[Diagnostic possibilities of gamma-topography using 111In-bleomycin in malignant tumors of the orbit].

Despite the fact that 111In-bleomycin is not an absolute tumorotropic RP, the results of the investigation have shown that its use is of great diagnostic importance for the recognition of orbital tumors and for the evaluation of their dissemination. True-positive results of gamma-topography were noted in 90% of cases. In some cases 111In-bleomycin permitted differential diagnosis between certain tumor types and inflammatory orbital processes. This RP was mainly accumulated in lymphosarcoma, primary and secondary orbital melanoma.

[Late results in treating uveal melanomas. A comparative evaluation of enucleation and organ-preserving treatment methods].

The paper analyses late results after treatment of 629 patients with choroidal and ciliary body melanomas. The method of drawing up thorough tables of mortality allowed to receive indices of 5-year survival and lethality after enucleation and organ-preserving methods of treatment. Comparative assessment of effectiveness of enucleation and organ-preserving methods of treatment proves the advantage of the latter in case of small uveal melanomas in the plan of preserving the life, ability to work and professional qualification of the patients.

[Combined use of computed tomography and gamma-topography in orbital diseases].

The authors analyzed the results of combined investigation of 103 patients with primary, secondary, benign and malignant orbital tumors and pseudotumors. CT high efficacy in the above tumors was shown. The results of gamma-topography (GT) of the orbit were shown to depend on the volume of a tumor and its histological type. The combined use of CT and GT made it possible to raise the significance of preoperative diagnosis and to predict organ-sparing therapy.

[Experience in the treatment of eye tumors using a narrow medical proton beam].

This paper presents the effects of proton beam therapy of 175 patients with tumors of the eye. Irradiation was performed with a narrow medical proton beam (Institute of Theoretical and Experimental Physics) with the energy of 70 MeV (Bragg's peak). The total dose was 6000-7000 rad, irradiation frequency was 5-6 sessions every other day. Irradiation was used to treat intraocular and orbital melanomas, melanomas an cancer lesions of the eyelid and conjunctiva. According to the WHO classification, tumors fell under the T2-T3N0M0 stage. The follow-up periods varied from 1 to 10 years (an average of 3.4 years). Complete tumor regression was observed in 45% of the cases, partial regression in 39%, and the absence of the effect in 16%. Twenty-two patients (12%) underwent enucleation due to progressive tumor growth (16 patients) or postradiation complications (6 patients). Hematogenous metastases within a period of 2-5 years following irradiation were noted in 4% of the cases. The results obtained indicated a high efficacy of proton beam therapy for large ocular tumors. The mortality rate from metastases following irradiation was lower than that following enucleation.

Ciliochoroidal melanomas treated with a narrow medical proton beam.

We treated 63 patients with intraocular melanomas by means of a narrow medical proton beam. Tumors were irradiated with 2,500 rad at each of four to five sessions, with an interval of one to two days between sessions. The melanomas ranged in diameter from 8 to 20 mm and were from 3.0 to 13.7 mm in thickness. Patients were followed up for three months to seven years. In 11 cases, the tumor was fully resorbed. Complications included radiation cataract, postradiation glaucoma, radiation retinopathy, and exudative retinal detachment. In 12 cases, enucleation was performed because tumor growth persisted. Four patients died during follow-up period because of metastasis. The eye was preserved in 47 cases.

Value of fluorescein iridography in diagnosis of tumours of the iridociliary zone.

Fluorescein iridoangiography of 33 eyes with benign tumours and 22 eyes with malignant tumours is discussed. In cases of leiomyoma the tumour vessels have a distinct pattern, and fluorescence of the tumour tissue is mottled and short-lived. Malignant tumours are characterised by distortion of their vessels. Simultaneously the patches of fluorescence appear in the tissue of the tumour and lead to a diffuse confluence. Changes in the architecture of the vessels of the iris near the tumour and absence of fluorescence at the pupil margin indicate growth of the neoplasm.
