Thyroid cancer among "liquidators" of the Chernobyl accident.

In 1986, immediately after the Chernobyl accident, the USSR Ministry of Health adopted a large scale programme of establishing an All-Union Distributed Registry of persons affected by radiation due to the accident. The registry was based at the Medical Radiological Research Centre of the Russian Academy of Medical Sciences (MRRC RAMS). In 1992, when the USSR was dissolved, this registry database contained information on 659,000 persons, including 284,000 Chernobyl accident emergency workers ("liquidators"). Currently, the Russian National Medical Dosimetric Registry (RNMDR) contains data on 435,276 persons, including 167,862 liquidators. This paper reviews the data for 47 verified thyroid cancers in the liquidator subgroup of the RNMDR. Analyses show that there is an excess relative risk of thyroid cancer per Gy of 5.31 (95% confidence intervals 0.04 and 10.58) and an excess absolute risk of thyroid cancer per 10(4) person-years per Gy of 1.15 (95% confidence intervals 0.08 and 2.22).

The Chernobyl accident and radiation risks: dynamics of epidemiological rates (morbidity, disability and death rates) according to the data in the national registry.

Ten years have elapsed since the Chernobyl accident. The gravest technologically generated accident throughout human history has attracted considerable attention from the whole world community. At the same time, the problem of estimating the total damage to life and health of people exposed to radiation remains very complicated. The negative effects of Chernobyl include a spectrum of factors which may reinforce each other. In particular, to date there are no theoretical models or practical recommendations on estimating the contribution of the social, psychological or emotional factors that surround diseases due to radiation accidents. On the other hand, for maximum effective rehabilitation of the affected population, the impartial determination of the contribution by both radiation and non-radiation components is necessary. Therefore, the continuation of long-standing investigations within the framework of the National and Radiation and Epidemiological Registry along with obtaining new scientific data in the field of radiation epidemiology is of great practical importance in limiting the health consequences of the accident.

[The results of medical and dosimetric research on the population and on those who worked in cleanup subjected to radiation exposure as a result of the Chernobyl catastrophe].

The Russian State Medical and Dosimetric Register (RSMDR) of subjects exposed to radiation due to the Chernobyl accident and a radiation epidemiological analysis of the data collected are described. At present, RSMDR covers 435,000 persons including 152,000 participants in the liquidation of the accident consequences (liquidators). Predictive estimates (based of the models recommended by the International Commission of Radiation Protection) and the actual data obtained 9 years after the accident are compared. It has been found that within 20 years after radiation in a cohort of liquidators an attributive risk for leukemia and for solid cancers will be 23.6 and 2.8%, respectively. The actual data after 10 years correspond with the predictive estimates. According to disability and morbidity rates, the 1986-1987 liquidators represent a high-risk group. Radiation epidemiological examinations for thyroid cancer in children (at the moment of the Chernobyl accident) were made in the Bryansk and Kaluga Regions. The relative risk for detected cancers was ascertained to be 7.15. This signifies that about 90% of the thyroid cancers detected are caused by a radiation factor.

Trace elements and thyroid cancer.

To evaluate the importance of trace amounts of elements in thyroid cancer etiology and diagnostics, instrumental neutron activation analysis has been used to estimate Ag, Co, Cr, Fe, Hg, I,Rb, Sb, Sc, Se, and Zn concentrations in malignant and benign thyroid nodules as well as in apparently intact paranodular thyroid tissue. Resected material from 135 patients was obtained from operations. Forty-five cancer cases were diagnosed and the rest were of benign nodules. The thyroid glands of 65 people, 53 male and 12 female, who died and unexpected death or committed suicide, were used as a control group. Trace element contents of the International Atomic Energy Agency reference material H-4 (animal muscle) were analysed simultaneously with the thyroid tissue in order to evaluate the accuracy of the obtained data. No dependence of trace element contents on sex and age (14-80 years) was found for normal thyroids. In paranodular tissue, the Ag, Co, Hg, I and Rb contents were much higher for malignant and benign nodules than they were for the standard. There was also a slight deficiency of Se in the nodules compared with the standard. This result supports the hypothesis that the direct toxic heavy metal influence on thyrocytes plays a major role in thyroid cancer etiology, provided that an adequate level of the defence mechanisms is absent. Iodine concentrations are 15 times lower, on average, in malignant compared with benign nodules. It is also shown that the radio between the iodine concentration in nodular and paranodular tissue can be used for in vivo thyroid cancer diagnostics.

'Rogue' cells observed in children exposed to radiation from the Chernobyl accident.

Eight 'rogue' lymphocyte metaphases containing a large number of aberrant chromosomes were noted during a survey of chromosomal damage in 328 Belarussian children. The study population comprised children of families living in territory contaminated by radiation from the Chernobyl accident. The majority of the sample had been evacuated within 1 week from very heavily polluted territory to areas that had received much less fallout. Two hundred cells were scored per subject and one rogue cell was found in a child exposed in utero; one in a child conceived after the accident and six in the postnatally exposed group. The possibility that the damage was due to exposure to radio-iodine concentrated in the thyroid gland, or to radiation from incorporated 'hot particles' of an alpha or beta/gamma emitter is discussed. It is concluded that the damage to these cells is unlikely to have been caused by radiation.

[Contact radiotherapy of rheumatoid arthritis].

The authors have devised a method of contact radiotherapy with open radionuclides (198Au) to be used in the treatment of patients suffering from rheumatoid arthritis. Indications for therapy have been defined. In 360 patients suffering from rheumatoid arthritis, radioactive colloid gold was administered to the affected joints. The therapeutic effect was slow in development. The best clinical results were recorded at month 12. Considerable improvement was noticed in 70% of the patients, improvement in 20%, and in 10% of the patients, no therapeutic effect could be recorded. Good effect was ascertained in patients with X-ray Stage I and II disease (70%).

[Intralymphatic administration of open radionuclides in complex treatment of rheumatoid arthritis].

A new method was developed for the treatment of patients with rheumatic arthritis using therapeutic effect of radioactive colloid aurum in the lower limb lymphatic vessels. Altogether 50 patients with various degrees of activity and stages of disease were treated. A positive therapeutic effect was observed in 84%. The method of endolymphatic radiotherapy with radioactive colloid aurum made it possible to give up cytostatic and glucocorticoid therapy and to reduce a dosage of nonsteroid antiinflammatory drugs.

[Functional status of the pituitary-thyroid system in children and adolescents exposed to radiation as a result of the accident at the Chernobyl Nuclear Power Station].

The paper is concerned with the results of a follow-up over the period of 1986-1989 of function of the hypophyseo-thyroid system in children and adolescents (approximately 6000 persons), residing in the South-West of the Kaluga Region, where radiation fallouts were observed after the Chernobyl accident. The results were based upon the analysis of medical examinations and determination of the blood levels of TSH, T4 and T3. A certain functional activation of the thyroid system was observed in a majority of the examinees. A conclusion has been made of a necessity of a long-term follow-up of the thyroid of persons who received radioactive iodine in childhood.

[Transvaginal ultrasonography in the assessment of local extension of rectal cancer].

To explore potentialities of transvaginal sonography in the diagnosis of rectal tumors 33 female patients were examined. Transvaginal sonography was shown to be optimum for accurate local staging of rectal tumors. The overall staging accuracy for transvaginal sonography in this study was 94%. There were one understaged and one overstaged cases. Pathological pararectal lymph nodes were detected in 57.5% of the patients.
