[Antibiotic prophylaxis in orthopedic surgery].

In orthopaedics where a great number of implants are in use, the chance for infection is high. To keep up the aseptic condition and giving antibiotic in the perioperative time is necessary. According to the effect on postoperative bleeding authors did compare two second generation cephalosporin, the cefamandol and the cefuroxim, which are mainly used in antibiotic prophylaxis. The gamma carboxylation of glutaminic acid is inhibited by cefamandol in the liver, which is necessary in the prothrombin synthesis. The cefuroxim is without any effect on prothrombin synthesis. The postoperative bleeding was significantly higher following cefamandol administration. According to the authors in orthopaedics, where the blood loss could be high, that type of antibiotic prophylaxis is recommended, which has no effect on bleeding.

[Experience with the Solcotrans orthopedic autotransfusion device in hip replacement surgery].

A study on 20 patients undergoing hip prosthetic surgery is reported. Solcotrans Orthopaedic, a sterile, disposable device for the post-operative salvage and reinfusion of drained whole blood was used on all patients. 70% of patients did not require additional homologous blood and there was a saving of 76% in homologous blood usage. The device is safe and simple to use and no clinically significant adverse effects were observed. Its use is recommended in patients undergoing hip prosthetic surgery in the absence of joint infection or malignancy.

Estulic monotherapy and combined antihypertensive therapy.

The antihypertensive effect of Estulic has been examined over a 12-month period in 101 hypertensive patients. Estulic tablet proved to be effective in form of monotherapy especially in the treatment of mild hypertension, in form of combination therapy in hypertension of medium severity. Using Estulic the number and doses, of the formerly administrated drugs could be reduced. The blood pressure of the patients was stabilized on a significantly lower level, changes in the laboratory parameters were not observed. In 60% of the patients improvement of the subjective complaints was observed. On the basis of the examinations guanfacin proved to be an effective drug in the treatment of hypertension of different severity degrees due to its favourable antihypertensive action, few side-effects, and the possibility of drug administration in daily single doses.
