Caring for older people. Carers.

Caring by families and friends is the backbone of community care. Carers face physical, emotional, social, and financial problems. They need recognition, information, and support from the health professionals with whom they and the person they care for come in contact. Much information is available to assist carers and to enable their doctors to help them in their caring role.

Pathogenicity differences between South African isolates of Ornithobacterium rhinotracheale.

Three selected South African Ornithobacterium rhinotracheale field isolates were identified and inoculated via the caudal abdominal airsac into 28-day-old broilers, which were monitored under controlled laboratory conditions. It was concluded from data that the O. rhinotracheale isolates were capable of causing primary disease, with statistically significant resultant mass loss. Respiratory and arthritis symptoms were reproduced. However, no sinusitis was observed. Airsacculitis- and arthritis-lesion scoring techniques showed significant pathogenicity differences between isolates. Furthermore, differences in symptomatology were also seen between isolates. A highly significant reisolation of O. rhinotracheale was made from the brains of broilers challenged with isolate 2. In conclusion, the economic importance and financial loss due to Ornithobacterium rhinotracheale infections in poultry were highlighted.

A survey of hospital toilet facilities.

OBJECTIVE: To assess the quality of toilet facilities available for disabled people in a large provincial teaching hospital. DESIGN: Survey of toilet facilities for patients on the wards and in the outpatient department. SETTING: Teaching hospital in Leeds. RESULTS: Although the quality of toilet facilities varied, none met the standards recommended by the British Standards Institution. The worst facilities were found on a ward accommodating elderly patients, where the toilets were unsuitable for use by disabled people and bedside commodes had to be used instead. CONCLUSION: Toilet provision within a major hospital failed to meet standards required for disabled people. Admission to hospital may therefore result in loss of independence and dignity. If hospitals are to be centres of excellence, greater consideration must be given to the requirements of disabled people in the design of new wards, and current inadequate facilities should be upgraded.

A fatality after antipyrine administration.

An 87-year-old woman died after the administration of an intravenous injection of antipyrine during the course of an investigation of the efficacy of oxerutins in venous ulceration. This is believed to be the first such fatality.

Concomitant Ornithobacterium rhinotracheale and Newcastle disease infection in broilers in South Africa.

Ornithobacterium rhinotracheale was first isolated from broilers in South Africa in 1991. The importance of O. rhinotracheale infections has been established, with growth suppression, respiratory symptoms, and arthritis commonly seen as complications. Dual infection with Newcastle disease and O. rhinotracheale in 28-day-old broilers led to more severe respiratory lesions and higher mortality rates than in birds with only Newcastle disease. It was concluded that the pathogenicity of Newcastle disease virus was enhanced when occurring in combination with O. rhinotracheale. Also of interest was the first isolation of O. rhinotracheale from the liver.
