[Value of transrectal and transvaginal ultrasonic tomography using an electronic linear scanner in staging bladder tumors].

The results of sonographic studies of 48 patients with carcinoma of the urinary bladder are discussed. The value of longitudinal intracavitary sonography for assessment of the local extent of malignant tumors of the bladder was studied. This method is thought to be extremely useful when tumors are localized on the neck, base and front wall of the bladder, whereas transabdominal sonography gives valuable diagnostic information if the lateral and back walls are involved. Combined sonography must be used for assessment of the local extent of carcinoma of the urinary bladder as it is of high accuracy, non-invasive and safe.

[Double contrast examination of the pericardial cavity in a case of exsudative pericarditis (author's transl)].

1)Radiographic examinations of the pericardial cavity by double contrast technique yield good information about the pericardial layer itself, the pericardial cavity and its contents. -- 2) Pericardial puncture and fluid aspiration and subsequent creation of a pneumopericardium with installation of water soluble contrast material are important diagnostic tools and have an important therapeutic effect as well. -- 3) For an appropriate examination 300--400 ml gas (oxygen in inflammatory, room air in carcinomatous conditions) and 5--8 ml of 60% water soluble contrast material (0.1 ml per kg bodyweight) should be installed.

[Pharmacolymphography (author's transl)].

In 110 patients with different malignant tumors various pharmacological substances (Ridol (G. Richter, Hungary), Arphonad (Hoffmann, Switzerland), Xavin (Chinoin, Hungary) etc.) were used in lymphography to improve inflow of contrast medium and visualization of lymph vessels and nodes. Different substances with vasodilatatory effects of varying degree were tested. Ridol and the combination Atropin-Promedol-Pipolphen proved to have a specific lymphotropic affinity. Time of contrast medium injection could be shortened and visualization of the intermediary lymphnode sinus as well as thoracic duct improved by pharmacolymphography. The optimal method consists of i.m. Ridol application 10 to 15 minutes prior to lymphangiography and endolymphatic injection of the combination Atropin-Promedol-Pipolphen.

[Pelvic phlebography by injection of contrast medium into the profundal dorsal penis vein for evaluation of tumor spread in cancer of the urinary bladder and prostate (author's transl)].

1. Tumors with infiltration of the mucosa and submucosa (T1) as muscularis (T2) do not cause anatomical alterations of visceral and parietal pelvic veins, as numerous phlebographies have confirmed. Internal pudendal veins and vessels of the bladder and prostate are involved in all patients with infiltration of bladder tumors into the paravesical tissue or prostate. -- 2. Phlebographic signs for metastatic lymph node involvement in carcinoma of the urinary bladder were observed in 24.5% (T2 in 11.7%, T3 in 33.8%, T4 in 25% of cases). These lesions were absent in tumors which involved the mucosal and submucosal layers (T1) only. -- 3. Alterations of the venous plexus depend on the extent of malignant infiltration in carcinoma of the urinary bladder. These can be recognized in stage II. -- 4. Pelvic phlebography may be helpful in differentiation of carcinomas and adenomas of the prostate.

[Lymphographic interpretation of benign lymphnodes lesions (author's transl)].

Clinical, lymphographic and morphologic non tumor induced lymphnode lesions of 164 patients are described, and a roentgenographic morphological correlation is performed. Diffuse non specific inflammatory and reactivy lymphnode lesions are divided into three groups. These changes cannot be distinguished from lymphnodes with micrometastases. Typical cirteria for segmental lymphnode fibrosis and hyperplasia are described. Fatty lymphnode degeneration may be segmental or diffuse, their characteristics are presented as well.

[Differential diagnosis of neck tumors by angiography (author's transl)].

Angiographic criteria and differential dignosis of tumors of the lateral neck are demonstrated in 200 examined patients. There is a typical angiographic pattern in chemodectomas. Extreme vascularization and localization in the area of the carotid bifurcation are characteristic for these tumors. Neurinomas are avascular or minimally vascularized. The character of carotid artery deviation allows to distinguish neurinomas from other neoplasms of the neck. Metastases of thyroidal carcinoma are usually represented by pronounced vascularized tumors with a smooth border. Metastases of squamous cell carcinomas as primary mesenchymal tumors are minimally vascularized or sometimes avascular. There is increased vascularization in combination with perifocal inflammation. It is impossible to differentiate metastases of squamous cell carcinomas from primary mesenchymal tumors by angiography.
