Carcinosarcoma of urinary bladder: report of 5 cases with immunohistologic study.

We studied 5 cases of carcinosarcoma of the urinary bladder. Immunoperoxidase studies were performed to identify the nature of the tumor and to establish the diagnosis. We suggest that these tumors represent a transition of epithelial to mesenchymal malignancy. Three of our patients had a history of radiation therapy prior to development of carcinosarcoma; of which 2 had a long interval between exposure to radiation and tumor development. Radiation is a risk factor for endometrial carcinosarcoma, and in this report we strongly suggest such an association for the urinary bladder.

Case report 751: Spindle cell lipoma causing marked bone erosion.

We present an unusual case of spindle cell lipoma of the foot which produced marked bone erosion. Spindle cell lipoma was initially described in the mid-1970s as having the characteristics of a lipoma with partial or total replacement of mature fat by collagen-forming spindle cells. This lesion is most commonly found in the subcutaneous tissue of the posterior neck and shoulder in men between 45 and 65 years of age.

Case report: malignant mesenchymoma of the mediastinum.

Malignant mesenchymoma is a malignant tumor arising in the soft tissues of the body. It is a rapidly growing tumor prone to recurrences and occasional metastasis. It occurs at all ages and locations but has a preference for the extremities and the retroperitoneum.

Evaluation of prothrombin time and partial thromboplastin time.

Prothrombin time (PT) and partial thromboplastin time (PTT) tests are commonly ordered for hospitalized patients. A significant reduction in laboratory workload and expense can be achieved if these tests are ordered appropriately. The likelihood that the patient will benefit from these tests is small.

Mucinous adenocarcinoma of the pancreas.

The authors report a patient with mucinous adenocarcinoma of the pancreas with multiple cystic spaces, lined with malignant columnar cells and papillary formation; adjacent pancreatic tissue shows chronic pancreatitis. This is an unusual case report.

Small cell carcinoma of the prostate with adenocarcinoma of the rectum.

Our case of a pure small cell carcinoma of the prostate was found concomitantly and separately from an adjacent primary adenocarcinoma of the rectum. There was no evidence of adenocarcinomatous differentiation in the prostate tumor and no primary small cell carcinoma was detected in other organs.

Diagnostic yield from 231 autopsies in a community hospital.

Autopsy reports of 231 adult autopsies were examined for clinically unexpected diagnoses. Analysis of data showed that 97 of 188 autopsy diagnoses were clinically unexpected. The diagnosis of pulmonary embolism was clinically the most frequently missed diagnosis relative to its actual occurrence. Also noted was that the proportion of cases with unexpected diagnoses increased from 1983 to 1987.

Silicone lymphadenopathy with concomitant malignant lymphoma.

Six cases of axillary lymphadenopathy induced by silicone elastomer following silastic finger joint arthroplasty have been reported; 2 cases had associated malignant lymphoma. This is the 3rd reported case of malignant lymphoma with concomitant silicone lymphadenopathy discovered 8 years after insertion of silastic finger joint arthroplasty for longstanding rheumatoid arthritis. Histology revealed a partially replaced reactive lymph node with immunoblastic lymphoma. Both sides contained single and clusters of multinucleated giant cells with silastic particles, as well as formed asteroid bodies. Malignant lymphoma is associated with rheumatoid disease, but to date, silicone particles have not been linked to malignant lymphoma.

Chondrosarcoma and malignant lymphoma.

We describe two primary malignancies occurring simultaneously. The coexistence of chondrosarcoma and malignant lymphoma, small lymphocytic type (MLSL) (WDLL) is a rare occurrence. In this case, the untreated malignant lymphoma appears to have increased the aggressiveness of the chondrosarcoma through its adverse effect on the host's immune system.

Cytological diagnosis of breast cancer.

One hundred thirty-one fine-needle aspirations and 85 nipple discharge smears were evaluated cytologically and correlated with histological diagnoses and with clinical impressions. This study demonstrates that cytological diagnosis of breast lesions is a useful and clinically accurate procedure.

Soft tissue metastasis of a chemodectoma: a case report and review of the literature.

A case of a single soft tissue metastases (abdominal wall) occurring 15 years after removal of chemodectoma of the right carotid bifurcation is described. The tumor had apparently been completely removed with excision of portions of the common, external and internal carotid arteries up to the base of the skull. Seven years later osteolytic and osteoblastic lesions were noted in the cervical and thoracic vertebrae which were radiated with 3500 rad. The 19 previously reported cases of chemodectomas with distant metastases are reviewed.
