective CA of circulating PMNs, the in vitro response of alveolar macrophage CA to r-IFN-gamma was also determined. We found: 1) a significant reduction in the CL response to PHA in COPD patients vs controls; 2) a low PMN-CA in 23 (57%) COPD patients; 3) a non-significant difference in phenotype analysis in patients and controls; 4) lower CA of AMs in COPD patients than in controls; 5) restoration in vitro of CA by r-IFN-gamma in the group of anergic COPD patients presenting depressed CA. We conclude that a defective cell-mediated immunity could be the basis of the enhanced susceptibility to infectious exacerbations in many COPD patients and that, in vitro, it could be reversed by r-IFN-gamma treatment.
The diagnosis of lung coin lesions (peripheral lesions with a diameter of no more than 6 cm) is one of the most important problems in pneumology. Among the numerous causes of coin lesions, the presence of malignancies must always be excluded. The Authors describe an unusual case of lung arteriovenous fistula in a patient suffering from Rendu-Osler-Weber disease. The importance of a multi-step diagnosis, starting with non-invasive procedures is stressed; this is to avoid life threatening complications which may occur after lung trans-thoracic biopsies of previously undiagnosed vascular abnormalities.
